Frequently Asked Questions
Immunology
If my child has an immune deficiency, does that mean she has AIDS? AIDS is one very serious type of immune deficiency. It is an acquired immune deficiency, meaning that it develops after birth. Not all immune deficiencies are acquired, however, and a child may be born with other conditions causing immune deficiency. These other conditions are generally far less serious than AIDS.
How serious is an immune deficiency? Immune deficiencies vary widely in their seriousness. Some will resolve in time, with or without intervention, leaving the child with a normal immune system. Others are more serious and may require ongoing treatment with intravenous immunoglobulin. A small group of extremely serious primary immune deficiencies require bone marrow transplantation from a parent or sibling.
Why is my child sick all the time? Does she have an immune deficiency?

Children With Immune Problems at Greater Bioterror Risk
Doctors warn that even smallpox vaccine is risky TUESDAY, March 4 (HealthScoutNews) Smallpox, anthrax, plague, botulism. The words spark considerable anxiety among Americans amid all the warnings about possible bioterrorism attacks. For those with primary immunodeficiency (PI), a genetic immune system disorder that afflicts more children than leukemia and lymphoma combined, the potential danger looms much larger. "Regardless of the agent we're talking about, these kids are more vulnerable," says Dr. Allan Lock, director of the developmental immunology program at the National Institute of Child Health and Human Development. "We're talking about an at-risk population." PI afflicts at least 500,000 Americans, many of them children, but often goes undiagnosed or is diagnosed only after patients become extremely ill or chronically debilitated, sometimes with life-threatening conditions, experts say.

Topic of the Month: November 2001: Allergies and the Immune System Click here for a larger view What is the immune system?
When you get a cold, what helps you get better? It’s your immune system – a very complicated system of cells that exist in many organs throughout the body. The diagram at the right portrays the many areas of the body where these specialized cells exist. Working together, the cells that make up your immune system must recognize and react to foreign substances such as viruses, bacteria, parasites, and fungi. The major components of the immune system and the roles they play
The major components of the immune system include: B-lymphocytes
  • Major function is to produce antibodies Develop from bone marrow There are 5 major classes of antibodies:
    • Immunoglobulin G (IgG) Immunoglobulin A (IgA) Immunoglobulin M (IgM) Immunoglobulin E (IgE) Immunoglobulin D (IgD)
    T-lymphocytes
    • Directly attack foreign antigens such as viruses, fungi, or transplanted tissues Act as regulators of the immune system Develop from bone marrow
    Phagocytes
    • Ingest and kill microorganisms Develop from bone marrow Prominent in the bloodstream, spleen, liver, lymph nodes and lungs

Immune Deficiency re: Treatment of CVID during pregnancy We look after patients with antibody deficiency and have recently been referred a pregnant lady with CVID. We do not have that much clinical experience with pregnant patients, as usually our patients present having already had their children. Do any of the immunologists have a protocol on how to manage pregnant ladies with CVID and ongoing monitoring of their immunoglobulin levels? To help respond to your questions, I consulted my colleague, Dr. Arnold Levinson, director of a large Adult Immunodeficiency Clinic and someone with extensive experience with CVID. His response is enclosed below. I have also enclosed below for your interest abstracts of two of the limited reports about CVID in pregnancy which I found in a Medline search.
Dr. Levinson's response:
We increase their IVIG dose in concern with their weight. My limited experience is that in addition to increased volume of distribution, I think there is increased catabolism and shortened half-life. So, don't be surprised if higher mg/kg (adjusted for weight) do not provide the desired trough levels. Particularly important considerations during last trimester when sufficient passive immunity needed to protect mom and fetus.

