Interstitial Cystitis
Allergy causes soft-tissue swelling and irritation. It causes irritation of mucus membrane, in the eyes, ears, nose and throat, and in the urinary tract. When there is irritation in the throat, there is disruption of the tissue integrity and severe irritation may result. The most obvious example is the chronic "sore throat" that many allergy suffers experience. The constant mucus drainage and resultant histamine release result in tiny disruptions in the protective surface layers of the mucus membrane lining the throat. This disruption makes the sensitive subsurface layers susceptible to irritants such as spices or acid foods like hot sauce and orange juice. And that leads to even more irritation and even hemorrhagic tissue changes (bleeding). This disruption also allows the ordinarily harmless bacteria that are necessary for digestion to set up local infections in the irritated tissues.

  • Interstitial cystitis (IC) is a chronic inflammatory condition of the bladder wall that frequently goes undiagnosed. The cause of IC is unknown. No treatment is uniformly effective for patients with IC. Although many treatments options are available, only one treatment (Elmiron) is specifically FDA-approved to treat the symptoms of IC. The majority of patients who are properly diagnosed and treated obtain relief. Symptoms of IC:  bladder pain, urinary urgency, day and night frequency (up to 60 times per day), suprapubic (lower abdominal) or perineal (area between scrotum and anus in men, or vagina and anus in women) pain and pressure. Sexual intercourse can be painful. More than 700,000 Americans have IC. Average age of onset is 40. Twenty-five percent of patients with IC are under the age of 30. Ninety percent of IC patients are women, however preliminary studies of men with nonbacterial prostatitis indicate that they may actually have IC as well.

Interstitial Cystitis Often Difficult to Diagnose, Treat

I'm a 37-year-old woman with a frustrating problem that has only been recently diagnosed. For years I've experienced urinary frequency, urgency and lower abdominal pain. After visits to several doctors over many years, I've been told I have interstitial cystitis and have started on a series of treatments that are showing some promise. What is interstitial cystitis and why is it so hard to diagnose and treat? A: Your experience is not an uncommon one. Interstitial cystitis, or IC, is often difficult to diagnose and to treat satisfactorily. According to Russell Lawson, MD, former professor and chief of urology at the Medical College of Wisconsin, IC should be suspected when a patient has problems that mimic a urinary tract infection – such as burning, frequency and urgency – but with negative urine cultures and no response to antibiotic therapy. At that point your physician should consider the possibility of IC and send you to a urologist for consultation. Urologists often use a procedure called cystoscopy to visualize the detrusor muscle and bladder wall. The detrusor muscle helps to empty the bladder and a cystoscopy will help to evaluate its current level of functioning. Key findings include the discovery of inflammation in the detrusor and a reduced bladder capacity.

Interstitial Cystitis (IC) is a chronic inflammatory disease of the bladder that results in symptoms of urgency, frequency, and pain. Several causes have been proposed for this disease, including immune and/or neural dysfunction, infectious agents, and alterations in the permeability of the tissue lining the inside of the bladder (urothelium). It is currently thought that two or more of these factors may be involved with the progression of the disease. Treatments for IC are limited, and researchers at the Clinic are currently investigating several new substances that may effectively relieve the pain associated with debilitating disorder. Antiproliferative factor (APF), a peptide found in the urine of IC patients, has previously been shown to decrease incorporation of thymidine by normal bladder epithelial cells. This study was performed to determine the effect of APF on the cell cycle of bladder epithelial cells so as to better understand its antiproliferative activity. Methods Explant cultures from normal bladder biopsy specimens were exposed to APF or mock control. DNA cytometry was performed using an automated image analysis system. Cell cycle phase fractions were calculated from the DNA frequency distributions and compared by two-way analysis of variance (ANOVA).

This brief overview of Idiopathic Human Intersitial Cystitis is taken from the publication Infect. Urol. I have downloaded and for a variety of reasons edited a version of that publication , which you may read, realizing that the original article has been adulterated . This information should be of use to veterinarians as a similar- -possibly identicalsyndrome ( Feline Idiopathic Cystitis ) has been documented in cats some of whom have been erroneously diagnosed with a behavioral inappropriate urination problem.
Interstitial Cystitis (IC) primarily affects women (approximately 90% of cases) though a related condition, Prostatodynia, occurs in men. Symptoms include chronic dysuria, pollakauria,pelvic pain/bladder discomfort, reduced bladder capacity, and the absence of documented urinary tract infection A large numner of victims complain of concurrent musculoskeletal pain and of fatigue. Men may also experience rectal or pelvic pain without laboratory evidence of infection or of inflammation in urine or prostatic fluids. About 20% of effected people will experience spontaneous remission.
Etiology Theories regarding the cause of human IC include autoimmune infection toxin ischemia and defect in the bladder glycosaminoglycan protective layer synthesis

This appears to be the most popular theory . An antinuclear antibody has been found in the sera of patients with IC that matches one found in patients with suspected immune-mediated arthralgia/myalgia raising the issue of a possible common autoantigen. It is most interesting that a large portion of IC patients complain of concurrent musculoskeletal/ joint stiffness!

