WHAT IS URGE INCONTINENCE AND ITS CAUSES?
All cases of urge incontinence (also called hyperactive or irritable bladder) involve an over-active bladder.
  • In such cases, the detrusor muscle, which surrounds the bladder, contracts inappropriately during the filling stage.
  • When this occurs, the urge to urinate cannot be voluntarily suppressed, even temporarily.
There are usually one of two types:
  • Detrusor Instability. In this type, the nerves serving the bladder have signaled the brain appropriately that the bladder is full, but the detrusor muscles are unable to be suppressed.
  • Detrusor Hyperreflexia. With this type, a neurologic abnormality impairs the signaling systems between the bladder and the central nervous system, and the brain is unable to inhibit the detrusor muscles controlling urination.
Symptoms of Urge Incontinence
The primary symptom of urge incontinence is the need to urinate frequently with subsequent leakage.
Causes of Urge Incontinence
Very often, the cause of detrusor instability and bladder hyperactivity is unknown. Some conditions that can produce the disorders leading to urge incontinence include the following:

WHAT IS OVERFLOW INCONTINENCE?
Overflow incontinence happens when there is an impediment to the normal flow of urine out of the bladder and the bladder cannot empty completely. Overflow incontinence can be due to a number of conditions:
  • A partial obstruction. In this case the urine cannot flow completely out of the bladder, so it never fully empties.
  • An inactive bladder muscle. In contrast to urge incontinence, the bladder is less active than normal, not more. It cannot empty properly and so becomes distended, or swells. Eventually this distention stretches the internal sphincter until it opens partially and leakage occurs.
Causes of Overflow Incontinence
The causes of the conditions leading to overflow incontinence include the following:
  • Tumors.
  • Certain medications (anticholinergics, antidepressants, antipsychotics, sedatives, narcotics, alpha-adrenergic agonists, beta-adrenergic agonists, calcium channel blockers).
  • Benign prostatic hyperplasia.
  • Scar tissue.
  • Nerve damage. In such cases, nerves in the bladder are damaged so that they are not sensitive to fullness and so do not trigger contraction. Damage can be caused by spinal cord injuries, previous surgery in the colon or rectum, and pelvic fractures. Diabetes, multiple sclerosis, and shingles can also affect nerves in this way.

I am 19 years old and had spinal surgery in 1998 for correction of scoliosis. However, shortly after the 2nd operation which involved the insertion of harrington rods, I lost the movement in my legs and control of my bladder. I have now regained the movement in my legs (although I cannot do various things such as run) but am still suffering incontinence. I have tried drugs (detrusitol), electrical stimulation, bladder emptying techniques etc. but I still suffer from frequency and urgency incontinence. Is there anything left that I can try so that I do not have to continue to suffer for the rest of my life? I look forward to hearing from you.     description The symptom includes involuntary loss of urine during physical exertion with increased intrabdominal pressure, such as with coughing, sneezing, laughing, exercise, lifting or sitting. The urine loss is usually not a large amount but the frequency with which it occurs may require wearing a pad constantly to avoid embarassment.
    Is it common? Urinary incontinence has been reported to affect 35% of American women over 50 years of age an almost 15% who have leakage on a daily basis. Approximately 60% of women with incontinence will have stress incontinence. Differentiating
    features When lying down with legs apart, coughing produces a visible spurt of urine from the urethra. Standing up, with one leg on a stool and holding a paper towel over the perineum and coughing produces a wet stain on the paper towel. A cotton-tipped stick lubricated with xylocaine gel is placed into the urethra up to, but not through the internal urethral sphincter (Q-tip test). With straining or coughing, the stick rises more than 30 degrees from it's resting angle. This demonstrates urethral hypermobility and differentiates genuine stress incontinence from an intrinsic urethral sphincteric insufficiency (ISD) without hypermobility.

    Clinical investigations in female urinary incontinence

    Female urinary incontinence (UI) is a condition with severe economic and psychosocial impact . I have studied the epidemiology of UI in the general female population and the prevailing management of UI by Norwegian general practitioners (GPs). Those who are interested in more details, may take a look at this synopsis . The main conclusions that may be drawn from these studies are that UI is very prevalent among adult women, mostly in the form of stress incontinence. At least 6% may be regarded as potential patients. Although complicated by methodological difficulties, studies of GPs' management indicate that available therapeutic tools are not used to their full potential. A literature review demonstrates that the history is the most important diagnostic tool in general practice, and that the value of clinical investigations (described below) is poorly documented.
    Gynaecological examination
    Review papers and guidelines agree that a gynaecological examination should be included in the basic evaluation of incontinent women.(1-6) Accordingly, all authors who have actually evaluated management programmes for UI have included a gynaecological examination in their programme.(7-10) However, in a British survey only 25% of incontinent women reported that their GP had actually done a gynaecological examination.(11)
