PITTSBURGH, May 7 – Paroxetine, a drug commonly used to treat depression, can improve symptoms in people with irritable bowel syndrome (IBS), according to a study in the May issue of the American Journal of Gastroenterology. In a randomized double-blind, placebo-controlled study, researchers at the University of Pittsburgh School of Medicine found that the drug relieved some symptoms of IBS and improved the well-being of people with IBS. "This study points out the benefits of this drug as a potential new and improved treatment for IBS, a disease that is very difficult for physicians to manage," said George Arnold, M.D., F.A.C.P., clinical professor of medicine at the University of Pittsburgh School of Medicine and principal investigator in the study. IBS is a chronic gastrointestinal disorder that affects 14-24 percent of women and 5-19 percent of men in western populations and is characterized by abdominal pain, altered bowel habits and abdominal bloating. It generally has been treated with high-fiber diet, drugs or both.

Introduction
Irritable bowel syndrome(IBS) is a illness characterized by intermittent abdominal cramps and constipation with alternating periods of diarrhea. Frequently the patient will notice mucus mixed in with the stool, but there should be no blood. It is estimated that over 20 million people have IBS. Two thirds of the patients are female.
Etiology
The exact cause of irritable bowel syndrome is not known. In the past it used to be called spastic colitis or just colitis. These terms are actually incorrect since it is not a true inflammation of the bowel wall. It is said to be made worse by stress, although I frequently see it in people that deny any stress whatsoever. One theory is that it is a result of inadequate fiber in modern day diets. Over the years our colons or large intestines evolved to handle a large amount of fiber. In the last 100 years fiber has been eliminated from our diets through the processing of foods. This lack of fiber causes irregular contractions of the large intestines.
Symptoms
The usual symptoms are alternating diarrhea and/or constipation associated with abdominal cramps. The symptoms will get worse if you eat poorly as we frequently do while on vacation or out of town. Periods of stress seem to worsen the symptoms. The cramps can be located in any part of the stomach. Sometimes the cramps are severe enough to cause a visit to the emergency room. There are many different degrees of severity. There may just be an occasional mild episode, to a debilitating illness that causes frequent absenteeism from work. It is important to realize that this illness will not shorten your life. It is more of a nuisance like a headache.

IBS is a very common disorder that affects the functioning of the bowel. Although the severity of the condition (also occasionally referred to as spastic colon ) can vary considerably from one patient to another, typical characteristics include changes in bowel habits ranging from diarrhea to constipation, cramp-like pains, bloating and a feeling of "gassiness."
For some sufferers, IBS is little more than a minor inconvenience; for others it can be totally disabling. In extreme cases, the patient is unable to travel, function in the workplace or enjoy socializing.

The bad news is that it may be a lifelong problem that requires changes in lifestyle or diet to control. What is irritable bowel syndrome?
You may have heard it called "spastic colon" or "nervous bowel." It's an abnormal reaction of the digestive process to food, stress, or other factors and maybe to a combination of things. No one knows the exact cause. Normally, gentle contractions of the bowel steadily move food along. During the process, much of the water is absorbed and the remaining waste is compressed into stool. With irritable bowel syndrome, the process is disrupted in some way, usually by being speeded up or slowed down. Are symptoms always the same?
No. Symptoms are often changeable, occurring one time and not the next. But problems often include urgent diarrhea or constipation (sometimes alternating during the same or different bouts), bloating, gas, cramping pains, stomach gurgling, and white mucus in the stool. Can I do anything to control the syndrome?
