Female urinary incontinence - studies of epidemiology and management in general practice

Female urinary incontinence (UI) is a condition with severe economic and psychosocial impact . I have studied the epidemiology of UI in the general female population and the prevailing management of UI by Norwegian general practitioners (GPs). Those who are interested in more details, may take a look at the synopsis below. The main conclusions that may be drawn from these studies are that UI is very prevalent among adult women, mostly in the form of stress incontinence. At least 6% may be regarded as potential patients. Although complicated by methodological difficulties, studies of GPs' management indicate that available therapeutic tools are not used to their full potential. A literature review demonstrates that the history is the most important diagnostic tool in general practice, and that the value of clinical investigations is poorly documented.
The general purpose of the present investigations was to describe the epidemiology of UI in the adult female population, and to describe the management offered these patients by Norwegian GPs. Secondly, the validity of some of the employed methods was investigated. Studies were undertaken with the following specific aims:
  • to validate a severity index and diagnostic questions for different types of female UI
  • to describe the prevalence of female UI in the general adult population, according to different types and severity

Urinary incontinence can result from anatomic, physiologic, or pathologic (disease) factors. Congenital and acquired disorders of muscle innervation (e.g., ALS, spina bifida, multiple sclerosis) eventually cause inadequate urinary storage or control. Acute and temporary incontinence are commonly caused by the following:
  • Childbirth
  • Limited mobility
  • Medication side effect
  • Urinary tract infection
Chronic incontinence is commonly caused by these factors:
  • Birth defects

    A patient information handout on urinary incontinence , written by Elizabeth S. Smoots, M.D., Seattle, Wash., is provided on page 2688. In most cases, the evaluation of urinary incontinence requires only a history, a physical examination, urinalysis and measurement of postvoid residual urine volume. The initial purposes of the evaluation are to identify conditions requiring referral or specialized work-up and to detect and treat reversible causes that may be present. If the patient does not appear to require referral and a reversible cause is not identified, the next step is to categorize the patient's symptoms as typical of either urge or stress incontinence and treat the patient accordingly. If treatment fails or a presumptive diagnosis of urge or stress incontinence cannot be reached, the final step would be to perform more sophisticated tests or refer the patient for testing to define the cause and determine the best treatment. U rinary incontinence, the unintentional leakage of urine at inappropriate times, is a classic syndrome in geriatric medicine. It affects approximately 13 million persons in the United States, most of whom are older adults.

    Historical Perspective - The Work of Arnold Kegel, MD
    The entire field of pelvic muscle rehabilitation in the treatment of urinary incontinence has its origins in the work of a single physician, Dr. Arnold Kegel , a Obstetrician/ Gynecologist who practiced in the Los Angeles area starting in the 1940s. But Kegel did NOT invent the idea of using pelvic muscle exercises to cure urinary incontinence; that had already been suggested by Dr. Joshua Davies of New York City in 1932. Kegel's contribution was the invention of the world's first "biofeedback" device, about 1947. His invention, the Kegel Perineometer (para-knee-om'-iter), consisted of an vaginal air-pressure cone or chamber connected by an air tube to an air-pressure gauge whose dial was calibrated in millimeters of mercury. The device enabled a woman (and her doctor-guide) to observe the strength and duration of her pelvic muscle contractions in order to learn truly effective exercise.
