Treating interstitial cystitis
Interstitial cystitis is a chronic inflammation of the bladder that causes people to urinate sometimes painfully as often as 40, 50, or 60 times a day. Their quality of life, research suggests, resembles that of a person on kidney dialysis or suffering from chronic cancer pain. Not surprisingly, the condition is officially recognized as a disability. There's no cure for interstitial cystitis, but many treatments offer some relief, either on their own or in combination. Treatment (see chart) is aimed at relieving pain and reducing inflammation. The two main approaches are oral medications and bladder instillations drugs that are introduced into the bladder by catheter and held for 15 minutes. The procedure usually takes place in a physician's office, but in some cases these drugs can be self-administered at home.
Common treatments for interstitial cystitis
Treatment Comment Oral drugs Tricyclic antidepressants Taken at low doses, tricyclic antidepressants relax the bladder and interfere with the release of neurochemicals that can cause bladder pain and inflammation. They may also improve sleep. Amitriptyline is the medication most commonly prescribed for interstitial cystitis.

I was diagnosed in the summer of 1994 with Interstitial Cystitis, a painful bladder disease characterized by FREQUENT urination (I was voiding over 70 times a day), spasming of the bladder, ulcers in the bladder among other symptoms. I was diagnosed extremely quickly, thanks to a sympathetic and "with it" doctor however the medication was doing little to relieve the symptoms. Early this spring I faced having to pay for the expensive treatments ($1000 plus) and/or surgery. My doctor happened to take a course on acupuncture and asked for "volunteers" among his patients so that he could decide how well acupuncture worked in relation to North American medicine. The symptoms of IC are such that it would be fairly easy to feel any improvement. Virginia Shemeliuk, age 45

Interstitial Cystitis What is interstitial Cystitis?
Interstitial cystitis (IC) is a disorder in which the bladder (the organ that stores urine before it is passed out of the body) is overly sensitive, and usual causes for this, such as infection, cannot be found. The major symptoms are:
  • pain in the pelvic area
  • urgent need to urinate often (up to 60 times a day).
The pain can be in the area of the lower abdomen, urethra (tube that carries urine from the bladder outside the body) or vagina. Having sex may be painful for women who have IC. The symptoms may range from just needing to urinate more often to severe pain and urgency, which make work, sexual activity and normal social functioning difficult or impossible. IC may stay about the same or get worse with time. Some patients may go into remission for extended periods. What causes interstitial cystitis?
The cause of IC is not known, but it may possibly result from conditions such as:
  • allergy
  • vascular (blood vessel) disease
  • autoimmune disease
  • defects in the lining of the bladder wall
  • presence of abnormal substances in the urine
  • unusual types of infections that are not found with standard tests.

Antimicrobials Ineffective in Treating Chronic Prostatitis / Pelvic Pain Syndrome (CP/PPS): Though the traditional means of treating men with CP/PPS, use of antibiotics and alpha-adrenergic receptor blockers (tamsulosin and ciprofloxacin) did not lead to significant change after six and 12 weeks of treatment. Use of Saw Palmetto in Men with CP/PSS: Saw palmetto could provide clinical benefit in men with pelvic pain syndrome/chronic prostatitis, according to this two-institution placebo-controlled study. 142 men were enrolled, and followed up at 12-month and 18-month intervals. Men taking saw palmetto showed improvement on four statistical measuring documents. Isolation of Biomarker for Interstitial Cystitis (IC): Characterized by an inflammation or thinning of the bladder lining, interstitial cystitis (IC) is a debilitating disorder affecting more than a million women in the United States. Currently, no diagnostic tool exists for this condition, and a proper diagnosis can often take years. The authors of this study take an important first step toward suggesting that a biomarker exists for IC – in an antiproliferative factor produced by epithelial cells in an IC-afflicted bladder.

Q: Interstitial Cystitis I have been suffering from cystitis for about six months. I try to drink a lot of water and take cranberry tablets daily but cannot shift it. I have taken two courses of antibiotics and am reluctant to take them again. Kelly, 24 Dr Trisha Macnair responds Cystitis is a miserable condition, so coping with six months of it requires heroic effort - well done! Two courses of antibiotics should be enough to deal with any bacterial cause (presuming that your doctor tested the urine for infection to check which antibiotics were appropriate) although sometimes resistant bacteria can persist. There are other, less common causes of cystitis and your doctor should rule these out. You may have Interstitial Cystitis But I suspect that rather than an acute infection requiring antibiotics, you may have a condition called Interstitial Cystitis. This is a chronic condition where the bladder wall becomes inflamed, causing all the symptoms often ascribed to an infection - the urgent need to pass urine frequently, pain etc. These symptoms usually have to have been present for at least 9 months before the diagnosis is made. Interstitial cystitis is a newly recognised condition - for decades it was given scant credibility and dismissed as a condition of 'hysterical women' in some reports, despite the fact that many thousands of women suffer from the symptoms. Only in the past 15 years has it been found to be a very real, physical problem.

The IC Research Coordinators, (from left to right) Karen Mataranglo, RN, BS and Lynda Kelsey, RN, MS, have a myriad of interventions to help the IC patient. Their primary focus is National Institutes of Health (NIH) IC studies. Currently the intravesical BCG NIH study is being conducted on site (though closed to enrollment); the study is due to conclude in the Spring 2004. Further studies are planned, which is an exciting forecast for IC. For Dr. Culkin's patients, the Coordinators do one-on-one counseling for the newly diagnosed patients as well as those who simply need an update on IC self help. The patients are provided with a plethora of information encouraging them to take an active role in managing their disease. The patient is told IC is 75% self care and 25% clinical care.
