In a study reported late last week, patients taking Enablex showed no evidence of memory or other cognitive problems. Researchers say the new medication called Enablex treats urinary incontinence from overactive bladder by specifically targeting the bladder muscles that control urinary urges. They also say the drug is less likely to affect the brain and have side effects such as memory problems compared with other drugs that are currently on the market. But an incontinence expert contacted by WebMD says that cannot be proven until the medications are compared to each other.

by Carolyn J. Strange No one wants to talk about it. Patients don't bring it up. Physicians don't ask. Meanwhile, millions suffer in silence as urinary incontinence erodes their quality of life. When it is discussed, all too often, patients are toldincorrectlythat it's a normal consequence of aging and that they should just get used to it. Misconceptions about incontinence abound among health-care professionals and the public alike. At least 13 million Americans experience urinary incontinence, the involuntary leakage of urine sufficient to be a problem, and 11 million of them are women, according to the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK). Incontinence is also more common in older people. It's not surprising that about half of nursing home residents are incontinent since it's a major cause of institutionalization, according to numerous sources. But that doesn't mean it's a normal part of being a woman or of aging. "Incontinence is never normal at any age," says Neil Resnick, M.D., chief of gerontology at Brigham and Women's Hospital, Boston, and an associate professor of medicine at Harvard Medical School. "It's treatable in the vast majority of people, most of whom have full continence restored. Most of the remaining people can at a minimum be improved," he says. Despite its prevalence, urinary incontinence is widely underdiagnosed and underreported, and many health-care providers remain uneducated about it, according to a 1996 clinical practice guideline update from the Agency for Health Care Policy Research. It's also expensive. Americans spent about $27.8 billion on incontinence in 1995, according to NIDDK. The health-care impact is likely to increase as the baby boomers age.

    In 1992, the Agency for Health Care Policy and Research (AHCPR) released its first guideline on urinary incontinence. Since then the guideline has become the standard of care for incontinence in many settings across the country. This update of the guideline reflects new findings in the rapidly changing field of treatment for urinary incontinence. To develop the update, AHCPR convened a multidisciplinary private-sector panel of physicians, nurses, allied health professionals, and health care consumers to study the effectiveness of diagnostic and treatment procedures for urinary incontinence, their costs, and how they affect patient outcomes. The results of this research show that incontinence can be improved, and in some cases, even cured. Anecdotal evidence shows that long-term care facilities that have adopted the guideline have improved the quality of life of their patients and saved money at the same time.

    What is urinary incontinence?
    Many women find it hard talking about their incontinence, but urinary incontinence is actually very common. Urinary incontinence is the involuntary leakage of urine from the bladder. This is a common problem that can affect both sexes but women are more commonly affected.
    Urine is normally prevented from leaking by the urinary sphincter, which is a tight ring of muscle at the neck of the bladder and the support of the muscles of the pelvic floor. Voluntary passing of urine involves relaxing the sphincter and pelvic floor muscles, together with a gentle contraction of the bladder muscle.
    For many women the leakage of a small amount of urine on an occasional basis is normal and does not prevent them from getting on with their lives. For other women, urinary incontinence may be serious enough to involve having to change their clothes and to avoid performing certain tasks or exercises. For these women, advice from a health professional is appropriate.
