This paper establishes a statistically significant association between maternal diet and subsequent hypospadias in boys born to vegetarian mothers. Hypospadias is a structural defect in the penis that has been increasing in frequency in several countries over the past 20 years, including the United States [ More... An important aspect of this study is that it was prospective. Mothers were interviewed during pregnancy and thus the study avoids the problem of biased recall. The study examined 7928 boys, of whom 51 had hypospadias. Of vegetarian mothers, 2.2% gave birth to a boy with hypospadias, compared to 0.6% of mothers who were omnivores. The adjusted odds ratio of hypospadias for vegetarian mothers was 4.99 (with 95% confidence intervals from 2.10-11.88).

The most extreme forms of hypospasias find the opening at the base, or even in the scrotum. Modest cases of this debilitating birth defect can be corrected through reconstructive surgery, but the most extreme cases sometimes cannot. The cause of hypospadias in humans has not been established, other than to observe that it must result from a developmental error in the womb that has prevented complete masculinization of the reproductive tract. Experiments with laboratory animals reveal that hypospadias can be repeatedly and reliable produced by exposure to several endocrine disrupting chemicals, including DDE and vinclozilin, a commonly used fungicide. The US Centers for Disease Control undertook a study of the rate of hypospadias in the US because of these animal studies. They had become concerned about endocrine disruption and decided that if it were a real problem, then there should be evidence in US health trends. They surveyed the animal studies looking for a reliable effect produced by endocrine disruption that, were it expressed in humans, would be readily and reliably detected. They were particularly interested in finding a health endpoint in which any changes in rate measured over time would not have been a result of differences in diagnosis or detection. This is a classic problem in longitudinal health studies. They selected hypospadias to study because it met these criteria.

Hypospadias is a birth defect that affects approximately seven in 1,000 male infants in the United States. In affected infants, the urethral opening is located along the underside of the penis, scrotum, or perineum; the condition usually is corrected by surgery. Hypospadias is classified in order of increasing severity as first, second, or third degree. In 2002, a study in Sweden noted that among male infants born to women who while pregnant had taken loratadine (Claritin ), a nonsedating antihistamine commonly used for seasonal allergies, hypospadias prevalence was twice that of the general population ( ). However, insufficient data were available to determine the severity of the hypospadias cases, and the study did not control for confounding variables (e.g., family history of hypospadias or maternal age). In 2003, a prospective study using data from four countries indicated that five of 142 pregnancies in women exposed to loratadine resulted in infants with major malformations, a prevalence consistent with that of the general population; none had hypospadias ( ). To further assess any potential association between loratadine and hypospadias, CDC analyzed data from the National Birth Defects Prevention Study (NBDPS). Backgrounder: Hypospadias Trends in Two US Surveillance Systems The Centers for Disease Control and Prevention s (CDC) National Center for Environmental Health recently published an article in the journal Pediatrics entitled, "Hypospadias Trends in Two US Surveillance Systems." The article focuses on data from the Metropolitan Atlanta Congenital Defects Program and a national birth defects monitoring system. The data show that rates of the male genital birth defect known as hypospadias (malplacement of the urinary outlet of the penis) have increased markedly over the past 25 to 30 years. This is the first report of an increase in the birth prevalence of hypospadias outside of Europe. Rates increased almost twofold in metropolitan Atlanta from 1968 to 1993, from around 18 per 10,000 births to approximately 40 per 10,000 births. Similarly, rates increased almost twofold in a national birth defects surveillance system based on hospital discharge data between 1970 and 1993. Hypospadias is a fairly common congenital condition that develops in baby boys. As the penis is developing in utero, the urethra usually forms an opening near the head of the penis. Answer Hypospadias is a condition where the urethral meatus (opening) is not on the tip of the penile glans; instead, the opening occurs on the ventral side (bottom or underside) of the penis.
Hypospadias may occasionally interfere with pregnancy. If the opening of the urethra is not close to the end of the penis, the sperm may not reach the woman's cervix during intercourse. Men with hypospadias may also have other less obvious abnormalities elsewhere in the male genital tract. Abnormalities in the prostate gland or seminal vesicles may be found in conjunction with hypospadias. Any of these abnormalities, as well as inadequate testicular function, may lead to problems with the semen that can affect fertility.
The first step is to consider a semen analysis. If that is normal, a urinary-LH-timed postcoital test may be a good idea. This test measures the level of sperm in the woman's body after intercourse timed to a specific point in her monthly cycle. If sperm are not found in the woman's cervical mucus 12-18 hours after intercourse, that suggests that sperm are not reaching the womb. If that's the case, intrauterine insemination may offer the best chance for pregnancy.

