James V. Donadio, M.D., and Joseph P. Grande, M.D., Ph.D. IgA nephropathy is a relatively newly recognized disease, first described by Berger and Hinglais in 1968. It is now generally known to be the most common form of primary glomerulonephritis throughout the world. Primary IgA nephropathy is an immune-complex–mediated glomerulonephritis defined immunohistologically by the presence of glomerular IgA deposits accompanied by a variety of histopathologic lesions. Although primary IgA nephropathy receives the most attention, many other diseases are also associated with glomerular IgA deposits ( Table 1 ). The most common of these is Schönlein–Henoch purpura. This condition may indeed be indistinguishable from primary IgA nephropathy and may represent a systemic form of the disease process.

What's it really like - patient experiences of IgA Nephropathy IgA is short for Immunoglobulin A, one of the types of antibody our body produces to fight infection. This circulates in the blood. "Nephropathy" is a scientific term for kidney disease.
What is IgA Nephropathy?
Scarring and inflammation within the kidney, that can only be seen clearly under the microscope. Therefore it is normally only diagnosed after a biopsy test of the kidney. What is seen under the microscope is that the "glomeruli", which are the tiny structures which filter the blood to make urine, are damaged by deposits of IgA.
What are the symptoms of IgA Nephropathy?
These are variable from case to case. In many cases there are no symptoms, but the damage to the glomeruli causes some blood to appear in the urine. This blood is often invisible, and only detected on routine medical check-ups. In other cases the blood may be visible, coming in attacks every so often. Sometimes an influenza type illness may spark off an attack of blood in the urine, which then clears after a few days.

Aged 27 and never having experienced a 'major life event' other than being born, when I learnt two years ago that my kidneys had failed, I was not sure to what pitch of melodrama I should sink. I discovered that I had kidney failure when my eyesight began to fade. I thought that my eyes were infected. I went to the ophthalmology department at the regional hospital. As a matter of course, the doctor took my blood pressure. It was abnormally high. I was wheeled to casualty, where they took blood samples and hooked me up to all sorts of fear-inducing machinery that puffed, beeped and sucked. A young doctor came and sat down on my bed. "Now. It looks quite serious." Gulp. "You have high levels of urea and creatinine (the end product of muscle metabolism) in your blood. Your blood pressure is dangerously high. All the indications point to the possibility that you have kidney failure." I grudgingly let them keep me in overnight. My blood pressure was brought under control and the intricacies of a renal diet were explained to me. I had never given any special thought to my kidneys. But during my first two-and-a-half week stay in hospital, I soon learnt that they have many more functions than just purifying blood. Kidneys are fist-sized organs situated in the small of the back, level with the bottom rib. They keep blood pressure stable, balance the body-fluid volume and filter waste products. They also produce urine to rid the body of waste substances; and hormones to do such things as manufacture red blood cells, so that oxygen can be transported around the body, and convert vitamin D to strengthen the bones. Far from being almost an evolutionary anachronism, as I had vainly hoped, kidneys are vital organs.

What is IgA nephropathy?
IgA nephropathy is a chronic kidney disease that may progress over a period of 10 to 20 years, and can lead to end-stage renal disease. It is caused by deposits of the protein immunoglobulin A (IgA) inside the glomeruli (filters) within the kidney. These glomeruli normally filter waste and excess water from the blood and send them to the bladder as urine. However, the IgA protein prevents this filtering process. This may lead to the following:
  • blood and protein in the urine swelling in the hands and feet
What causes IgA nephropathy?
The IgA protein is a normal part of the body's immune system. It is unknown what causes IgA deposits in the glomeruli, however, in more than half of families, IgA nephropathy is inherited by an autosomal dominant gene. Autosomal dominant inheritance means that the gene is located on one of the autosomes (chromosome pairs 1 through 22). This means that males and females are equally affected, and "dominant" means that only one gene is necessary to have the trait. When a parent has a dominant trait, there is a 50 percent chance that any child they have will also inherit the trait.

IgA Nephropathy is a kidney disorder occuring during childhood and young adulthood. It usually follows a viral infection of the upper respiratory or gastrointestinal tracts. The major symptom is the passing of blood in the urine (hematuria). There may be associated pain in the loin area.
Symptoms
The first recognizable symptom of IgA Nephropathy is bloody urine (hematuria) caused by inflammation of the kidney (acute nephritis or glomerulonephritis). There is often a mild loss of protein in the urine (proteinuria) with slowly progressive changes in the kidney. Pain in the loins may occur, but it is unusual for people with this condition to show signs of high blood pressure (hypertension) or swelling (edema) during the initial phase of the disease.

IgA Nephropathy (IgAN) is one of the most common glomerular diseases world-wide. Its natural history is consistent with a clinical onset in the first decades of life. Clinical indicators of poor prognosis are proteinuria and hypertension. Angiotensin-converting enzyme-inhibitors (ACE-I) are a promising treatment, since Angiotensin II plays a key role in glomerular hypertension and permselectivity and modulates mesangial and tubular cell functions. This Concerned Action was designed to evaluate the effect of ACE-I (benazepril 0.2 mg/Kg) versus placebo in young IgAN patients ( 1g/day 50 ml/min/1.73m2). Activity and chronicity indexes are evaluated on renal biopsies together with the spatial distribution of IgA and C3 deposits by confocal microscopy. Ambulatory blood pressure measurements allow the identification of medium-high pressure levels. Enrolled patients are investigated for factors modulating the disease progression, including genetic predictors (gene polymorphism of HLA, IgA switch region, ACE, AT1 receptor, adducin, complement factors) and immunologic parameters (aberrantly glycosylated and macromolecular IgA in serum and cytokine urinary excretion).”

IgA Nephropathy What is it? IgA nephropathy, also known as IgA Disease or Berger’s Disease after the doctor who first recognised it, is the commonest of a group of conditions, which have the general name glomerulonephritis. Glomerulonephritis is an inflammation of the kidneys, in particular the glomeruli, the microscopic filters which allow the kidney to clear waste products and excess water from the blood. IgA is an antibody. Antibodies are a part of the body’s natural defences against infection. Everyone has IgA. It is usually found in our blood, the inner lining of the intestine and the airways of the lungs. If you have IgA nephropathy, a very small amount of the IgA has stuck in your kidneys causing inflammation and is stopping the kidneys working properly. The rest of your IgA will be working quite normally, defending you against infection. It is not yet known why the IgA sticks in the kidney. What are the symptoms of IgA nephropathy? The commonest symptom of IgA nephropathy is episodes of blood in the urine (haematuria . This often coincides with a sore throat or respiratory infection. Sometimes the episodes of blood appear to be caused by a bowel infection or even exercise. The urine may look like anything from slightly smoky, through tea without milk (most common), to bright red. It is not usual to see blood clots. It is usually painless, although may be accompanied by kidney discomfort.
