1583 Occlusal etiology of the Temporomandibular Joint disorders: A case-control study M. ALARC"N , and D.A. PAREDES, Universidad Peruana Cayetano Heredia, Lima, Peru Objectives: To determine if the occlusal factors are etiologically associated to the TMJ disorders. Methods: A sample of 85 persons (37 cases and 48 controls) between 15 and 20 years was selected based on the Craniomandibular Index (TMJ sounds and TMJ capsule palpation components). Later, with a blind operator, the occlusal characteristics of the cases and controls selection were described. Two types of TMJ disorders cases were selected: TMJ sounds disorders and TMJ capsule disorders. Through logistic regression cases and controls were related to occlusal factors: 1) centric contact, maximum intercuspidation and occlusal stability criteria, 2) non working side contacts, 3) working side and protrusive contacts. Through logistic regression cases and controls were related to occlusal factors and the gender like a possible effect modifier. Results: No one occlusal factor was associated to TMJ sounds cases in general, only the gender [OR:2.77 (1.09,7.02)] is significant like a covariable. A preventive relationship was found between the presence of factor 2 and the TMJ sounds cases with clicking [OR:0.49 (0.28, 0.83)]. That wasn’t found for the TMJ sounds cases with articular crepitation. For the cases with TMJ capsule disorders there was association with the occlusal factor 2 [OR:5.4 (1.02,28.5)], which is not affected by the gender.

Clinical conferences are being held once in a month to discuss the diagnosis and treatment of patients with TMD who need a multidisciplinary approach. A home page on TMD was open in WWW (World-Wide Web) to display them and to make them accessible from the members of the team as well as other doctors outside the hospital. The contents include chief complaints, clinical history, present status, radiographic, CT and MRI images, treatment history and problems in each year. each case is discussed in amiling list or in conference corner in WWW. So far, 25 cases discussed in 17 conferences were displayed in WWW. The system seems to be quite effective in establishing the treatment of TMD because information on each case is available anytime to anyone involved in the study and treatment of TMD.

Temporomandibular Joint (TMJ) Disorders About ten million Americans who suffer from such seemingly disparate symptoms as headaches, earaches, tenderness of the jaw muscles, or dull, aching facial pain often share a common problem. They all suffer from what has come to be known as temporomandibular joint (TMJ) disorders. These disorders can have a variety of causes and are believed to result when the chewing muscles and jaw joints do not work together correctly. In many cases, TMJ disorders can be successfully treated. HOW THE CHEWING MUSCLES AND JAW JOINTS WORK
The structures that make it possible to open and close the mouth are very specialized and work together when you chew, speak and swallow. These structures include the muscles, ligaments, bones and joints of the jaw. Five pairs of muscles allow you to open and close your mouth. They also control forward, backward, and side-to-side movements of the lower jaw. Also involved in these movements are the temporomandibular joints. Each of these important joints has two sections, connected by a disk, that make possible the hinge and gliding actions needed to open the mouth widely (Fig. 1).