What is it?
Hypospadias is an abnormality of the penis which has three features:
  • the opening is not in the correct place - it is usually further back from the tip of the penis on the underside there is often bending or curvature of the penis ("chordee") which is more pronounced during erection the foreskin is missing on the lower half, giving a hooded or incomplete appearance.
The condition is usually identified at birth but sometimes it can be missed for several years.
How common is it?
Hypospadias is fairly common. It occurs in approximately one in 300 boys. The incidence is increasing.
What causes hypospadias?
The cause is unknown. It is usually an isolated abnormality. It is not strictly an inherited trait, like eye or hair colour, but it tends to occur more frequently in families where there is a history of hypospadias. The risk of having another child with hypospadias is about 1:30 or 14% of male siblings (so you should not let it affect your family planning).

Definition, Diagnosis, and Treatment
Definition: Definition: This is an abnormality of development of the penis. There are a number of separate alterations from normal, that when present in various combinations, are the spectrum of the disorder called Hypospadias. Basically, hypospadias means that the opening of the urethra,(meatus) is not in its normal position at the tip of the glans or head of the penis. This urethral defect is most often combined with an abnormality of the prepuce (foreskin) which is often opened and looks like a hood over the end of the penis. The urethral meatus can open, in this disorder, anywhere from a slight dislocation on the glans, down to the perineum behind the scrotum, which in that situation the scrotum would be separated. The glans (head) is often opened and flattened, which will later require repair as part of the corrective procedure. The urethra can also be patulous or gaping which is another variable. The corpus spongiosum, the spongelike tissue around the urethra is often not developed properly, is scarred or is absent in that part of the urethra involved. There is a "urethral plate" or posterior mucosa present in most cases. Chordee, or curvature of the penis, is often combined with the urethral defect thus giving the penis a shortened appearance. The farther the meatus (opening) is from the end of the penis, usually the more chordee that is present. The curve is most often ventral or curving downward. It can also be lateral, and in addition, various degrees of turning in the long axis can also be present.

In this congenital defect of the penis, the tube that passes urine from the bladder to the outside of the body (the urethra) opens on the underside of the penis or in the scrotum, instead of the end of the penis. In addition, bands of fibrous tissue on the underside of the penis often give it a sharp, downward curvature called chordee. Occasionally, boys with hypospadias also have other, less readily apparent, abnormalities of the urinary tract. To detect such abnormalities, the pediatrician may recommend an examination of the entire urinary system including the kidneys and bladder by means of special X-ray or imaging studies. HOW DOES HYPOSPADIAS DEVELOP?
Hypospadias is a developmental defect originating in the fetus. Until the sixth week of embryonic life, rudiments of both male and female productive systems are present in the fetus. By eight weeks, the penis and urinary structures begin to form in the male fetus. The urethra develops as folds of tissue along the underside of the penis fuse together. Embryology Formation of the genital tubercle and lateral genital folds (indifferent) occurs during 6/40. DHT dependent elongation of the phallus and urethral groove forms between lateral folds (during 10/40), and the epithelial lining of the groove forms the urethral plate. Treatment of Hypospadias in Children at Mayo Clinic in Rochester
Hypospadias is characterized by an abnormal position of the opening of the penis that occurs in 3 in 1,000 newborn boys. It is also associated with abnormal bending of the penis. Hypospadias may vary in severity from mild cases characterized by the opening of the urethra near the tip of the penis on the glans to more severe cases where the opening of the urethra is located on the midshaft or near the base of the penis. Hypospadias is a congenital defect (present at birth). Mayo Clinic pediatric urologists have extensive experience with surgery to correct hypospadias and perform these operations weekly.
Treatment Options
The best treatment option for hypospadias is penile surgery, which should be performed around 1 year of age. Success rates with most operations range between 80 and 90 percent. Mayo Clinic pediatric urologists advise that a boy be seen by a specialist shortly after birth to discuss the timing of surgery, types of operations and any other questions parents may have. Boys with hypospadias should not be circumcised, so the foreskin can be preserved for use in later surgical repair. Standard surgical techniques produce excellent cosmetic and functional results and should not interfere with sexual function or fertility. Most hypospadias surgery can be done on an outpatient basis (no overnight stay). It may be warranted that the patient stay in the Rochester area for 5 to 6 days after surgery for follow-up care.
