Urinary Incontinence: Embarrasing but Treatable

What is urinary incontinence?
Urinary incontinence means that you can't always control when you urinate. As a result, you wet your clothes. This can be embarrassing, but it can be treated. About 12 million adults in the United States have urinary incontinence. It's most common in women over 50 years old. But it can also affect younger people, especially women who have just given birth. Be sure to talk to your doctor if you have this problem. If you hide your incontinence, you risk getting rashes, sores, and skin and urinary tract infections. Also, you may find yourself avoiding friends and family because of fear and embarrassment. Return to top

Urinary incontinence
VOL 108 / NO 1 / JULY 2000 / POSTGRADUATE MEDICINE Bladder control is a problem for more than 10 million Americans of all ages. In fact, at least one of every 25 Americans has some symptoms related to loss of bladder control. When left untreated, urinary incontinence can be debilitating and can lead to social isolation, psychological distress, skin problems, and even premature admission to a nursing home. What causes incontinence?
There are several types of incontinence, with different causes:
  • Stress incontinence involves leaking of urine during physical activities that cause sudden increases in abdominal pressure, such as coughing, laughing, or sneezing. Weakness of the muscles that control outflow of urine from the bladder is the usual cause. This type of incontinence is often related to stretching of the pelvic muscles during childbirth.
  • Urge incontinence is an uncontrollable need to urinate that may occur both during the day and at night. This type of incontinence may be caused by inflammation or infection of the bladder or a nerve disorder.
  • Overflow incontinence causes frequent or constant dribbling of urine because the bladder is unable to empty completely and overfills. Nerve and muscle disorders or injuries are common causes of overflow incontinence.

Peer Review Status: Internally Peer Reviewed Incontinence is certainly a common problem that we see in urology. It tends to be somewhat more common as patients age, but certainly it cuts across all age groups and sexes. The good news is that incontinence is generally a very treatable problem. However, it is important to have a good and complete evaluation prior to treatment. Urinary incontinence is a common problem. Patients should not feel embarrassed or apprehensive about talking to their physicians about this condition. The majority of patients can be successfully treated using either conservative or surgical therapy. Incontinence should not be considered in an inevitable part of getting older. The best source of information for the patient with incontinence is their family physician. They will be able to either diagnose and treat the problems themselves or make an appropriate referral. I am a 43-year-old female; 3 full term pregnancies. I have urinary leakage. I wear a pad everyday. Do I need surgery, or is there something else available?

A Medical Condition that can be Treated
More than 19 million North American adults have some form of incontinence. It is not necessarily part of growing older, nor is it something that should be accepted after childbirth. Incontinence is always a symptom of some other health problem. Many bladder control problems can be cured and all can be managed.
In many cases, a team of health care professionals including your doctor, nurse and nurse practitioner will work with you to diagnose and offer treatment options. Together, you will choose the appropriate course of action. Remember, the first step is to see your health professional. He or she is best able to offer you specific advice related to your situation.
Help is Available
Incontinence may be a symptom of an underlying disorder that warrants medical attention, or simply a side effect of certain medications.
Health care professionals are well qualified to assess incontinence and offer numerous treatment options including muscle strengthening exercises, dietary modification, medication or surgery.
Today, clinics and treatment centers throughout the nation are helping people to manage and treat incontinence. Increased awareness has helped promote a greater understanding of incontinence in today's society. People with incontinence lead full, active lives doing the same things they have enjoyed for years.

by Diane K. Newman, RNC, MSN, CRNP, FAAN The area of pelvic health in women is a growing area of concern for health care providers as well as women with disorders that involve the pelvic area (bladder, pelvic floor muscle, rectum and uterus.) Chronic pelvic pain and vulvodynia, two frustrating pelvic disorders seen in young adult women, are not well understood. Research on these two conditions, which are often linked under the umbrella of “chronic pain syndromes”, is scarce, especially as it relates to successful treatments. Women with these complaints tend to visit specialists who provide non-surgical treatments for incontinence and other pelvic disorders because they seem appropriate. This article will provide an overview of the clinical picture of both conditions, describe specific evaluation techniques, and outline practical treatment options that can be provided.