What is interstitial cystitis (IC)?
Interstitial cystitis (IC) is complex, chronic disorder characterized by an inflamed, or irritated, bladder wall. It can lead to scarring and stiffening of the bladder, decreased bladder capacity, glomerulations (pinpoint bleeding) and, in rare cases, ulcers in the bladder lining. IC may also be known as:
  • painful bladder syndrome
  • frequency-urgency-dysuria syndrome
An estimated 500,000 people have IC, and about 90 percent of them are women. People of any age can be affected with IC, although it is rare in children. About two-thirds of patients are in their 20's, 30's, or 40's. There is no evidence that the disorder is hereditary, or genetically passed from parent to child. What are some of the types of IC?
IC symptoms and severity vary greatly. It is believed that it may be several diseases, not just one. Two types of IC are usually diagnosed and listed below: nonulcerative IC: this is the most common type of IC, and usually affects young to middle-age women who have a normal, near normal, or increased bladder capacity. Glomerulations can be seen in the bladder wall.

An inflammation of the bladder wall, interstitial cystitis is thought to result from the breakdown of a layer of protein that normally protects the bladder lining from toxins in urine. The toxins irritate the bladder wall, which bleeds and can become scarred and stiff. At this stage, the bladder can't expand normally and can't hold as much urine as it used to. Dear Reader, What you describe may be a chronic inflammatory condition of the bladder called interstitial cystitis (IC). Symptoms of IC vary from case to case, but often include mild discomfort, pressure, tenderness, or even intense pain in the bladder and surrounding area. People, usually women, who have IC may also experience a frequent and urgent need to urinate. A relatively newly recognized disorder, IC has been a diagnosis for only about 20 years now, and is difficult to diagnose correctly. Because symptoms vary so widely, many scientists believe IC is a syndrome rather than a specific disease. Most scientists also do not believe that interstitial cystitis is caused by bacteria, and there are many theories on what does cause IC. We do know, however, that IC is neither a psychosomatic nor a stress-related disorder. In this case, it is important to see a urologist, who would have to diagnose IC through a process of ruling out other possibilities. An extensive list of other, similar disorders that need to be checked off the list before a diagnosis of IC can be made include bacterial cystitis (urinary tract infection), bladder cancer, kidney problems and kidney stones, tuberculosis, vaginal infections, sexually transmitted infections (STIs), endometriosis, radiation cystitis, and neurological disorders. A urologist may also perform a cystoscopy (an examination of the bladder with a tube) to check for hemorrhages on the bladder wall. These occur in about 95 percent of patients with IC.

Interstitial cystitis , a urinary bladder disease of unknown cause characterised by pelvic pain, urinary frequency (as often as every 30 minutes), pain with sexual intercourse, but no pain with urination. It is more common in women (20 : 1) than in men. The cause of interstitial cystitis is unknown, though several theories have been advanced (these include autoimmune, neutitic, allergic). The theory garnering the most current support is that of the missing bladder lining. Normally, the bladder has an inner, mucous, lining. When this lining goes missing (for unknown reasons), urinary chemicals can "leach" into surrounding tissues causing pain and urinary symptoms. Treatment is oriented toward replacing the presumed missing lining with medication, avoidance of bladder irritants such as dietary spices and alcohol, stress reduction, and pain management. Interstitial cystitis is a condition in which the walls of the bladder are irritated and inflamed. Although it is often initially diagnosed as a bladder infection, interstitial cystitis does not appear to be directly caused by an infection. It is a relatively uncommon condition affecting approximately one person in 3000. Ninety percent of those diagnosed with interstitial cystitis are women. Women of any age can have interstitial cystitis but it is most often diagnosed during a woman's reproductive years. Interstitial cystitis can be very painful and is often one of the underlying causes of chronic pelvic pain . It may also be more common in women with other chronic health conditions including:

Is interstitial cystitis related to cancer? No, there is no evidence that interstitial cystitis is associated with cancer. Women with interstitial cystitis (IC) do not have an increased risk of developing bladder cancer. Does interstitial cystitis affect fertility and pregnancy? There are no studies that suggest that interstitial cystitis influences fertility - although it may affect your interest in sexual activity. A survey done by the Interstitial Cystitis Association found that although women reported a mild increase in symptoms during pregnancy, this primarily related to urinary frequency. Pain increases were not reported. Some women are actually freed from symptoms of IC during pregnancy and breast-feeding suggesting a hormonal component to the disease.
