If you have a relative with kidney failure, treatment by dialysis will probably be the first step, followed by a place on the national waiting list for a possible transplant. Sometimes the wait for a kidney can be a long one and in that situation, members of the family may begin to think about what they can do to help. This leaflet explains what happens when a kidney is donated by a relative. THINKING ABOUT DONATION Kidney transplantation is made possible by donation from people who have died in hospital. The majority will have died in an intensive care unit as a result of severe brain injury – a process called cadaveric donation. Unfortunately, there are never enough kidneys available to meet the needs of a growing waiting list of patients.

General Comments and Information: (this is not specific to your pet but is for background information concerning possible problems or diagnoses being considered): by Atlantic Veterinary Internal Medicine Group Chronic renal failure (CRF) is, unfortunately, a leading cause of illness in dogs and cats. This disease can be due to long-term inflammation from unknown causes or from specific diseases. The most common cause is chronic interstitial nephritis (CIN) and this has no identified cause. It can occur at any age but is more common after 10 years. Other causes or associated conditions include chronic viral disease, high blood pressure, blood clots (emboli), cancer, high thyroid hormone levels, severe shock, chronic inflammation, abnormal protein deposition (amyloid), and dehydration. This failure (insufficiency) of the kidneys results in poor ability to concentrate toxic waste products form the blood/ plasma into the urine and to conserve/eliminate water (concentrate the urine, or dilute the urine). CRF commonly results in increased risk of urinary tract infections (UTI) and vigilance for these is needed by periodic testing of urine. Many problems occur when there is kidney dysfunction. These include increased thirst and urination, loss of smell, poor appetite, high blood pressure, anemia, lethargy weight loss, protein loss in urine, problems of calcium and phosphorous metabolism, low serum potassium (muscle problems), platelet dysfunction (bleeding), gastritis and ulcers or vomiting, and occasionally brain dysfunction (uremic encephalopathy).

This definition can occasionally create confusion because some will equate kidney failure with failure to make urine. Ironically, most cats in kidney failure are actually producing large quantities of urine, but the body’s wastes are not being effectively eliminated.

Kidney Failure What is kidney failure? The kidneys are a pair of organs located just behind the stomach. They filter bacteria and extra salt and water from the body. The kidneys stop working when illness or injury keeps them from filtering properly. In kidney failure, these bacteria and salts build up and can have bad effects on the heart, brain, lungs, and other organs. This may even cause serious illness or death. How do you get kidney failure? Kidney failure can happen quickly (days) or more slowly (months or years). Many illnesses can cause kidneys to fail, including diabetes and high blood pressure. Most people with chronic kidney failure need to take medicines, and many need dialysis. Some people with kidney failure are very sick and need to be in the hospital. About 10 percent of the time, it is because of a severe allergic reaction to medicines. This is called interstitial nephritis (say: in-ter-STI-shul nef-RYE-tus).

WARNING Dog Owners
This week I had the first case in history of raisin toxicity ever seen at MedVet. My patient was a 56 pound, 5 yr old male neutered lab mix who ate half a canister of raisins ometime between 7:30 AM and 4:30 PM on Tuesday. He started with vomiting, diarrhea and shaking about 1 AM on Wednesday but the owner didn't call my emergency service until 7 AM.
1 1/2 times maintenance and watch the kidney values for the next 48-72 hours.
The dog's BUN (blood urea nitrogen level) was already at 32 (normal less than 27) and creatinine over 5 (1.9 is the high end of normal). Both are monitors of kidney function in the bloodstream. We placed an I V catheter and started the fluids. Rechecked the renal values at 5 PM and the BUN was over 40 and creatinine over 7 with no urine production after a liter of fluids.
At the point I felt the dog was in acute renal failure and sent him on to MedVet for a urinary catheter to monitor urine output overnight as well as overnight care. He started vomiting again overnight at MedVet and his renal values have continued to increase daily. He produced urine when given lasix as a diuretic. He was on 3 different anti-vomiting medications and they still couldn't control his vomiting.

More than 40 papers were presented on diabetes management, growth and development, obesity, neonatal health, immunology, gastrointestinal health, renal health, geriatric nutrition, and physical stress and nutrition.

More than 16 million Americans suffer from diabetes and the number is increasing. The number of dogs and cats with diabetes is also on the rise. Although there are obvious differences between humans and animals, Iams and leading universities have found some connections between diet and the disease that may ultimately benefit humans. Starch is the food component most likely responsible for the rise in blood glucose immediately after a meal, according to researchers. However, it has been proven that certain forms of starch, including sorghum and barley, actually help control a dog's blood sugar levels.

    Diabetes is the most common cause of kidney failure. However, it's also true that most people with diabetes don't go on to have end-stage kidney disease (ESRD). There are many things you can do to control your diabetes and minimize your risk of kidney failure including stopping smoking and keeping blood pressure under control. A federal study called the Diabetes Control and Complications Trial proved in 1993 that intensive management of diabetes can reduce the risk of kidney disease by 50 percent.
    Intensive management (called tight control) means frequent testing of blood sugar, calculating insulin intake on the basis of diet and exercise, following a diet and exercise plan, and talking often with a health care team whose members are skilled at treating diabetes. What happens?

As soon as the kidneys' filtering capacity falls much below 50 percent, however, problems begin to appear, worsening as filtering capacity drops. With special measures, such as dietary restriction and medication, however, children can survive even after kidney function has fallen to five or ten percent of normal. WHEN SHOULD I SUSPECT THAT MY CHILD HAS KIDNEY FAILURE?
Acute kidney failure is usually a complication of some other serious illness. Generally, parents first notice that the child stops urinating or urinates very infrequently. Other symptoms include body swelling, drowsiness and irregular heartbeat. The first signs of chronic kidney failure are usually loss of energy and easy fatigue. As the condition progresses, the child may lose appetite (particularly in the morning), pass less urine than usual and drink more fluids. The child's skin also may grow pale and develop a sallow, muddy appearance. Other symptoms include headaches, muscle cramps, nausea, weight loss, puffy face, bone or joint pain and bruised, dry or itchy skin.
