Juvenile Rheumatoid Arthritis
What is juvenile rheumatoid arthritis? Juvenile rheumatoid arthritis (JRA) is a form of arthritis in children ages 16 or younger that causes inflammation and stiffness of joints for more than six weeks. Unlike adult rheumatoid arthritis, which is chronic and lasts a lifetime, children often experience periods of remission of juvenile rheumatoid arthritis. However, the disease can affect bone development in the growing child.
The disease can be classified into three categories based on the number of and which joints are involved, the symptoms present and their duration, and the presence of specific antibodies produced by the immune system. These three variables often help physicians determine the progression of JRA and include the following:
  • pauciarticular - This is a common form of juvenile rheumatoid arthritis, involving four or less joints. Large joints, such as the knees, are typically affected. This type of disease most often affects girls younger than 8 years old.

    Juvenile rheumatoid arthritis (JRA) is a disease that affects about 1 in 1,000 children aged 16 and younger, resulting in inflamed, swollen joints that are often stiff and painful. The cause of this disease is considered to be autoimmune in nature but is otherwise poorly understood. Unlike adults with rheumatoid arthritis , most children with JRA do not have long-term disease and disability, and go on to lead healthy adult lives. To reflect this, an increasing number of international experts are now referring to JRA as juvenile idiopathic arthritis (JIA); idiopathic means "of unknown cause." As the international terminology transitions into use, you may hear different terms used to describe each type of childhood arthritis. For clarification of the new international "juvenile idiopathic arthritis" classification, as compared with the American "juvenile rheumatoid arthritis" and the European "juvenile chronic arthritis," see classification There are three types of juvenile rheumatoid arthritis.     WHAT IS JUVENILE RHEUMATOID ARTHRITIS?
    Juvenile rheumatoid arthritis (JRA) is a chronic condition that causes joint swelling and stiffness for more than six consecutive weeks in a child 16 years of age or less. About 285,000 children in the United States have juvenile arthritis, and about 100,000 have juvenile rheumatoid arthritis. There are three types of JRA:
    • Pauciarticular Polyarticular Systemic
    WHAT CAUSES JRA?
    The cause of JRA is unknown. It is a disease in which the immune system begins to attack healthy cells and tissues. It is possible that the child's genetic makeup contributes to a tendency for developing JRA, and then an environmental factor (such as an infection or virus) triggers the onset of JRA.
    WHO IS AT RISK?
    Children can develop JRA as early as infancy, but it occurs most frequently in the toddler to teen years. Girls develop JRA more frequently than boys.
    HOW IS IT DIAGNOSED?
    There is no single test that can be used to diagnose JRA. A doctor diagnoses JRA by examining the child and considering the child's medical history. Doctors usually suspect JRA, along with other possible conditions, when they see a child with persistent pain or joint swelling, unexplained rashes and fever, swelling of the lymph nodes or inflammation of internal organs. Blood tests can be used to rule out other causes.

    What Is Arthritis?
    Arthritis means joint inflammation, and refers to a group of diseases that cause pain, swelling, stiffness and loss of motion in the joints. "Arthritis" is often used as a more general term to refer to the more than 100 rheumatic diseases that may affect the joints but can also cause pain, swelling, and stiffness in other supporting structures of the body such as muscles, tendons, ligaments, and bones. Some rheumatic diseases can affect other parts of the body, including various internal organs. Children can develop almost all types of arthritis that affect adults, but the most common type of arthritis that affects children is juvenile rheumatoid arthritis.
    What Is Juvenile Rheumatoid Arthritis?
    Juvenile rheumatoid arthritis (JRA) is arthritis that causes joint inflammation and stiffness for more than 6 weeks in a child of 16 years of age or less. Inflammation causes redness, swelling, warmth, and soreness in the joints, although many children with JRA do not complain of joint pain. Any joint can be affected and inflammation may limit the mobility of affected joints. Doctors classify three kinds of JRA by the number of joints involved, the symptoms, and the presence or absence of certain antibodies in the blood. (Antibodies are special proteins made by the immune system.) These classifications help the doctor determine how the disease will progress.

    History
    This 31 year old woman presented with a five month history of a rash involving her medial upper arms. The patient presented to the Charles C. Harris Skin and Cancer Pavilion in October, 2000, with asymptomatic lesions on her arms of five months duration. She had been evaluated by a rheumatologist and a dermatologist who performed skin biopsies and laboratory studies. On review of systems, she reported morning stiffness in her hands, elbows, and knees, with improvement after 30 minutes. She also reported a ten pound weight loss since the birth of her son in April, 2000. She denied fever, malaise, headaches, visual difficulties, or other problems. She was treated with plaquenil 200 mg. twice daily. At a one-month follow-up examination, the lesions resolved although her joint symptoms persisted.

It has been a very rewarding experience to participate in the progress that has been made in the treatment of these children and the resultant improvement in function. The emergence of the discipline of pediatric rheumatology (first sub-speciality certification awarded in 1992) has been associated with an increased understanding of some of the factors that regulate the inflammatory process leading to the use of a wider variety of therapies. The combination of earlier, more aggressive intervention with more informative diagnostic tools (serologic and radiologic) has resulted in better patient outcomes.     Juvenile arthritis is a rheumatic disease of childhood characterized by chronic inflammation of the connective tissue membranes that line the spaces between certain bones and moveable joints (synovial membranes). Individuals with the disorder typically experience inflammation (arthritis), swelling, and pain of affected joints. In severe cases, destructive changes of the synovial membranes and associated joint structures (articular structures) may eventually result in impaired development, limited mobility, and possible deformity of affected joints.
