Kernicterus
Damage to the brain from high bilirubin levels in the blood. When bilirubin finds its way into brain cells it paralyzes the energy producing mechanisms causing the cells to die. Premature infants are particularly prone to kernicterus. Excess bilirubin produces signs of jaundice and high levels of bilirubin in the first few days of life. The baby also becomes progressively inactive, sometimes adopting a characteristic posture with arched back and neck. Without treatment, the baby is likely to die at the end of the first week of life. Kernicterus from jaundice

We were told that our son was jaundiced when we left the hospital and not to worry about a thing and follow up with our doctor in 2 weeks. His bilirubin levels were not tested before leaving the hospital. When we saw our family doctor when our son was 13 days old, we thought his jaundice was going away. His bilirubin level at 13 days was 29. We were told to go to NICU for phototherapy. The cause of our son's jaundice was breastmilk jaundice.
At our 4 month check, our new pediatrician had concerns about our son's development. He's now 14 months old, has been diagnosed with kernicterus - damage resulting to the basal ganglia as a result of hyperbilirubinemia. He has athetoid cerebral palsy with fluctuating tone, as well as possible hearing problems and strabismus (eyes aren't working well together).
This could have been prevented with a simple blood test before leaving the hospital and with being told to see our doctor within 1-3 days following discharge from the hospital. If the phototherapy would have been started sooner, I'm positive that our son would not be having the challenges that he is facing. We now know that large, breastfed boys should definitely have their bilirubin levels tested if they look jaundiced...I wish we knew then what we know now...

Kernicterus: Athetoid Cerebral Palsy, 6/2003

Athetoid cerebral palsy is a form of CP that is characterized by uncontrolled movements of the trunk of the body, arms, and/or legs. In years past, it was often due to a large increase in the amount of a body chemical—bilirubin—in the blood of the newborn infant. This increase results in yellowing of the infant’s skin called jaundice. It also injures essential parts of the brain controlling the coordination of movements, the basal ganglia and the cerebellum. The brain injury is called Kernicterus and was a major cause of athetoid cerebral palsy.
There are a number of reasons for an increase in blood bilirubin in the newborn infant; among these are a genetic factor preventing adequate bilirubin clearance from the infant’s blood; the effect of certain medications; an infection causing increased destruction of infant red blood cells (a product of red blood cell destruction is bilirubin) and Rh factor blood incompatibility between the mother and the infant resulting in increased infant red blood cell destruction. In the past, Rh blood factor incompatibility was a major cause of Kernicterus.
With increased knowledge about blood types and Rh factors, improved methods of prevention were discovered. Also, methods for early detection of increased bilirubin and treatment were developed and applied; this later included photo (light) therapy and when necessary, an exchange blood transfusion. With control of the Rh factor incompatibility, the occurrence of new cases of Kernicterus almost disappeared and with that, the occurrence of athetoid CP diminished.

ABBREVIATIONS. AAP, American Academy of Pediatrics; G-6-PD, glucose-6-phosphate dehydrogenase. OBJECTIVE The American Academy of Pediatrics (AAP) Subcommittee on Hyperbilirubinemia is currently revising the practice parameter (guidelines) on neonatal hyperbilirubinemia published in October 1994).[1] Although this revision is in progress, the Subcommittee wishes to bring the issue of kernicterus to the attention of the pediatric community and provide additional information pending a more formal assessment of the literature and an analysis of the risks and benefits of new approaches to the jaundiced infant. The Joint Commission on Accreditation of Healthcare Organizations has already issued an alert on this subject.[2] BACKGROUND Kernicterus, or bilirubin encephalopathy, is a condition caused by bilirubin toxicity to the basal ganglia and various brainstem nuclei. In the acute phase, severely jaundiced infants become lethargic, hypotonic and suck poorly. If the hyperbilirubinemia is not treated, the infant becomes hypertonic and may develop a fever and a high,pitched cry. The hypertonia is manifested by backward arching of the neck (retrocollis) and trunk (opisthotonus). Surviving infants usually develop a severe form of athetoid cerebral palsy, hearing loss, dental dysplasia, paralysis of upward gaze and, less often, intellectual and other handicaps.

In this report, we describe 5 instances of unexpected kernicterus in sick very low birth weight (VLBW) infants. The condition is probably underreported. Pallidal injury was picked up with ultrasound first in 1 infant. The diagnosis is facilitated by the use of magnetic resonance imaging (MRI). The history and (serial) MR behavior of globus pallidus of 3 term infants with kernicterus is added. The diagnostic challenge of preterm kernicterus is reported in detail, focusing on the acute context. Case Reports Patient 1 Patients 2-5 (Preterm Infants) Patients 6-8 (Term Infants) [FIGURE 5 OMITTED] Patient 7 was born vaginally at 37 weeks of gestation, birth weight 3950 g, Apgar scores 9 and 10 at 1 and 5 minutes, respectively. After discharge home within 24 hours, she was readmitted on day 6 with decreased diuresis and refusal of breastfeeding. Clinically jaundiced from day 2, the first estimated TSB level on admission was 776/[micro]mol/L. She had 1 exchange transfusion and phototherapy. Blood group incompatibility was excluded. Prateus mirabilis bacteremia and urinary infection were diagnosed. Epileptiform changes were seen on EEG in the acute stage. MRI (Fig 6) demonstrated T1 hypersignal in globus pallidus on days 6 and 14, with mildly increased T2-signal intensity. Pallidal echoreflections appeared normal. At 3 months, T1-signal intensity was normal, whereas T2 signals had increased in globus pallidus. Hearing loss was confirmed with ABR. The infant was by then hypotonic with repetitive tongue thrusting.

    Definition
    Rh incompatibility is a condition which develops when there is a difference in Rh blood type between that of the pregnant mother (Rh negative) and that of the fetus (Rh positive).
    Causes and Risk
    During pregnancy, red blood cells from the fetus can get into the mother's bloodstream as she nourishes her child through the placenta. If the mother is Rh negative, her system cannot tolerate the presence of Rh positive red blood cells. In such cases, the mother's immune system treats the Rh positive fetal cells as if they were a foreign substance and makes antibodies against the fetal blood cells. These anti-Rh antibodies may cross the placenta into the fetus, where they destroy the fetus' circulating red blood cells. First-born infants are often not affected (unless the mother has had previous miscarriages/abortions, which could have sensitized her system) as it takes time for the mother to develop antibodies against the fetal blood. However, second children who are also Rh-positive may be harmed.
