Klinefelter's syndrome is a genetic disorder affecting males. In Klinefelter's syndrome, the male has one or more extra X chromosomes . Males normally have one X and one Y chromosome. Males with Klinefelter's syndrome commonly have one extra X chromosome. In rare cases, some males with Klinefelter's syndrome have as many as three or four X chromosomes or one or more extra Y chromosomes. Many men with an extra X chromosome are not aware that they have it and go on to lead normal lives. Males with Klinefelter's syndrome may be described as XXY males or males with XXY syndrome. Klinefelter's syndrome occurs in about 1 in 500 to 1 in 1,000 males.

Question: I am a 34-year-old man with Klinefelter's syndrome. I'm upset that I can't have children. Is Klinefelter's syndrome hereditary? Can it be cured? No name / No state given Answer: Normally, a male has only one X and one Y chromosome and a female has two of the X chromosomes. In Klinefelter's syndrome, the male has two or more X chromosomes in addition to one Y chromosome. Klinefelter's syndrome usually isn't present in other family members. The extra genetic materials cause abnormal development of the testicles, resulting in decreased production of sperm and male sex hormones. Signs and symptoms of Klinefelter's syndrome can vary but may include:
  • Small testicles
  • Infertility
  • Long arms and legs
  • Enlarged breast tissue (gynecomastia)
  • Scant facial and body hair
  • Decreased sex drive (libido) and function
No treatment can correct this genetic abnormality. But testosterone replacement therapy can improve symptoms due to testosterone deficiency. Most men with Klinefelter's syndrome are infertile because they don't make enough sperm. But no man with Klinefelter's syndrome should assume he's infertile without further testing.

I get a shot of testosterone every 3 weeks but it does not seem to do much for my libido or anything else. I'm 65, have diabetes mellitus, and am on blood thinners (to prevent clotting in my legs) and I have poor circulation. I guess I should 'hang up my sexual hat', but I didn't find out about the XXY until I was 53, and I went through puberty at 54!
Answer Even though you found out that you have Klinefelter's syndrome 11 years ago I'm wondering just how much your physician has explained to you. Here are a few remarks on it.
There are 23 pairs of chromosomes found in each cell of the body, and each contain genes that determine our coloring, our features, and our sex. Women inherit two X chromosomes - one from each parent, written XX. Men inherit an X chromosome from their mothers and a Y chromosome from their fathers, written XY.
The X chromosome determines whether a child is going to be a male or a female. X-chromosome mutations cause various human genetic disorders, such as Turner's syndrome and Klinefelter's syndrome.

Klinefelter's is the most common sex chromosome disorder, affecting approximately 1 in 500 males across all ethnic and racial groups. Because the "extra" chromosome is a sex chromosome, most of the physical effects of Klinefelter's syndrome become apparent during puberty, if at all. Men with Klinefelter's tend to be tall and have decreased facial hair and muscle mass. They are also more likely to have breast development and very small testicles. Some of these physical differences are thought to be the result of decreased testosterone levels. Injections of synthetic testosterone have been found to increase muscle development and bone density, giving men with Klinefelter's syndrome a more traditionally masculine appearance. However, this replenishment of testosterone cannot correct the infertility that most with Klinefelter's have as a result of the condition. Some research suggests that early (recommended by ages 11 - 13 years) testosterone replacement therapy may decrease the incidence and severity of learning disabilities that can be related to having Klinefelter's, potentially improving overall quality of life. Research on the cognitive problems associated with Klinefelter's are still in the early stages though.

Klinefelter's syndrome is a condition caused by a chromosomal abnormality in males (specifically a nondisjunction ); sufferers have a pair of X chromosomes instead of just one. The XXY chromosome arrangement is a common genetic abnormality, occurring as frequently as 1 in 1,000 male births, but the symptoms that may result from having the extra chromosome is less common. Because of this extra chromosome, individuals with the abnormality are usually referred to as "XXY Males" rather than as "suffering from Klinefelter's syndrome." In mammals with more than one X chromosome, the genes on all but one X chromosome are barred from being expressed. This happens in XXY males as well as XX females. A few genes, however, have corresponding genes on the Y chromosome and are not barred. These triploid genes in XXY males may be responsible for the symptoms of Klinefelter's syndrome. In adults, the syndrome is characterised by gynecomastia (enlarged breasts), a rounded body type with abnormal body proportions, sparse facial and body hair , small testes , and an inability to produce sperm . It's quite common - about 1 in 500 boy babies have Klinefelter's and it is more likely to occur the older the mother. Typical physical features People who have Klinefelter's syndrome tend to have a distinctive physical appearance although in childhood it may go unnoticed and it is often not until puberty that these features develop. These individuals are hypogonadic males. The majority of affected individuals are of 47,XXY karyotype (82% of cases). Meiotic nondisjunction accounts for the extra chromosome, which may be of either maternal or paternal origin. Advanced maternal age and irradiation of either parent are risk factors. Klinefelter's syndrome occurs in 1 in 850 live male births, making it a common genetic disease involving the sex chromosomes and the most common cause of male hypogonadism. Hypogonadism is seen in all cases, resulting in elevated gonadotropin levels (especially FSH) and variably reduced testosterone levels. Mean plasma estradiol levels are elevated by an unknown mechanism. The ratio of oestrogens to testosterone determines the extent of femininization. Clinical symptoms are usually not evident until puberty. They include: small atrophic testes with a small penis, eunuchoid body habitus with abnormally long legs, lack of secondary male sexual characterisitics and gynecomastia. The mean IQ is usually lower than that of the general population. Patients with Klinefelter's syndrome are infertile. Azoospermia is seen related to testicular atrophy. In some patients, complete atrophy of the testicular tubules is present and they are replaced by pink, hyaline, collagenous ghosts. In others, apparently normal tubules are interspersed with atrophic tubules. In still others, all tubules are incompletely developed, consisting of cords of cells that have never developed a lumen or progressed to mature spermatogenesis. While some authors contend that Leydig cell hyperplasia is present in patients with Klinefelter's syndrome, others argue that the Leydig cell appears hyperplastic only because of atrophy and crowding of the tubules. In polysomic X individuals (e.g. 48, XXXY or 49, XXXXY), additional genital abnormalities are seen, including cryptorchidism, hypospadias, and more severe testicular hypoplasia.

Klinefelter-Reifenstein syndrome Reifenstein-Albright XXY syndrome Synonyms: Aspermatogenesis-gynaecomastia syndrome, chromosome XXY syndrome, gynaecomastia-aspermatogenesis syndrome, medullary gonadal dysgenesis, primary microorchidism, puberal seminiferous tubule failure, sclerosing tubular degeneration, seminiferous tubule dysgenesis, XXY syndrome. Associated persons: Fuller Albright Harry Fitch Klinefelter, Jun. Edward Conrad Reifenstein jun. Description: A congenital endocrine disturbance characterized chiefly by dull mentality and/or behavioural problems associated with hypogenitalism and hypogonadism. The affected children have small and firm testes, lack of sperm formation, but normal size penis, and gynecomastia (usually minimal). Long limbs and relatively slim and tall statures, but tendency towards obesity in some adults. Other features are immaturity, shyness, lack of judgement, assertive unrealistic activity. The majority of Klinefelter’s individuals have an extra female chromosome, resulting in an XXY pattern and a total of 47 chromosomes instead of 46. Other patterns of chromosomal aberration such as XXY, XXYY, and some mosaic patterns may result in the same syndrome. The most frequent type of intersexuality, one per 500-700 live male births.
