Hypospadias

The urethra has an excretory function in both sexes, to pass urine to the outside, and also a reproductive function in the male, as a passage for sperm. The external urethral sphincter is the skeletal muscle that allows voluntary control over urination.
Anatomy
Not surprisingly men have a longer urethra than women. This means that women tend to be more susceptible to infections of the bladder (cystitis). The length of a male's urethra, and the fact in contains a number of bends makes catheterisation more difficult.

What is hypospadias?
Hypospadias is the medical word for a condition in which the urinary opening (meatus) does not form as usual while a baby is growing before birth. It is one of the most common birth defects in boys, with approximately 8 out of 1,000 male newborns being affected. Rarely, it can occur in girls too, but is not as obvious. This brochure will discuss hypospadias in boys. Some cases of hypospadias are very minor and others are more complex. In a normal male, the urinary opening is usually exactly in the middle of the penis at the tip. The pictures below show the normal location of the opening and the different places where the opening might occur in boys with hypospadias.

Hypospadias is a condition of varying severity in which the opening of the urethra ( the urethral meatus) is not at the tip of the penis, where it usually is, but instead is variably located along the head and shaft below, on the bottom of the penis. Not infrequently, chordee is also present along with hypospadias. This is a band of fibrous tissue which connects the tip of the penis to the frenulum in the bottom, thereby, causing a bowing/ bending of the penile tip which is a characteristic appearance of the chordee. The two conditions may also be present independently. As far as two openings at the tip of the penis is a separate entity referred to as urethral duplication. The two openings may be one on top of the other, or side by side, the former being the more common one. There are different types of urethral duplication in terms of their internal anatomy varing from both openings being functional and connected to the bladder, to one being only a “blind pit” or just an impression of an opening. In any case this requires close and careful examination. Given your history of encounters as you described above, you may seek another urologist’s opinion to settle the controversy, since without an examination, I could not make a definitive statement.

Hypospadius Definition Return to top Hypospadias repair is surgery to correct a birth defect in boys called hypospadias. This is a condition in which the urethra does not properly exit the penis at the tip. Hypospadias can be a mild defect (with the urethral opening slightly out of place, near the tip of the penis) or severe (with the urethral opening closer to the scrotum). Hypospadias is also frequently associated with a shortening or downward bending of the penis and malformation of the foreskin. Description Return to top Surgical repair of hypospadias is done while the child is unconscious and pain-free (using general anesthesia). The type of repair depends on the severity of the defect. Mild defects may be repaired in a single procedure, while severe defects may require two or more procedures. A small piece of foreskin or tissue from another site can be used to create a tube to extend the urethral length to allow placement of the urethral opening at the tip of the penis.

Pregnant women were advised by scientists yesterday not to eat soy because of concerns that it contains high levels of a female hormone that could affect the sexual development of boys. The warning is supported by the Scientific Advisory Committee on Nutrition, which advises the Government. It said last week there was "clear evidence" that soy-based formula milk could damage children's sexual development and fertility as adults. Even though no such effects have been observed in Asia, where soy is a major part of the diet, Dr Klein's team warned that their findings were enough to spark concern. One study has linked a vegetarian diet during pregnancy to an increased risk of hypospadias, a condition where the urethra emerges along the shaft of the penis rather than at the tip. Some researchers suspect that this is due to genistein in soy. Large amounts of genistein are found in some baby formula milks and supplements taken by women as an alternative to hormone replacement therapy.

Hypospadias is a congenital disorder associated with maldevelopment of the male urethra. In general, the opening of the urethra (meatus) is not at the tip if the head of the penis (glans penis), but rather along the undersurface of the penis. Associated findings in boys with hypospadias are a downward curvature of the penis with erections (chordee) and a "hooded" foreskin, where the foreskin is present along the top of the penis (dorsal penis), but deficient along the bottom of the penis (ventral penis). The surgical treatment of hypospadias is is designed to position the meatus at the tip of the penis (the normal location), and correct any downward curvature. The choice of surgical technique is made on an individual basis. TREATMENT OPTIONS Treatment options include the use of the redundant dorsal skin as a flap to augment the deficient uretha, incision of the urethra dorsally and ventral closure (Snodgrass repair), and other techniques. At our center, we often manage patients with a history of prior repairs that have failed and who have urethral openings proximal to the normal location without available local skin for use in repair and/or strictures of the reconstructed urethra. These patients generally require complex reconstructions that may include the use of buccal mucosa grafts or staged repairs with a split thickness skin graft. However, it is our experience that even complex repairs are associated with a high success rate.

Hypospadias What is Hypospadias? Hypospadias is a common congenital anomaly (birth defect) of the penis that affects approximately 1 out of 300 boys. The urethral opening is not in the normal anatomic location but rather somewhere on the undersurface of the penis. Boys with hypospadias often are missing the undersurface half of their foreskin so that the foreskin forms a hood. There is often a bend (called chordee) in the erect penis. What causes it? Hypospadias results from incomplete development of the urethra. It is sometimes inherited. Other than inherited cases, the cause is usually not known. Why is it important? Hypospadias may cause deviation of the urinary stream so that the boy is forced to sit to urinate. Additionally, sexual function may be hampered by the location of the urethral opening or by the bend in the penis. Can it be corrected?

Events and Classes Hypospadias Print Format Severe hypospadias, a deformity in which the urethra opens on the underside of the penis, is a birth defect that can have a devastating effect. In the most serious cases, boys can't urinate normally. Men, who have this condition and have not been treated, may not be able to function sexually. The rate in which this defect occurs has doubled according to several studies conducted in the United States and Europe over the past 30 years. In a study in metropolitan Atlanta from 1968 to 1993, the rate increased from 18 per 10,000 births to 40 per 10,000 births. Researchers speculate that the increase may have something to do with exposure to environmental toxins. Some have suggested the increase also may be due to increased interest in detecting and reporting the condition. However, a federal Centers for Disease Control and Prevention report suggests since hypospadias not only are becoming more common, but severe cases are increasing, other explanations should be sought. Fortunately, most of the 1 in 100 boys born with this condition have mild cases that can be repaired relatively easily with a single outpatient operation between the ages of 6 months and 18 months. Our experienced pediatric urology specialists also are skilled at reconstructing severe deformities.

Signs and Symptoms Hypospadias are deformities of the penis resulting from incomplete development of the urethra, the tube that carries urine from the bladder to the penis. It can occur in many different ways, including:
  • Opening of the urethra below the tip on the bottom side of the penis Abnormal appearance of the glans penis (the tip) Incomplete foreskin in which the foreskin extends only around the top of the penis Curvature of the penis during an erection (called chordee) Buried penis Abnormal position of scrotum with respect to penis
This condition is second only to undescended testes as a congenital abnormality affecting a boy's genitalia. It occurs in 1 in 250 to 300 males born in the United States. There is a family tendency towards hypospadias, with an increased incidence noted in boys whose father or brothers have hypospadias. Other than inherited cases, the cause usually isn't known.
