The principal orientation of BSSM is toward basic science of sexual function and dysfunction, and the clinical aspects of diagnosis and treatment of sexual problems in both men and women. The Society recognizes the importance of a holistic approach to understanding of human sexuality and its problems, and encourages research into all of its many aspects. The Society will encourage, foster and maintain the highest standards in clinical practice amongst its members, and promote the adoption of similar standards amongst UK health professionals in general. Erectile dysfunction (ED), also known as impotence, is the inability to achieve or sustain an erection for satisfactory sexual activity. Erectile dysfunction is different from other conditions that interfere with sexual intercourse, such as lack of sexual desire and problems with ejaculation and orgasm. This article focuses on the evaluation and treatment of erectile dysfunction. How common is erectile dysfunction?
Erectile dysfunction (ED, impotence) varies in severity; some men have a total inability to achieve an erection, others have an inconsistent ability to achieve an erection, and still others can sustain only brief erections. The variations in severity of erectile dysfunction make estimating its frequency difficult. Many men also are reluctant to discuss erectile dysfunction with their doctors, and thus the condition is under-diagnosed. Nevertheless, experts have estimated that erectile dysfunction affects 30 million men in the Untied States. While erectile dysfunction can occur at any age, it is uncommon among young men and more common in the elderly. By age 45, most men have experienced erectile dysfunction at least some of the time. According to the Massachusetts Male Aging Study, complete impotence increases from 5% among men 40 years of age to 15% among men 70 years and older. Population studies conducted in the Netherlands found that some degree of erectile dysfunction occurred in 20% of men between ages 50 to 54, and in 50% of men between ages 70 to 78. In 1999, the National Ambulatory Medical Care Survey counted 1,520,000 doctor-office visits for erectile dysfunction.

      The problem of an inadequate erection is probably one of the biggest issues a man confronts. Most men experience erectile problems on occasion, but impotence, also called erectile dysfunction, is defined as "the persistent failure to develop and maintain erections of sufficient rigidity for penetrative sexual intercourse." Of course, men have other kinds of sexual problems including lack of desire and problems with ejaculation, yet impotence is the most common and troubling. The best way for men to begin solving erectile problems is by reading about men's sexual system - anatomy, physiology, diseases, drugs, diagnosis and treatments. Some problems may be solved simply and others may require a visit to your family doctor or a urologist. Urology is the branch of medicine concerned with the urinary tract in both genders, and with the genital tract or reproductive system in the male. Urogenital is a word that refers to the urinary and genital organs. [The medical specialty of Obstetrics and Gynecology specializes in the reproduction (genital) system of females. Nephrology is the medical specialty concerned with the kidneys.] According to the National Kidney Foundation, kidney and urologic diseases affect more than 13 million people (about 5 percent of the US population) and claim about 260,000 lives annually. The number of people affected by these diseases is expected to grow as the populations of older adults and racial and ethnic minorities, groups disproportionately affected by the diseases, increase. The most serious and debilitating of kidney and urinary tract diseases include:
  • end-stage renal disease (ESRD)
  • stone disease
  • urinary incontinence
  • benign prostatic hyperplasia (BPH)
  • interstitial cystitis
  • urinary tract infection
  • polycystic kidney disease
Other, non-related diseases have a tremendous impact on the disability and death associated with kidney and urologic diseases, including diabetes and hypertension.

The penis is the male sex organ and serves as a passageway for urine. It consists of three cylindrical masses of erectile tissues and the urethra surrounded by a loose layer of skin Two paired, copus cavernosa of the penis lie dorsally while one ventrally-located corpus spongiosum surrounds the urethra. These erectile tissues are composed of venous spaces lined with epithelial cells separated by connective tissue and smooth muscle cells. Most of the penile urethra is lined with pseudostratified epithelium, but in the glans it becomes stratified squamous epithelium. Glands secrete mucous into the urethral lumen all along the length of the penile urethra. The glans is highly innervated with sensory neurons, that are stimulated by contact and friction during sexual relations. The stimulation may result in ejaculation of semen and pleasure sensation(s). The arterial supply to the penis originates at the internal pudendal arteries. Nutritive arteries supply oxygen and nutrients to the vascular tissues and trabeculae. Other arteries (helicine) empty directly into the erectile tissues. There is minimal blood flow (i.e. pooling) to the flaccid penis.

Causes Chronic tobacco use:
Smoking has been shown to affect arterial blood flow and may also affect the arteries in the penis, reducing the blood flow necessary to maintain an erection.
Diabetes: This is one of the most common causes of impotence, because diabetes can cause changes in blood flow through narrowing of the arteries, or damage to nerve endings in the penis.
Hormonal Imbalance: Only a small percentage of cases of impotence are due to hormonal problems, such as insufficient testosterone.
Neurologic Impairment: Injuries to the spinal cord or brain, or neurological diseases, such as Alzheimer's Disease, can lead to impotence. Erectile dysfunction is commonly associated with multiple sclerosis and Parkinson's disease.
Pelvic Surgery: Surgery or radiation to the prostrate, bladder, rectum or colon can cause nerve damage in the surrounding area. The damage may interfere with signals that must pass between the brain and the sexual organs to allow erection and orgasm.

Derby, Ph.D., of the New England Research Institutes in Watertown, Mass., and colleagues analyzed data gathered on the health and lifestyles of nearly 600 men as part of the Massachusetts Male Aging Study. They found that men who began exercising regularly during the study, or who continued to do so throughout, reduced their risk of becoming impotent.
