Irritable bowel syndrome (IBS) is a very common gastrointestinal condition that is diagnosed when a person has any of a variety of abdominal symptoms and/or a change in bowel habits in the absence of detectable organic pathology. It has been called a number of different names including "Spastic Colon." Some individuals with IBS have abnormal sensations in their abdominal organs. It usually occurs in people who are between the ages of 20 and 50. It is seen in both sexes but is more common in women. It can be a chronic condition which tends to come and go over one's lifetime. Although it causes varying degrees of discomfort and inconvenient symptoms, it does not progress to any other diseases or cancer. What causes IBS? IBS may be due to a variety of factors such as stress, diet, or hormones. Studies have shown that people with IBS may have changes in the way their intestinal muscles move food and liquid through the digestive tract. What are the Symptoms?

Irritable bowel syndrome (IBS) is said to affect about 1 in 5 women and 1 in 10 men, and be a major reason for visits to GPs and gastroenterologists. For all this, there is a surprising lack of effective treatments, and textbooks are often surprisingly unhelpful. A new systematic review [1] may not provide all the answers, but it turns an impenetrable fog into a heavy mist, and gives some clues for patients and their carers.
Search and inclusions
Comprehensive searching of four databases, including the Cochrane Library was supplemented by manual searches of bibliographies, but only for English language reports. For inclusions a trial had to satisfy six criteria:
  • Treat irritable bowel syndrome. Study adult patients. Administer a pharmacological intervention to more than 10 patients for at least two weeks. Include a placebo control group. Report outcome measures of global improvement or individual symptoms or both.

Peppermint oil for irritable bowel syndrome
Clinical bottom line: Based on poorly designed trials, peppermint oil may help reduce symptoms associated with irritable bowel syndrome. Based on current data, a dose of 0.2 to 0.4 ml three times daily has an NNT of 3.1 (2.3 to 4.9) for global improvement over two to four weeks. However, further trials are required to establish whether peppermint oil is of benefit in properly diagnosed IBS patients, and whether benefit is maintained beyond two to four weeks. Peppermint oil appears to be associated with unpleasant adverse effects; further trials are required to assess harm adequately. Irritable bowel syndrome (IBS) is a common functional gastrointestinal disorder, with almost one quarter of the general population reporting some symptoms. In the US, IBS accounts for up to 3.5 million physician consultations per year. Peppermint oil has carminative and antispasmodic properties. The active agent is menthol, a cyclic monoterpene with calcium channel blocking activity similar to dihydropyridine calcium antagonists.

On this page: Irritable bowel syndrome (IBS) is a disorder that interferes with the normal functions of the large intestine (colon). It is characterized by a group of symptomscrampy abdominal pain, bloating, constipation, and diarrhea. One in five Americans has IBS, making it one of the most common disorders diagnosed by doctors. It occurs more often in women than in men, and it usually begins around age 20. IBS causes a great deal of discomfort and distress, but it does not permanently harm the intestines and does not lead to intestinal bleeding or to any serious disease such as cancer. Most people can control their symptoms with diet, stress management, and medications prescribed by their physician. But for some people, IBS can be disabling. What is irritable bowel syndrome (IBS)?
Irritable bowel syndrome (IBS) is an intestinal disorder that causes:
  • crampy pain
  • gassiness
  • bloating
  • changes in bowel habits
IBS has inaccurately been called by many names:
  • colitis
  • mucous colitis
  • spastic colon
  • spastic bowel
  • functional bowel disease
IBS is called a functional disorder because there is no sign of disease when the colon is examined. Because physicians have been unable to find an organic cause, IBS often has been thought to be caused by emotional conflict or stress. While stress may worsen IBS symptoms, research suggests that other factors also are important. IBS often causes a great deal of discomfort and distress, but it is not believed to:
  • cause permanent harm to the intestines.
  • lead to intestinal bleeding of the bowel.
  • lead to a serious disease such as cancer.

This document presents the official recommendations for the American Gastroenterological Association (AGA) on irritable bowel syndrome. It was approved by the AGA Patient Care Committee on October 5, 1996, and by the AGA Governing Board on November 10, 1996. T he following guidelines were developed to assist the physician in the diagnosis and management of patients with irritable bowel syndrome (IBS). They emanate from a comprehensive review of the medical literature pertaining to IBS. The IBS is a combination of chronic or recurrent gastrointestinal symptoms not explained by structural or biochemical abnormalities, which is attributed to the intestines and associated with symptoms of pain and disturbed defecation and/or symptoms of bloatedness and distension.
Epidemiology and Health Impact
In the United States, most European countries, China, and Japan, IBS affects 14%-24% of women and 5%-19% of men, with a decrease in reporting frequency among older persons. The prevalence appears similar in whites and blacks, but may be lower in Hispanics. Although up to 70% of persons with IBS symptoms do not seek medical attention, it accounts for 12% of primary care and 28% of gastroenterological practice (41% of all functional gastrointestinal disorders). Persons with IBS miss three times as many work days, see physicians more often for gastrointestinal or nongastrointestinal complaints, and in one study, incurred annual health care costs of $742 (1992 dollars) compared with $429 for those without IBS.
