Juvenile Rheumatoid Arthritis What is Juvenile Rheumatoid Arthritis? Juvenile rheumatoid arthritis (JRA) is arthritis that causes joint inflammation and stiffness for more than 6 weeks in a child of 16 years of age or less. Inflammation causes redness, swelling, warmth, and soreness in the joints, although many children with JRA do not complain of joint pain. What is juvenile rheumatoid arthritis?
Juvenile rheumatoid arthritis (JRA) is the most common form of arthritis in children. As many as half the children with this condition have a mild form and have few problems. However, some forms of this disease can produce long-term problems. There are three types of JRA:
  • Pauciarticular JRA affects 4 or fewer joints and may cause inflammation of the eye. The joints most likely to be affected are the knees, elbows, wrists, or ankles. This is the most common type of JRA. Half of the children with JRA have this type. Polyarticular JRA affects many joints (5 or more). About 30% of children with JRA have this type. The smaller joints like those in the hands and feet may be affected as well as large joints.

Juvenile Rheumatoid Arthritis
What is Juvenile Rheumatoid Arthritis? There are several types of Juvenile Rheumatoid Arthritis (JRA). Some of the children have arthritis ranging from very mild limited joint involvement to severe systemic disease associated with high fever, rash, and lymph node enlargement. We will explain the different types of JRA in more detail. Briefly, the three major subtypes include: systemic, polyarticular and pauciarticular. In other countries, these diseases may be called Juvenile Arthritis or Juvenile Chronic Arthritis which omits the "rheumatoid." The name of the diagnosis could change again in a few years as the pediatric rheumatologists around the world discuss and learn more about the disease.   • Juvenile rheumatoid arthritis is also known as JRA. It is arthritis that affects children age 16 or younger. In JRA, the body's immune system starts to attack instead of protect the body's tissues. The attack can cause redness, swelling, and pain in the joints. There are three main types of JRA. They include pauciarticular, polyarticular, and systemic. Ask your doctor.
What causes it?
  • The cause is not known. It may be due to both genetic factors (from parents) and environmental factors (for example, from infections).
Who can get it?
  • Children age 16 and under can get JRA. It is more common in females.
What are the signs and symptoms?
Symptoms of JRA usually show up around the age of 4 or 5. To be diagnosed for JRA, symptoms must last at least 6 weeks. They may include:

There are two distinct types in children: (1) Seropositive (adult) type which accounts for 15% of the cases in children, has a female predominance, the age of onset is less than 10 years old, it involves the hands predominantly, then the wrist, feet, knees, hips. (2) Stills disease is seronegative, affects the knees, wrists, carpus, ankles, tarsus, cervical spine and has 3 variants (systemic, polyarticular, monarticular or pauciarticular). Etiology/Pathophysiology:
Is a connective tissue disease with a predilection for joint involvement. Pathologically is the same as adult rheumatoid arthritis but they differ in clinical presentation, joint involvement and radiographic findings. Distribution is: knee (90% ), ankle (70% ), wrist (70% ), elbow (40% ), hip (35% ), and shoulder. Pathology:
Not applicable Imaging Findings:
In legs symmetric bilateral periarticular osteopenia involving the epiphyses and metaphyses is seen with overgrowth of the distal femoral and proximal tibial epiphyses, expansion of the intercondylar notch, and epiphyseal erosion with decreased joint space. In the hands periarticular osteopenia is seen around the metatarsal phalangeal and interphalangeal joints with swelling around the wrists and joints of fingers, accelerated skeletal maturation, erosive changes in the carpal bones, distal radius, and ulna with joint space narrowing. The spine is also involved in up to 70% of patients and you see soft tissue swelling, destruction and obliteration of articular surfaces and joint spaces, generalized rarefaction of bone, and focal subchondral necrosis. Affects the true joints between the articular facets the most, and does not affect synarthroses like the intervertebral discs. Diffuse ankylosis of posterior articular joints without disc space abnormalities is diagnostic of JRA.

