Reproductive Aged Population Affected: 9% About 5.3 million people in the U.S. suffer from infertility, affecting men and women equally. The average couple attempting conception has about a 20% chance of conceiving per menstrual cycle. This means that it is unlikely that a pregnancy will result after the first or even second month of trying. However, 95% of couples do conceive within 13 months. The minority who do not become pregnant after a year's time still have a good chance only about a quarter of these will never conceive. By Pamela Prager Couples who face infertility not only face the emotional pain associated with not being able to have a child, but may also face obstacles put in front of them by their health insurance and employers. The following guidelines are designed to assist couples to overcome some of these obstacles on their own.
However, every situation is different under the law, and an attorney should be contacted for assistance with specific legal problems. Should it be necessary to seek legal advice, the references listed at the end of this article will be of assistance in evaluating your case.
INSURANCE
Many insurance carriers do not provide health insurance coverage for infertility, or provide only very limited insurance coverage. If your claims for infertility treatment have been denied, take the following steps:

Determine whether or not you live in a state that has mandates for infertility insurance coverage. There are a handful of states that mandate insurance coverage for infertility to some extent or under certain conditions.

Infertility is the inability to conceive and become pregnant after 12 months of regular, unprotected sex. Infertility is a common medical condition, affecting approximately 6 million Americans every year. It is estimated that about 10% to 15% of heterosexual, married couples who try to conceive are unable to do so after 1 year. Twelve months may seem like an arbitrary length of time, but it makes sense given that most fertile couples become pregnant within a year. So if a couple does not conceive after a year of effort, it is likely that the man, the woman, or both partners are infertile. Infertility affects men and women equally. Eighty percent of infertility cases have a known cause, and half of these are wholly or partially due to male infertility factors, most commonly azoospermia (the absence of any sperm) or oligospermia (the presence of too few sperm). Infertility
Infertility affects more than 4.9 million couples in the United States. Defined as the inability to conceive after a year of unprotected intercourse, or the inability to carry a child to term, infertility is a growing problem among women and men. Experts say this is because more women are waiting until they are older to have children, which increases the chances that their fertility has been affected, possibly by a previous sexually transmitted infection, pelvic inflammatory disease, scarring from earlier surgery, or the aging process itself. While 86 percent of couples do conceive a child within two years without assistance, one in seven has difficulty. The cause of infertility may lie within the body of the woman or the man, or it may be a combination of factors from both partners. It is estimated that 40 percent of infertility problems can be attributed to causes within the man, and 40 percent to the woman, while 20 percent are caused by problems within both the man and the woman or are unexplained. An estimated 25 percent of infertile couples have more than one factor causing infertility. If conception is not successful after more than a year of unprotected intercourse (six months if the woman is aged 35 or older), it may be time to seek help. A gynecologist or a specialist known as a reproductive endocrinologist can recommend simple self-help measures to improve the chances of conception. A woman can try to predict ovulation by using the sympto-thermal method of charting her cycle. Intercourse during the six days that end in ovulation is most likely to cause pregnancy. It is important to be sure that the semen is placed high in the vagina, near the cervix.

That's a fact, according to a study on "Tight-fitting Underwear and Sperm Quality" published June 29, 1996, in the scientific journal The Lancet. Tight-fitting underwearas well as hot tubs and saunasis not recommended for men trying to father a child because it may raise testes temperature to a point where it interferes with sperm production. Infertility, which affects as many as 1 in 6 couples, is defined as the inability of a couple to conceive a child after one year of unprotected sex and is usually diagnosed through medical interviews and physical examinations of both partners. Infertility can be caused by a wide variety of factors. One-third of the time the problem lies with the male, one-third of the time it lies with the female, and one-third of the time infertility is caused by problems with both the male and the female. Sometimes either the sperm or the egg are unhealthy or there is some sort of mechanical obstruction keeping the sperm from fertilizing the egg. Once an egg is fertilized it must be able to become implanted in the uterus and then the embryo must be healthy and have the proper hormonal environment in which to develop properly. The Defeat of Clinical Andrology Versus the Triumph of ICSI
The capsule of the session on male infertility “ICSI seems to be the ultimate treatment for male infertility” instigates old-fashioned andrologists, such as the first author of the present opinion paper, to think again about what they have been doing for the best part of their professional life!
In the past, andrologists have tolerated many frustrations when infertile men continued to return to their consultation because the proposed “conventional treatment” was unsuccessful. Clearly, those men who were successful are lost to the andrologist’s site. Rarely do they bother to transmit the happy message when success has been reached. The couple is then taken into care by the gynecologist, who tends to highlight his own merits in achieving the pregnancy.
So, the venue of a spectacular method to “cure” male infertility, namely ICSI, was a welcome progress. The Glad Tidings were proclaimed through the public media and in the medical press (in that sequence) that all couples could now achieve pregnancy with their own genetic material, even if the man has no spermatozoa, or suffers from Klinefelter syndrome. Simple calculations were made to prove that the success rate of this treatment was so high (up to, or even higher than 40% per attempt!) that a maximum of 3 cycles would be needed to get all couples pregnant.

Our most frequent discussions about Herpes in dogs occur because of neonatal puppy death. Herpes causes puppies to die in the first two to three weeks of life, when puppies cannot regulate their body temperature. After three weeks of age the puppies acquire the ability to regulate their body temperature, and to mount a fever response. At that time, the disease no longer causes death, but can cause a respiratory disease. We rarely find much in references about this disease in the adult dog. Where it is mentioned, it is generally in regard to future breeding advisability for a bitch that has produced a litter of puppies that had herpes, or regarding the presence of vesicles (blister like lesions) on the vulva or prepuce. Recent papers published by Dr. James Everman of Washington State University, have demonstrated that the disease as we know it in dogs is very similar to the equivalent disease in horses (Rhino). This information fills in the remainder of the picture for dogs in a very dramatic fashion. The fact that Herpes virus infection in dogs has been given such little attention by veterinarians, when its effects might well have anticipated by reproductive experts, serves as an excellent demonstration of the historical lack of interest in canine reproduction by the Veterinary establishment in general.
