Publication Date: July 2001 Questions and Answers About Juvenile Rheumatoid Arthritis What Is Arthritis? Arthritis means "joint inflammation" and refers to a group of diseases that cause pain, swelling, stiffness, and loss of motion in the joints. "Arthritis" is often used as a more general term to refer to the more than 100 rheumatic diseases that may affect the joints but can also cause pain, swelling, and stiffness in other supporting structures of the body such as muscles, tendons, ligaments, and bones. Some rheumatic diseases can affect other parts of the body, including various internal organs. Children can develop almost all types of arthritis that affect adults, but the most common type that affects children is juvenile rheumatoid arthritis (JRA). What Is Juvenile Rheumatoid Arthritis?

The most prevalent form of juvenile arthritis is juvenile rheumatoid arthritis, or JRA, affecting approximately 75,000 children in the United States.

It may begin with a swollen knuckle, a spiking fever, or an unexplainable rash. But no matter what symptoms appear, hearing the word "arthritis" used as a diagnosis for your child can be unexpected and confusing. Arthritis is an inflammation of the joints that is characterized by swelling, heat, and pain . Arthritis can be short-term or chronic. In rare cases, it can last a lifetime. Juvenile arthritis , or JA , is the term used for all the types of arthritis that affect children. Although we often associate this disorder with the elderly, many forms of arthritis afflict more than 285,000 children in the United States. The most prevalent form of juvenile arthritis is juvenile rheumatoid arthritis , or JRA , affecting approximately 75,000 children in the United States. Read the article below to learn about the symptoms, signs, and treatment of juvenile rheumatoid arthritis in children. What Causes Juvenile Rheumatoid (Idiopathic) Arthritis?
The medical community doesn't know exactly what causes rheumatoid arthritis (also referred to as idiopathic arthritis) in children. Research indicates that it is an autoimmune disease. In autoimmune diseases , white blood cells lose the ability to tell the difference between the body's own healthy cells and tissues and harmful invaders like bacteria and viruses . The immune system, which is supposed to protect the body from these harmful invaders, instead releases chemicals that can damage healthy tissues and cause inflammation and pain.

about Juvenile Rheumatoid Arthritis~ Our daughter Anna was diagnosed with a mild case of JRA in July 1999. Anna has pauci-articular JRA, which means that she has four or fewer joints affected. For Anna, the JRA is limited to her left knee.
As Anna's Mom, I have gone through all kinds of emotions since she was diagnosed (even with a mild case!). I have spent hours at the computer researching every page I could find with some sort of information on JRA. That's why I wanted to do this page. I want to include many links to the varied sites available to make research less time-consuming for other parents and families. Over four years later (2003), I can tell you that life DOES go back to almost normalwith just a few extra things to think about and take into consideration. Things don't knock me off balance like they used to, but that's also in part to getting some of my own health issues taken care of (so parentsmake sure you take care of yourself, too!)
For any parent whose child has been diagnosed with a chronic illness, I think it is crucial for you to surround yourself with friends and family who will listen to you and encourage you and keep you from feeling totally overwhelmed by everything. For me, it was my quest for information which helped me feel a little more in control, and my friends and family who helped me to regain my emotional balance. I found a message board where other JRA moms (and a few dads occasionally) talk and ask questions and share information and give advice. It has been an additional great system of support, hope, and encouragement.

Do you know a girl at school who says her joints seem swollen in the morning? Maybe you've met a boy who says he can hardly move because his body feels so stiff early in the day. But then those same kids will run around in gym class a few hours later with everybody else. They may tell you they have juvenile rheumatoid arthritis, or JRA for short. Maybe your reaction is, "Arthritis is something only old people get. How can a kid have it?" Read on to discover more about this disease and how it affects kids under age 16. What Is Juvenile Rheumatoid Arthritis?
Arthritis
(say: ar- thry -tus) is a disease that causes swelling and pain in a person's joints. Joints are the places where bones meet. Arthritis can also prevent people from moving their joints properly. Juvenile rheumatoid arthritis (say: joo -vuh-nyl roo -muh-toid ar- thry -tus) is a disease that affects kids under age 16. These kids have arthritis in one or more joints and swelling and pain that has lasted for at least 6 weeks. Although it has a similar name, juvenile rheumatoid arthritis is not the same disease as rheumatoid arthritis, a disease that affects adults. JRA has different symptoms and doesn't damage bones as quickly as the rheumatoid arthritis that affects older people.

