-Cotton Mather, 1724
Kidney stones or renal stones are gravel-like collections of chemicals that may appear in any area of the urinary system, from the kidney to the bladder. They may be small or large, single or multiple.
What are the causes of Renal Stones ? Of the four main types of urinary stones, calcium stones are the most common. Almost 95% of all kidney stones are clacium stones. They occur when there is too much calcium in the urine. Defective kidney function may allow too much calcium in the urine, or excessive calcium may be absorbed from the stomach and intestines. Some calcium stones are caused by an excess of a chemical called oxalate, present in many foods, which binds easily with calcium to form a stone. The risk of calcium stone formation is increased if you have certain medical diseases; for example, hyperparathyroidism and inflammatory bowel disease.
A second type of kidney stone is made of uric acid. These stones are much less common than calcium stones. Uric acid stones occur because you have too much uric acid in your urine. They might occur if you have become dehydrated; for example, during strenuous exercise on a hot day or perhaps during an illness. Uric acid stones are common in people who have gout, a disease that causes high uric acid levels in the blood.

Definition
Kidney stones are solid accumulations of material that form in the tubal system of the kidney. Kidney stones cause problems when they block the flow of urine through or out of the kidney. When the stones move along the ureter, they cause severe pain
Description
Urine is formed by the kidneys. Blood flows into the kidneys, and specialized tubes (nephrons) within the kidneys allow a certain amount of fluid from the blood, and certain substances dissolved in that fluid, to flow out of the body as urine. Sometimes, a problem causes the dissolved substances to become solid again. Tiny crystals may form in the urine, meet, and cling together to create a larger solid mass called a kidney stone. Many people do not ever find out that they have stones in their kidneys. These stones are small enough to allow the kidney to continue functioning normally, never causing any pain. These are called "silent stones." Kidney stones cause problems when they interfere with the normal flow of urine. They can block (obstruct) the flow down the tube (the ureter) that carries urine from the kidney to the bladder. The kidney is not accustomed to experiencing any pressure. When pressure builds from backed-up urine, the kidney may swell ( hydronephrosis ). If the kidney is subjected to this pressure for some time, it may cause damage to the delicate kidney structures. When the kidney stone is lodged further down the ureter, the backed-up urine may also cause the ureter to swell (hydroureter). Because the ureters are muscular tubes, the presence of a stone will make these muscular tubes spasm, causing severe pain.

What Are Kidney Stones?
Who Gets Them and Why?
Kidney stones form when a concentration of crystals from the urine builds up in the kidney. Caucasians get them more than persons of African descent; so do people with irritable bowel syndrome. Stones are more common during the summer when people sweat and their urine becomes more concentrated. The agony of a kidney stone passing through the urinary tract begins suddenly in the area between the ribs and the hip and moves toward the groin. The pain is sharp, cramping, acute, and intermittent, and may cause nausea and vomiting. Most kidney stones are passed within 48 hours. The doctor will prescribe medication for pain, bed rest, and lots of fluids during this time. The patient may be asked to urinate through a strainer so that the stone can be caught and analyzed. This article originally appeared in the Winter 2002 edition of Health News magazine.

WHAT ARE KIDNEY STONES?
The Kidneys and Urinary Tract
The urinary system helps to maintain proper water and salt balance throughout the body. The process of urination begins in the kidneys, which process fluids and dissolved waste matter to produce urine. The two kidneys are located deep behind the abdomen below the ribs and toward the middle-back. They play a major role in this process:
  • Each kidney contains over a million nephrons . These are the functional units of the kidney.
  • Each nephron is composed of a tiny tuft of blood vessels (a glomerulus ) enclosed in a funnel-like structure called a Bowman's capsule s.
  • Each glomerulus filters waste products, water, and salts out of the blood fluid (plasma) that has entered the kidney as it circulates through the body.
  • About 1% of the plasma is converted into urine. The rest is returned into circulation to prevent dehydration. Urine's primary components are acids and urea and creatine (nitrogen compounds).
  • Urine passes from the Bowman's capsule into tiny tubules that lead to large collecting tubes to the center of the kidney. As it passes through this network it becomes increasingly concentrated.

Bacteria could cause heart attacks, study finds
By Maggie Fox LONDON (Reuter) - British researchers said Tuesday they have found intriguing evidence that a common bacteria can cause heart attacks. They said men who had suffered one heart attack and who had antibodies to Chlamydia pneumoniae were four times more likely to suffer second heart attacks. Treating them for the infection lowered the risk. The findings, published in the American Heart Association journal Circulation , add to a growing body of evidence that heart attacks may sometimes be due to infection rather than genetics or lifestyle.
Here's Why, Maybe -2/25/99
``We know that antibodies seem to be linked to heart disease,'' Dr. Sandeep Gupta at St. George's Hospital Medical School in London, who led the study, said in a telephone interview. In addition, the chlamydia bacteria, which cause a chest infection and which are a close relative of a common sexually transmitted disease, have turned up in the fatty plaques that line clogged arteries. Gupta's British Heart Foundation team joined the race of >researchers trying to establish a more than circumstantial link between the bug and heart attacks. His team took 213 survivors of heart attacks and divided them into three groups according to how many chlamydia antibodies they had in their blood.

The stones can block the urinary tract or enter the bladder and are usually treated with ultrasound to pulverize them, though surgery is sometimes necessary. Some doctors suggest patients with kidney stones go on low-calcium diets, but an analysis of the dietary habits and health of nearly 100,000 female nurses aged 27 to 44 found those who consumed more calcium in their diet reduced their risk of kidney stones.

Creation Date: August 1998
Last Revision Date: August 1998
Peer Review Status: Internally Peer Reviewed Kidney stones are common. I have had only a few such patients but they have been memorable because of severe pain. About 16% of men and 8% of women will have stones in their lifetime. Among typical patient populations, four of five are men. The peak onset age is between 20 and 30. The problem often runs in families. Without treatment recurrence rates are 14% at one year, 35% at five, 52% at 10 and 75% at 20 years. Kidney stones commonly cause illness but uncommonly cause kidney failure or death. Abnormal urine from bodily chemical processes or environmental causes make salts exceed their solubility in urine. There are 4 major groups. Calcium stones, either oxalate or phosphate, comprise 85%.Some people absorb excess calcium or leech bone calcium. Other stones are uric acid (6%) in acid urine (half have gout), struvite stones (16%) formed with infection in alkaline urine (magnesium ammonium phosphate), and rarely cystine stones (1%). Calcium stone formation is usually inherited and unexplained. Too much Vitamin C can contribute. Low urinary citric acid from too much meat, bowel disease, acid retention, or excessive exercise can help precipitate stones. Causative factors are found in a third. On the average, stone formers produce a new stone every two to three years.
