Griffith, MD October 5, 1998 (Reviewed: September 16, 2002) Urinary incontinence, the involuntary loss of urine, occurs in at least a third of the elderly in the USA. Women are affected twice as often as men are. The article summarized here reviews the different types of urinary incontinence and outlines treatment alternatives. In order to treat chronic urinary incontinence successfully, it is necessary to make an exact diagnosis, based on its cause. The types of incontinence considered in this review are urge, stress, mixed, overflow and functional. Urge incontinence This is the commonest type of urinary incontinence encountered in elderly women. It usually results from abnormal detrusor muscle contractions. There are two subtypes - motor and sensory. Motor urge incontinence presents with urgency, frequency, nocturia and incontinence. In a number of cases, a neurological lesion is responsible - this may be caused by conditions such as stroke, Parkinson's disease, brain tumors or multiple sclerosis. Sensory urge incontinence is due to bladder hypersensitivity, where patients experience discomfort and urgency at low bladder volumes, commonly caused by cystitis. Detrusor hyperactivity with impaired bladder contractility, a newly recognized condition, occurs in frail elderly women; they have bladder contractions but cannot empty the bladder completely.