The internet has been a valuable source of information and support for me, so I wanted to create a website that might in some ways be helpful to others. If you know a child who has Juvenile Rheumatoid Arthritis (JRA) or if you want to learn more about this condition, you may find it interesting to read about our personal experience. I'm not a medical professional ... and I don't play one on the web ... but I felt that others might benefit from the sort of information that I can share. You can also get information here about a popular email-based support and information group for people involved with Juvenile Arthritis, called The JRA List
At the very least, I hope that my collection of related internet links may be useful to you in helping to sort through the millions of arthritis-related websites in existence, while you search for more information and the answers to your questions about JRA. One of the main reasons for putting together this website is my personal commitment to helping spread awareness of Juvenile Rheumatoid Arthritis (JRA). Until 1995, I didn't even know what those initials stood for. Unfortunately, most people still don't. Despite increasing numbers of cases, JRA has still not received a lot of public recognition. For many, it remains a more personal struggle. Hopefully, this will change. When there's greater public awareness of a significant health problem it's more likely there will be funding made available for its' study. With continued research comes the development of new, safer, and more effective medications. It is my hope that one day ... in the not so distant future ... a cure will be discovered. Possibly even a vaccine that will help prevent JRA in future generations of children.

What Is It? If your question is not answered here, please contact one of our information specialists. JRA is the most common form of arthritis in children. It may be a mild condition that causes few problems over time, but it can be much more persistent and cause joint and tissue damage in other children. JRA can produce serious complications in more severe cases. Arthritis is best described by four major changes in the joints that may develop. The most common features of JRA are: joint inflammation, joint contracture (stiff, bent joint), joint damage and/or alteration or change in growth. Other symptoms include joint stiffness following rest or decreased activity level (also referred to morning stiffness or gelling), and weakness in muscles and other soft tissues around involved joints. However, because JRA affects each child differently, your child may not experience all of these changes. Children also vary in the degree to which they are affected by any particular symptom.

Juvenile rheumatoid arthritis (JRA) is arthritis that causes joint inflammation and stiffness for more than 6 weeks in a child of 16 years of age or less. Inflammation causes redness, swelling, warmth, and soreness in the joints, although many children with JRA do not complain of joint pain. Any joint can be affected and inflammation may limit the mobility of affected joints.

New Drug Therapy for Juvenile Rheumatoid Arthritis
It's hard to believe that not long ago, a seven-year-old Wisconsin girl was in such constant pain that she used a tricycle to move between classrooms at school, she could not walk across the street or even use coloring books. Once ravaged by juvenile rheumatoid arthritis (JRA), the girl now rides a bicycle, walks to the park and back on her own, rough-houses with her older brother, and even runs and skips like a little dervish. It's all thanks to a new drug therapy she received under the care of James Nocton, MD , assistant professor of pediatrics at the Medical College of Wisconsin. Dr. Nocton is based at Children's Hospital of Wisconsin, an affiliate of MCW. The little girl was one of the first 50 juvenile rheumatoid arthritis patients in the country to try Enbrel, a genetically engineered human protein which seems to halt progress of the disease in some patients. She began taking Enbrel as part of a national clinical trial by the U.S. Food and Drug Administration and the drug's manufacturer, Immunex Corp. Children's Hospital is one of nine participating test sites.

The website includes all the information necessary to administer this therapy. Physicians should not attempt antibiotic therapy until they have read the protocol completely as this therapy is unlike any other they may be currently using. There are many components to these diseases and their treatment and antibiotic therapy must be tailored to the individual. Treatment periods are usually long and medication and dosage adjustments may be required. Nearly two million doctors and rheumatic patients visited this site last year The authors of this web page do not engage in the practice of medicine; we are not a medical authority, nor do we claim to have medical knowledge. HEIDI, juvenile rheumatoid arthritis After talking with an infectious disease specialist, it was decided she probably had juvenile rheumatoid arthritis.
http://www.rheumatic.org/heidi.htm
HEIDI, juvenile rheumatoid arthritis At almost 2 years of age, Heidi started getting fevers for no apparent reason. The doctor suspected a virus. The fevers became more consistent and blood tests were ordered. By this time Heidi was uncomfortable lying down or sitting up. She didn't want to be left alone. The doctor called us with the test results and said we had a very sick little girl. Her white cell count was extremely high and we were to take her to the hospital at once. Blood was drawn every four hours for various tests. There were bone scans, x-rays and a spinal tap but all revealed nothing. Her fevers were 104 degrees and above - nothing they did would cool the fever or bring any comfort. During that hospital stay the new developments appeared. She began cradling her right arm. Being a thumb sucker, this diminished another form of security. It was all such a strange, frightening experience to such a small child. Heidi stopped eating. The doctor searched for the source of the infection. After talking with an infectious disease specialist, it was decided she probably had juvenile rheumatoid arthritis. We were comforted by the fact we knew what we were dealing with, but wondered what the future would bring. The first remedy administered was approximately 21 baby aspirin a day. We ordered a case so we would have plenty on hand. By the end of the first week she was sleeping twenty-two hours or more each day. Something was just not right even though lab tests showed the aspirin level in her system to be correct. As parents, being around someone like that day in and day out gives an advantage the doctors do not have. Something had to change. To test my instinct, the next morning's dose of aspirin was eliminated. Sure enough, there was a change by mid-day. She was now staying awake longer periods and had an interest in eating again. It wasn't until later we discovered it was becoming toxic to her system.