Juvenile rheumatoid arthritis (JRA) is defined as causing joint inflammation and stiffness for more than six weeks in children under 16 years of age. It is estimated that between 30,000 and 50,000 children in the United States have JRA, making it the most common rheumatic disorder among this age group. Although the causes of JRA are not well understood and no cure exists, medications are available to reduce swelling and pain. Rheumatoid arthritis (RA) involves inflammation of the lining of many different joints in your body. In some people, RA can also affect other parts of the body, including the blood, the lungs, and the heart. Inflammation of the joint lining, called the synovium , can cause pain, stiffness, swelling, warmth, and redness. The affected joint may also lose its shape, resulting in loss of normal movement. What is JRA? JRA is rheumatoid arthritis in children. Like in adults, the disease is due to an auto-immune disorder. Unlike in adults, the clinical pattern can differ markedly. What are the symptoms? JRA can present in several different ways.
  • Pauciarticular arthritis. In these children, four or fewer joints are involved. The commonest joints involved are the knee and ankle. In 50% of cases, only one joint, commonly the knee is involved. The pain and swelling of the joint comes on gradually in a child typically 2 to 4 years of age. Girls are more frequently involved than boys. The child has no fever, is not sick, and has no other generalized problems.
  • Polyarthritis with minimal systemic problems. The child could be very young, from 1 to 3 years of age, or older, from 8 to 10 years of age. There is no sex predilection in the younger age group, but a definite predilection for girls in the older age group. Five or more joints are involved, which could be any joint in the body. These children have only mild symptoms of low grade fever, malaise, and mild anemia.
  • Polyarthritis with systemic problems. These children acutely with high fever and red swollen joints that mimic infection. The spleen and liver are enlarged, a skin rash is common, and there may be subcutaneous nodules over the elbow and leg. The heart may be enlarged, with inflammation of the pericardium or covering of the heart (called pericarditis).

    • The inflammation in the joints causes pain, stiffness, and swelling as well as many other symptoms.
      The inflammation often affects other organs and systems of the body as well. If the inflammation is not slowed down or stopped, it eventually destroys the affected joints and other tissues.
    Juvenile rheumatoid arthritis (JRA), or juvenile arthritis, is not a single disease, but a group of diseases. What they all have in common is chronic joint inflammation. Besides this common feature, these diseases are very different in their symptoms, their treatments, and their outcomes.
    • Pauciarticular disease affects only a few joints, fewer than 5. The large joints, such as the shoulder, elbow, hip, and knee, are most likely to be affected. This type is most common in children younger than 8 years. Children who develop this disease have a 20-30% chance of developing inflammatory eye problems and need frequent eye examinations. Children who develop this disease when older than 8 years have a higher-than-normal risk of developing an adult form of arthritis. About 50% of all children with JRA have this type.

Credits Juvenile Rheumatoid Arthritis Topic Overview Juvenile rheumatoid arthritis (JRA) is a disease that affects about 1 in 1,000 children aged 16 and younger, resulting in inflamed, swollen joints that are often stiff and painful. The cause of this disease is considered to be autoimmune in nature but is otherwise poorly understood. Unlike adults with rheumatoid arthritis , most children with JRA do not have long-term disease and disability, and go on to lead healthy adult lives. To reflect this, an increasing number of international experts are now referring to JRA as juvenile idiopathic arthritis (JIA); idiopathic means "of unknown cause." As the international terminology transitions into use, you may hear different terms used to describe each type of childhood arthritis. For clarification of the new international "juvenile idiopathic arthritis" classification, as compared with the American "juvenile rheumatoid arthritis" and the European "juvenile chronic arthritis," see classification There are three types of juvenile rheumatoid arthritis. Day-to-Day To the Arthritis Center Juvenile rheumatoid arthritis (JRA) is the most common type of arthritis affecting children. Rheumatoid arthritis is a chronic (long-lasting) disease that can affect joints in any part of the body. In this disease, the immune system mistakenly targets the tissue that lines the inside of the joint, called the synovium. The synovium responds by making excess fluid (synovial fluid), which leads to swelling, pain and stiffness. The inflammation can then spread to the surrounding tissues, eventually damaging cartilage and bone. Other areas of the body including the eyes, kidneys, lungs and heart also may be affected by the inflammation. Without treatment, JRA can interfere with a child's normal growth and development. There are three main subtypes of JRA, which are based on symptoms and the number of joints involved.