New Science Sheds Light on Immune Deficiencies
by Ginger Julian What do 20/20, Oprah Winfrey, the Wall Street Journal, the Journal of the American Medical Association, Reader's Digest, and Prevention Magazine have in common? They are all reporting on the dramatic reduction of seasonal depression in people who have used full-spectmm light therapy. Interestingly, a small but high-tech Santa Barbara company on the cutting edge of the new science of light therapy has been producing products for SAD (Seasonal Affective Disorder) patients for seven years now. However, what is exciting today is new evidence about how light therapy is affecting people with immune deficiency diseases such as Chronic Fatigue Immune Deficiency Syndrome (CFIDS), AIDS, Gulf War syndrome, cancer and leukemia. Increasingly, studies are showing that the use of radiation-shielded full- spectrum light is contributing in many ways to the human healing process, both mentally and physically. How? Light has been discovered to be an important nutrient which feeds and replenishes the human nervous system (the body's electrical system). In addition, important facts overlooked until now by modern medicine have shown that not only is the body made up of tissue it's wired! And this wiring needs an energy source like cells need food. Pharmaceutical companies have attempted to respond to immune deficiencies with new research on drugs and special nutrients, but according to the leading pioneer of full-spectrum lights, Dr. John Ott, light is also a nutrient which nourishes and replenishes our bodies, thereby strengthening our immune system. People with immune-system deficiency diseases are finding that sitting in front of light boxes which contain

Contact Us Home Norwich Bulletin - 12/29/2001 Immune Deficiencies And Your Pet I have a friend who is a medical doctor who has twenty-two cats and recently began chemotherapy. She never once considered giving up any of her cats and this column is dedicated to the many people with immune deficiencies whose doctors aren’t quite as understanding as to the importance of a pet.
You see, there is one medicine that makes you feel better and calms your fears with no side effects; the problem is that many doctors tell people with immune deficiencies they can’t have it – and that medicine is your pet. The truth, as my friend explained to me, is that people with AIDS and other weakened immune systems can live happily and safely with their animals.
Many people are advised to give up their pets because of diabetes, cirrhosis of the liver, chemotherapy, or other conditions that temporarily suppress immunity. This is really not necessary, especially when that beloved pet may be the most important source of pleasure and companionship that a person has.
After all, research documents that illness progresses more rapidly in depressed patients and what could be more depressing than having to give up your eight year old cat or dog? Research also shows that immune deficient people with pets tend to be less depressed and less lonely and therefore, actually improve their health.

San Francisco, CA 94143-0105 Primary T-cell immunodeficiencies are rare inherited disorders that affect T-cell development and function. These disorders usually present in infancy or early childhood; however, the age of symptom onset may vary depending on the underlying gene defect. Although T-cell immune responses may be selectively affected, abnormal B-cell function often is associated, in part because of concomitant intrinsic B-cell defects but also because production of most antibodies is dependent on T-cell help.

I have been told that I tested postive for toxoplasm antibodies. I have HV Castleman's. I still have many more test ahead of me to check my brain. I have an area on my brain that they are not sure what it is from. The doctors are doing a complete DNA breakdown on me. I do not have HIV and tested negitive for HHV-8. They mentioned a possible immune deficiency of some kind. Has anyone been tested postive for Toxoplasmosis or any immune deficiencies other than HIV? I feel that I am just going in constant circles. Important note: Information in this article was accurate in November 1982. The state of the art may have changed since the publication date.
Current Trends Acquired Immune Deficiency Syndrome (AIDS): Precautions for Clinical and Laboratory Staffs MMWR Weekly, November 05, 1982 / 31(43);577-80
Centers for Disease Control and Prevention The etiology of the underlying immune deficiencies seen in AIDS cases is unknown. One hypothesis consistent with current observations is that a transmissible agent may be involved. If so, transmission of the agent would appear most commonly to require intimate, direct contact involving mucosal surfaces, such as sexual contact among homosexual males, or through parenteral spread, such as occurs among intravenous drug abusers and possibly hemophilia patients using Factor VIII products. Airborne spread and interpersonal spread through casual contact do not seem likely. These patterns resemble the distribution of disease and modes of spread of hepatitis B virus, and hepatitis B virus infections occur very frequently among AIDS cases.
  • The following precautions are advised in providing care to AIDS patients: Extraordinary care must be taken to avoid accidental wounds from sharp instruments contaminated with potentially infectious material and to avoid contact of open skin lesions with material from AIDS patients.
