Interstitial Cystitis of the Bladder

I am a female in my 70's and have had a cystoscopy under sedation and was diagnosed as having an inflammation of the bladder. I was told to avoid coffee, tea, chocolate and diet sodas. I'm taking Prosed DS twice a day but it doesn't seem to be helping that much. Are there other foods I should avoid or is there anything else I could do to give me relief from the almost continuous burning feeling?
A: You may have interstitial cystitis which is a chronic problem of the bladder mainly experienced by women. Besides pain and burning, patients often feel an urgency to void and urinate very frequently in small amounts, have pelvic pressure with sitting, and experience pain with sex which may persist for days afterwards. Some women have ulcers or bleeding of the bladder. The good news is that you are not alone. WHAT IS INTERSTITIAL CYSTITIS? Many cat owners are familiar with the signs of feline lower urinary disease in the cat. Whether we call the syndrome FUS for feline urologic syndrome or use the new name of FLUTD for feline lower urinary tract disease, it amounts to bladder pain, straining, and bloody urine. Interstitial cystitis (“cystitis” simply means inflammed bladder) is human disease that is also characterized by excruciating bladder pain and straining. In 1990, Dr. M. Clasper made the suggestion that these two conditions might be closely related such that humans could benefit from studies involving affected cats and affected cats could benefit from studies of affected humans. YOU KNOW A PERSON HAS INTERSTITIAL CYSTITIS WHEN: There are several diagnostic and historical features that “clinch” this diagnosis in a human:
  • The person must experience urinary pain and/or urgency. The problem usually has been going on for more than 9 months and the person must have a urination frequency of at least eight times a day.
    The person must have less than 350cc (about a 1 and ½ cups) of bladder capacity while awake.

Interstitial Cystitis - Pelvic Pain from the Bladder

The many causes of pelvic pain make it a difficult condition to assign a diagnosis to. Some of the causes, such as interstitial cystitis (IC), are even more difficult to diagnose. The average patient may go many years with symptoms before the diagnosis is made. For that reason it is important to recognize the symptoms of IC. Interstitial cystitis is characterized by urinary frequency, urgency and pain. The pain can vary from a mild burning or discomfort, to an excruciating pain in the bladder, lower abdomen, perineum, pelvis, vagina, low back pain and thighs according to Sant GR: Interstitial cystitis a urogynecologic perspective . Contemporary Ob/Gyn 1998;43(6):119-130. It can include pain with intercourse and is often indistinguishable from other causes of pelvic pain. The pain can flare-up or go into remission. It usually gets worse around the time of menses. Sexual intercourse makes the pain worse. Patients may void 8 times or more at night and sleep loss may lead to depression. If a woman is voiding more than 8 times during the day or 4 times or more during the night, IC should be considered especially if there is pain or urgency present. Because the symptoms may be similar to an acute or chronic bacterial infection, patients have often had multiple treatments with antibiotics when in fact the urine culture does not show infection at all. Interstitial cystitis is characterized by an inflammatory response in the bladder wall but bacterial cultures are negative. Antibiotics do not help the pain and urinary frequency. End stage interstitial cystitis (IC) often results in ulcers in the bladder wall that can be seen when a scope (cystoscopy) is used to view the inside of the bladder. The gold standard in diagnosis of IC used to be by cystoscopy in which either ulcers are seen or petechial hemorrhages. Biopsy of the bladder mucosa may not be needed if the appearance is typical. Now. however, the concept of IC is changing.

