Recessive Polycystic Kidney Disease (RPKD)
This condition is inherited in an autosomal recessive pattern, giving a 25% recurrence risk for parents having subsequent children. The kidneys are affected bilaterally, so that in utero, there is typically oligohydramnios because of poor renal function and failure to form significant amounts of fetal urine. The most significant result from oligohydramnios is pulmonary hypoplasia, so that newborns do not have sufficient lung capacity to survive, irrespective of any attempt to treat renal failure. RPKD may be termed "Type I" cystic disease in the Potter's classification. Grossly, the kidneys are markedly enlarged and tend to fill the retroperitoneum and displace abdominal contents. The kidneys tend to be symmetrically enlarged. The cysts are quite small and uniform, perhaps 1 to 2 mm on average. Microscopically, the characteristic finding in the later third trimester is cystic change with the cysts elongated and radially arranged. The few remaining glomeruli are not involved by the cysts, and the intervening parenchyma is not increased. In the second trimester, the cysts may not be as well-developed. A helpful finding at autopsy is the presence of congenital hepatic fibrosis, which accompanies RPKD.

    Chapter 18) idney disease is a major health problem in this country, afflicting some eight million Americans. Kidney and urinary tract diseases together affect an estimated 20 million people, causing more than 95,000 deaths a year and contributing to an additional quarter of a million. Kidney disorders run the gamut from minor infections to total kidney failure. Kidney disease can cause high blood pressure, anemia, and elevated cholesterol. When chronic, it can lead to depression and sexual dysfunction. Kidney stones, diagnosed in more than one million Americans annually, can be extremely painful and are a significant cause of hospital stays and lost work days. But the picture is not entirely bleak. Thanks to major medical advances, diagnosis and treatment of kidney problems have improved significantly in the past 30 years. Even people with complete kidney failure can now lead reasonably normal lives because of modern dialysis techniques and new successes in transplantation. Today dialysis keeps alive more than 120,000 Americans who would otherwise perish because of kidney failure. Kidney transplants, first performed in the U.S. some 30 years ago, have saved the lives of thousands more.

    Is your cat drinking a lot of water? Does it visit the litter pan a bit too often? In 1978, my three year old white Persian stud began exhibiting these symptoms one day. Each time I passed the bathroom, there he was in the litter pan. After the third time past the door, with him back in the litter pan, I immediately thought, renal blockage. Whipped him up and over to the vet. That was not the problem. He was simply urinating too frequently. So frequently in fact that the vet called it "Polyuria." There was no blockage, no crystal formations in the bladder. He was simply urinating frequently, to the extent that within six hours, he had become dehydrated. The vet pumped him full of fluids, peritoneally(through the abdominal wall) He did x-rays to look for tumors. He then gave me the bad news. The cat likely had a progressive kidney disease that they didnt know much about, called Polycystic Kidney Disease. The cats would experience episodes of polyuria over a one year period before succumbing to the disease. Upon autopsy, they would find lesions on the kidneys, the cause was not known, but the disease was progressive, and the diagnosis: incurable. However, my cat could go home, live a happy, but short life, with increasingly frequent episodes, before he would eventually die. Supportively he received renal tablets(Renaseptic), and of course, some antibiotics. Apollo was fine for six months. I did not stress him with breedings. In fact, I was in the middle of moving and had placed nearly all of the cats, except Apollo and my two folds.

    Dubuque, IA Lupus nephritis or lupus glomerulonephritis is the medical term for the kidney disease which occurs in systemic lupus erythematosus (SLE). It is estimated that about a third of patients with lupus will develop nephritis which requires medical evaluation and treatment. Lupus nephritis is an important and potentially serious symptom of lupus.
    CLINICAL COURSE OF LUPUS NEPHRITIS
    There are very few signs or symptoms of lupus nephritis. It does not produce pain in the abdomen or back, nor does it cause pain or burning during urination. The loss of protein in the urine from lupus nephritis may lead to fluid retention with weight gain and swelling (edema). This may result in puffiness in the legs, ankles and/or fingers. This puffiness is often the first symptom of lupus nephritis noted by the patient. The clinical course of lupus nephritis is highly variable. Often findings of lupus nephritis are evident only in urine studies. In many patients, the urine abnormalities are very mild and may be present on one examination and absent the next. This form of lupus nephritis is rather common and, generally does not require any special medical evaluation or treatment. However, in some patients, the abnormal findings on urine studies persist or may even worsen over time. Patients with this type of lupus nephritis are at risk for loss of kidney function. They may require additional studies to evaluate the extent of their lupus nephritis and to determine the best treatment approach to control their disease.

    The kidneys play a critical role in overall health. Acting like a water filter that separates out harmful minerals and chemicals, the kidneys process about 200 quarts of blood in an adult every day to produce about 1 to 2 quarts of urine, which is made up of waste products and extra water. Urine leaves the kidneys (almost all people are born with two) through tubes called ureters , which empty into the bladder. Without the kidneys, waste products would build up in the blood and damage the body. The filtration process occurs in microscopic units inside the kidneys called nephrons ; each kidney has about a million nephrons. As they remove waste products and extra water, the nephrons return chemicals the body needs (such as sodium, phosphorus, and potassium) to the blood. Each chemical is vital to overall health and must be in proper balance. Too much or too little can be harmful. The kidneys also produce three important hormones: erythropoietin (uh-rith-ro- poy -uh-tin ), which stimulates the bone marrow to make red blood cells;
