Many people with renal failure experience changes and difficulties in their emotional and sexual lives. People with kidney disease are not unique in this. Living with dialysis or having a transplant adds complexities to life that may cause problems for both themselves and a partner. Whatever their nature, be they physical changes in body function or adjustments in lifestyle to adapt to kidney disease, for each individual the effect of any change is different. For some the changes do not cause a problem, for others, they may create severe difficulties in the way they see themselves and in their relationships with their partner or with other people close to them. This leaflet is about the kinds of sexual and emotional difficulties people experience and the sort of help that is available. Whatever problems you may be experiencing, there will always be someone on your renal unit who will be either willing to listen and discuss these with you or knows somebody who can. Sometimes, the reassurance gained by talking about it may be all that is required. The demands of dialysis - The effect of stress The demands of a life on dialysis may be stressful. Regular dialysis, taking tablets and keeping to a diet and fluid restriction, can cause people to feel anxious and depressed. Equally, the impact that dialysis can have on work, income and plans that have been made for life ahead, can lead to emotional difficulties that affect not just the person on dialysis but their partner and family also. Some people have emotions that develop as a result of losing their old way of life and self, suffering a sort of grief. This can be expressed by a variety of emotional states. If you feel this way it is important to take the opportunity to talk to somebody. Your nurse or doctor can help you to get in touch with somebody who is qualified to help with these kinds of difficulties.

What happens in polycystic kidney disease?
Polycystic kidney disease (often shortened to PCKD or PKD ) is the commonest inherited cause of kidney failure. It may be inherited from either your mother or your father. Sometimes they will not know that they carry the disease. Not everyone with PCKD gets trouble from their kidneys.
Can anything else cause kidney cysts?
It is quite common for healthy people to have one or two cysts, and occasionally people may have several. But when the kidney is filled with many cysts, PCKD is the most likely diagnosis. There are some other rare diseases that very occasionally cause many kidney cysts. Infantile polycystic kidney disease is rare, causes liver damage as well as kidney trouble, and unfortunately often leads to affected children dying when they are young. The kidneys have a major influence on blood pressure and it is particularly important to monitor blood pressure whenever kidney disease is suspected. What trouble does it cause?
High blood pressure rarely produces any symptoms at all, unless it is very high. However people with high blood pressure have more strokes and heart attacks than people with normal blood pressures, and it may damage kidneys. How often high blood pressure damages normal kidneys is uncertain, but it is probably quite rare compared to the heart disease and strokes that it causes. Kidney damage caused by blood pressure seems to be more common in some races, particularly in Black people.

Issues and Trends
Chronic kidney failure is the most significant result of chronic kidney disease. When kidney function has deteriorated and is no longer adequate to sustain life and the process is considered irreversible, renal replacement therapy (RRT)—dialysis or transplantation—becomes necessary to maintain life. Treated chronic kidney failure, also called end-stage renal disease (ESRD), is the most feared consequence of kidney disease. Chronic renal insufficiency, however, is more common than treated chronic kidney failure and can also severely affect health and well-being. Therefore, ideally, programs should be directed at preventing the development of chronic renal insufficiency and its subsequent progression to ESRD.
Unfortunately, chronic renal insufficiency is usually asymptomatic, and the exact number of people affected is unknown. The best available estimates are based on national surveys. Current estimates indicate approximately 10 million persons aged 12 years and older have some form of chronic kidney disease.

Polycystic kidney disease (PKD) is one of the most common of all life-threatening human genetic disorders. It is an incurable genetic disorder characterized by the formation of fluid-filled cysts in the kidneys of affected individuals. These cysts multiply over time. It was originally believed that the cysts eventually caused kidney failure by crowding out the healthy kidney tissue. It is now thought that the kidney damage seen in PKD is actually the result of the body's immune system. The immune system, in its attempts to rid the kidney of the cysts, instead progressively destroys the formerly healthy kidney tissue. Description A healthy kidney is about the same size as a human fist. PKD cysts, which can be as small as the head of a pin or as large as a grapefruit, can expand the kidneys until each one is bigger than a football and weighs as much as 38lb (17 kg). There are two types of PKD: infantile PKD, which generally shows symptoms prior to birth; and adult onset PKD. Individuals affected with infantile PKD are often stillborn. Among the liveborn individuals affected with infantile PKD, very few of these children survive to the age of two. The adult onset form of PKD is much more common. The time and degree of symptom onset in the adult form of PKD can vary widely, even within a single family with two or more affected individuals. Symptoms of this form of PKD usually start to appear between the ages of 20 and 50. Organ deterioration progresses more slowly in adult onset PKD than it does in the infantile form; but, if left untreated, adult onset PKD also eventually leads to kidney failure.

Kidney disease, in the form of chronic renal failure, is a common problem in older cats. I have seen kidney failure in cats as young as 4 years, but more frequently in much older cats. The most noticeable symptom is an increase in water consumption and urination ("drink-a-lot, pee-a-lot syndrome"). A blood test should be done if you notice these symptoms, as there are several conditions that can cause this. The increase in drinking and urinating is due to loss of the kidney's ability to concentrate the urine. The kidneys have a very large reserve capacity, and symptoms of failure are not seen until approximately 75% of kidney tissue is nonfunctional. In my experience, kidney failure is the most common cause of death in older cats. Chronic renal failure is progressive and incurable. No conventional or alternative medical treatment can reverse its course, since the disease involves the loss of kidney cells and replacement by scar tissue. The rate of progression in any individual cat probably cannot be slowed to any significant degree. When the process is advanced, the kidneys become small and lumpy, and the amount of functional tissue is greatly limited. The most significant problems caused by the loss of function are build-up of blood toxins, and anemia. These can cause weight loss, lethargy, vomiting, loss of appetite, weakness, and other signs of illness. Some cats are able to maintain their body weight and live relatively comfortable lives for months to years, while others succumb to the disease more quickly. In conventional medicine, there are drugs that can minimize anemia, and phosphate binders to prevent phosphorus precipitates from further damaging the kidneys. These may not be palatable, and may cause adverse reactions. It may also be important in some cases to supplement potassium in the food. Some of the best and simplest treatments include:
