Isaacs' syndrome (neuromiotonia) is clinicaly characterized by muscle stiffness, cramps, difficulty in muscle relaxation after optional move and excessive sweating. Involuntary muscle cramps don't stop during sleep. The syndrome may develop spontaneously, have hereditary character or accompany polyneuropathy. 73-year old patient admitted to Dept.Neurology Regional Hospital ``Copernicus'' in Lodz because of painful cramps of calfs occurring during the whole day, burning and paresthesiae of shanks weakness of lower limbs muscle strength during walking.Neurological examination revealed many fasciculations in lower limbs and a few in upper limbs, atrophy of deltoid muscles, muscle stiffness, painful cramps of lower limbs muscles, absence of Achilles' tendon reflexes, dysaesthesiae of ``socks'' type, excessive sweating.We performed the EMG examination which showed in the rest continuous bioelectrical activity of muscle in the shape of single motor unit discharges, fasciculations and myokymia, that confirmed diagnosis of Isaacs' syndrome. We start treatment with phenytoin receiving improvement-cramps retired. Maladaptive efforts to adjust to remarriage can provoke or exacerbate parental alienation syndrome. The remarried parent, the other parent, the stepparent, and the child each may contribute to the disturbance. Underlying dynamics include jealousy, narcissistic injury, desire for revenge, the wish to erase the exspouse from the child's life in order to "make room" for the stepparent, competitive feelings between the exspouse and stepparent, the new couple's attempt to unite around a common enemy and avoid recognition of conflicts in the marriage, the child's attempt to resolve inner conflict, and parent-child boundary violations. These dynamics are discussed and suggestions for treatment are offered. Divorcing parents frequently try to undermine their children's affection for the other parent. When such efforts are characterized by severe and repetitive denigration, the children run the risk of suffering estrangement from the parent being criticized. Gardner (1985 ) introduced the term parental alienation syndrome (PAS) to describe this phenomenon.

Research Projects 1. Regulation of Ca channels in nerve and muscle cells Voltage-gated (or voltage-dependent) Ca channels are composed of at least 5 (T, L, N, P/Q, R) types as determined by electrophysiological and pharmacological properties. In neurons, N- and P/Q-type channels are located in presynaptic terminals and trigger release of neurotransmitters. In cell bodies and dendrites of neurons, almost all the types of Ca channels are found, but their composition seems to be different in different types of neurons. Although their physiological roles are not fully understood, the channels are thought to play roles in plasticity of neural circuits and regulation of enzymes and gene expression. In the last decade, several classes of genes coding different types of Ca channels have been cloned and sequenced. It is interesting to investigate how expression and subcellular localization of each type of the channel are regulated. Ca channels are playing important roles also in muscle cells. In the skeletal and cardiac muscles, a plenty number of the channels are located in the transvers tubular system and transduce electrical signal to an intracellular signal that triggers release of Ca from sarcoplasmic reticulum. In addition, the channels contribute to the generation of action potential in the pacemaker cells, that is important for the automaticity of the heart. Activity of Ca channels is regulated by transmitters and hormones. For example, L-type Ca channel is up-regulated by beta-adrenergic stimulation through cAMP-protein kinase A (PKA) system. It has been shown that PKA phophorylates alpha and/or beta subunit of the channel protein. However, an exact molecular mechanism underlying regulation of the channel function is not fully understood. We have recently found that unidentified factors in cytoplasm of cardiac myocytes significantly affect activity of the Ca channel. We are now exploring to isolate the factors to elucidate the mechanism of action of the factors.

Health Conditions and Diseases Musculoskeletal Disorders Book: Fibromyalgia for Dummies - I think this book fails to supply the most important information to those with fibromyalgia. It makes the dissorder seem so mysterious, when it is not. Much is known about FM that this book skims over or doesn't mention. Sleep is the major component to FM, because of a deficiency of serotonin, preventing us from getting delta sleep, the restorative sleep that revitalizes energy, and heals/relaxes muscles. Check out "Fibromyalgia" by Daniel J. Wallace. That does a much better job.
Book: What Your Doctor May Not Tell You About Fibromyalgia : The Revolutionary Treatment That Can Reverse The Disease
Book: Muscles: Testing and Function - This is an excellent book. It is a important reference book which I use and read cover to cover. The biggest plus about this book is the concept of alignment, and the various grouping of restrictions and weaknesses within the body. It shows this and explains these (rectrictions vs weakeness) very clearly and specifically which makes it easy to pick up theses patterns in people's bodies. Although there are better books which are set out better, the commentary and insights contained within the book are invaluable. It is a bit of a bible in many physical therapy circles. Don't get too caught up, and remember that this book should only be a start point as things are more complicated than this.