What is interstitial cystitis?
Interstitial cystitis (IC) is a chronic bladder condition that can cause pain and other symptoms. People with IC have an inflamed or irritated bladder wall. This inflammation can lead to:
  • scarring and stiffening of the bladder; less bladder capacity (the bladder is able to hold less urine); pinpoint bleeding in the bladder lining. In rare cases, ulcers form in the bladder lining.
Of the more than 700,000 Americans estimated to have IC, about 90 percent are women. Severe cases of IC can be very disabling.
What are the causes of interstitial cystitis?
No one knows what causes interstitial cystitis. Because IC varies so much in symptoms and severity, most researchers believe that it may actually be several diseases. One theory being studied is that IC is an autoimmune response following a bladder infection. Another theory is that there are substances in urine which are irritating to people with IC. Other theories are also being studied. Interstitial cystitis is different from urinary tract infections, which are caused by bacteria and can be treated with antibiotics. Studies have shown that antibiotics are not useful for treating IC.

P atients with irritative voiding symptoms and negative urine cultures frequently present a diagnostic challenge for primary care physicians. Interstitial cystitis is a severely debilitating disease of the urinary bladder. Symptoms of interstitial cystitis include excessive urgency and frequency of urination, suprapubic pain, dyspareunia and chronic pelvic pain. Interstitial cystitis interferes with employment, social relationships and sexual activity. The course of the disease is usually marked by flare-ups and remissions. Interstitial cystitis is a chronic disease that persists throughout the patient's life. On clinical evaluation, patients with interstitial cystitis should have no other definable pathology, such as urinary infections, carcinoma, or radiation-induced or medication-induced cystitis (Parsons CL. Interstitial cystitis, new concepts in pathogenesis, diagnosis and management. Presentation at 3rd annual meeting of the American Urological Association, June 4, 1998, San Diego, Calif.).

Interstitial Cystitis: A Bladder Problem
What is interstitial cystitis? Interstitial cystitis is a chronic bladder problem. About 750,000 Americans have interstitial cystitis. Most of them are women. People with interstitial cystitis have a bladder wall that is inflamed and irritated (red and sore). This inflammation can scar the bladder or make it stiff. A stiff bladder can't expand as urine fills it. There may be pinpoint bleeding from the walls of the bladder. A few people get sores in the bladder lining. People with interstitial cystitis may have many of the following symptoms:
  • An urgent need to urinate, both in the daytime and during the night Pressure, pain and tenderness around the bladder, pelvis and perineum (the area between the anus and vagina or the anus and scrotum). This pain and pressure may increase as the bladder fills and decrease as it empties in urination.

affects primarily women. It is not to be confused with normal cystitis. Transcript: Norman Swan: Now your letters are always welcome here on the Health Report and one particularly sparked my interest recently. It came from Diane Turner in Warragul, Victoria who asked us to cover the hidden, painful and debilitating problem of interstitial cystitis. Since it primarily affects women (and isn't by the way to be confused with normal cystitis), I went to Dr Helen O'Connell. Helen is at the Royal Melbourne Hospital and is the only female urologist in Australasia. Dr O'Connell: Interstitial cystitis is a condition where the patients predominant symptom is bladder related discomfort. It's a non malignant, non infective condition which may be associated with changes that are apparent when you look at the bladder, but sometimes the bladder may appear absolutely normal, that the patient may be crippled with discomfort. Summary: This is a hidden, painful and debilitating bladder problem which affects primarily women. It is not to be confused with normal cystitis. (This is a repeat of part of a programme first broadcast on 13th October 1997). Norman Swan: Now your letters are always welcome here on the Health Report and one particularly sparked my interest recently. It came from Diane Turner in Warragul, Victoria who asked us to cover the hidden, painful and debilitating problem of interstitial cystitis.
Since it primarily affects women (and isn't by the way to be confused with normal cystitis), I went to Dr Helen O'Connell. Helen is at the Royal Melbourne Hospital and is the only female urologist in Australasia.
Dr O'Connell: Interstitial cystitis is a condition where the patients predominant symptom is bladder related discomfort. It's a non malignant, non infective condition which may be associated with changes that are apparent when you look at the bladder, but sometimes the bladder may appear absolutely normal, that the patient may be crippled with discomfort.
Norman Swan: And what sort of discomfort do they complain of. What are the typical range of stories?
