Gender Differences in Orthostatic Hypotension
Orthostatic hypotension primarily affects women. There are also gender differences in the blood pressure to the brain, as well as the body. The brain has additional mechanisms for regulating blood pressure. Woman may experience classic orthostatic hypotension symptoms, but not be diagnosed correctly by some doctors. Doctors in an office setting can only test for this by measuring blood pressure in lying down and then more upright positions and comparing the results. What You Can Do for Orthostatic Hypotension
Some Orthostatic Hypotension is Caused By Medications
Some medications used to treat other health problems can result in orthostatic hypotension. Two types of medication in particular are antidepressants and anti-hypertension medications. The popular Serotonin Re-uptake Inhibitor antidepressants (like Prozac and most other modern antidepressants) can induce hypotension, although they have less of an adverse effect than the previously used tricyclic antidepressants. Anti-hypertension medications are most often used for the elderly as blood pressure increases with age. Ironically, the use of these medications can sometimes swing blood pressure to the opposite extreme, resulting in hypotension. If you are taking any of these medication and are experiencing the symptoms of hypotension, you may want to talk to your doctor about switching to a different drug, a lower dose, or discontinuing use of the medication altogether.

Kausler About 40 percent of healthy older adults and 33 percent of nursing-home patients have a condition called postprandia hypotension. PPH a lowering of systolic blood pressure (the top number in a blood pressure reading) after eating a meal is a common but serious health condition in late adulthood. The incidence might be much higher for older adults with Parkinson's disease or heart failure. Systolic blood pressure indicates blood pressure during a heart beat, and diastolic (the lower number in a reading) blood pressure indicates pressure between heart beats. A pronounced lowering of systolic pressure might result in dizziness, falling, fainting or even a stroke.

Hypotension and Bedside Leukocyte Reduction Filters
(You are encouraged to copy and distribute this letter) May 4, 1999 Dear Colleague: This is to alert you to the possibility that patients who receive blood products transfused through a bedside leukocyte reduction filter may develop a precipitous drop in blood pressure. These reactions have a rapid onset. In some cases, patients also develop respiratory distress and shock. In most situations, the reactions resolve when the transfusion is discontinued and when appropriate medical intervention is performed. Reports It was also noted that there are almost no adverse event reports associated with the use of in-process, leukocyte-reduced blood products, i.e., blood products that are leukocyte-reduced by blood collection centers or laboratories at the time of collection or after the product was stored. This may be because the longer storage time allows more time for bradykinin to break down before the product is transfused. Cause of problem A review of the literature indicates that many authors believe these reactions occur more frequently in patients receiving ACE inhibitor therapy. It is hypothesized that these reactions are due to the ACE inhibitors' tendency to retard degradation of bradykinin, a potent vasodilator. Some authors have shown evidence that bradykinin may be generated as a result of filtration at room temperature or when the storage period for filtered blood is insufficient to inactivate bradykinin. Others have discussed the possibility that these reactions occur when the negatively charged membranes in these filters activate Factor XII, one of the blood clotting factors, which triggers a production of bradykinin. However, there have also been adverse events reported with the use of positively charged filters.

DEFINITION Shock is inadequate perfusion of vital organs. Hypotension is a systolic blood pressure less than 90 mm Hg systolic or a mean arterial blood pressure less than 65 mm Hg. Although shock and hypotension frequently occur together, this is not always the case because tissue perfusion is also determined by vascular resistance.
TOXIC CAUSES There are many toxic causes of hypotension and/or shock. Important examples, classified according to mechanism, include: Decreased cardiac contractility:
  • Barbiturates Beta adrenergic blockers Calcium channel blockers (especially diltiazem and verapamil) Carbon monoxide Clonidine and other centrally acting alpha-2 adrenergicagonists Opioids Type Ia or Ic antiarrhythmics (e.g., quinidine, procainamide, disopyramide, encainide, flecainide) Tricyclic antidepressants
Loss of peripheral vascular resistance:
  • Albuterol and other beta-2 adrenergic stimulants Alpha-adrenergic blockers Calcium channel blockers (dihydropyridines) Carbamazepine Hydralazine Nitrites Phenothiazines Sedative-Hypnotics Tricyclic antidepressants
Volume loss:
  • Amanitin-containing mushrooms Arsenic Colchicine Corrosives Copper sulfate Iron Salicylates
Bradycardia or AV block:
  • Beta-adrenergic blockers Calcium channel blockers (diltiazem and verapamil) Cardiac glycosides (digoxin and digitoxin) Cholinesterase inhibitors (organophosphorus agents and carbamates)

NON-TOXIC CAUSES
  • Anaphylaxis Cardiac failure Hypothermia Hypovolaemia Myocardial infarction/ischaemia Pulmonary embolus

Hypotension, also known as low blood pressure, is caused when blood vessels dilate reducing the blood pressure. As an immediate reaction sensors in the body send out signals to the brain and heart to increase the heart rate. This in turn increases the output of blood that is pumped to the body by the heart. Even so, the result is little or no change in the blood pressure. When a person has low blood pressure the blood is unable to supply enough oxygen and nutrients to the cells. This also makes it impossible for the blood to properly remove waste from the cells. In some cases a normal healthy person will have a normally low blood pressure while they are resting. In this case the person will tend to live longer. bodyOffer(17371) Blood pressure in the body is determined by three factors. The amount of blood that is pumped from the heart, the capacity of the blood vessels and volume of blood in the vessels. The greater the amount of blood that is pumped by the heart per minute, the higher a persons blood pressure will be. When the heart beats slow or the contractions of the heart are weakened the amount of blood that is pumped may be reduced. There are many factors that affect the blood pressure in the body. When more blood is circulated through the body the blood pressure will rise. It will also rise when the blood vessels have a smaller capacity. Thus any loss of blood from dehydration or excessive bleeding will reduce the bodies blood pressure, as will any dilation of the blood vessels.
