True Hermaphroditism: Considerations in the Management of Patients Presenting in Early and Adult Life Abstract
A twenty-seven year old man with penoscrotal hypospadias and unilateral undescended gonad was diagnosed with lateral true hermaphroditism during an evaluation for infertility. Phenotype male true hermaphrodites seldom present in adult life, and rarely for infertility. This case illustrates how diagnosis and treatment may be delayed until a significant personal event, such as infertility, prompts medical evaluation.
Introduction
True hermaphroditism is a rare disorder of intersexuality, which if not diagnosed in neonatal life, usually manifests during adolescence.1 True hermaphroditism refers to the presence of both testicular and ovarian tissue in the same individual. The external genitalia in these individuals may range from normal male to normal female, however, most phenotypic males have hypospadias. Ambiguous genitalia or severe hypospadias in a patient without two palpable gonads should be evaluated further for the presence of an intersex state.2 In infancy, genetic counseling and determination of the dominant phenotype are crucial to gender assignment and rearing. Late diagnosis, however, poses additional dilemmas for the patient in that the malignant potential of undescended or dysmorphic gonads is greater and issues of fertility may become significant. We report the second case of true hermaphroditism presenting as infertility in a phenotype male patient. A discussion of early and late diagnosis, potential for fertility, and risk of malignancy in patients with true hermaphroditism is offered.

This leaflet explains hypospadias repair, and what to expect when your child comes to Great Ormond Street Hospital for this operation.
What is a hypospadias?
This means that your child has a problem with his penis. There are three problems that may be present:
  • the hole through which he passes urine is not at the tip of the penis; the foreskin is all at the back of the penis, with none at the front; his penis may be bent when stiff.
Why does my child need this operation?
The problems above may prevent your son passing urine standing up and, when he is older, having a sexual relationship. All of these can be corrected by the repair operation.

Ich rate allen Männern mit diesem Problem sich die Selbsthilfegruppen im Web (Adressen siehe unten) anzusehen und, falls möglich, beizutreten. - Du wirst sehen, dass Du mit diesem problem nicht alleine bist! Hypospadie ist eine angeborene Kondition bei der die Harnöffnung des Penises nicht an der Spitze ist, sondern irgendwo entlang des Schaftes, zwischen Spitze und Basis, zu liegen kommt. Ausserdem kommt es bei Hypospadie auch oft zu einer Verkrümmung des Penises, was ihm ein hackenförmiges Aussehen gibt, weil die Eichel sich nach unten biegt und die Vorhaut unvollständig ist. Im Moment gibt es nicht sehr viel Information über diese Kondition am Web, aber ich hoffe, dass diese Seite all jenen hilft, die mit Hypospadie leben. Hypospadie mag nicht sehr bekannt sein, aber für die Männer, die damit leben, können die Folgen verheerend sein. Hypospadie betrifft die Wurzel unserer Männlichkeit, und wie ich auch auf der Seite über Penisgössen schreibe, ist es für die meisten Männer ungemein wichtig, sich mit dem Rest vergleichen zu können. This is caused by incomplete development of the urethra in utero between 8 and 20 weeks of gestation. Various degrees of hypospadias result in an opening located anywhere along the length of the urethra. Degrees of hypospadias are classified according to location, including anterior (50% of cases), middle (20% of cases), and posterior (30% of cases). Hypospadias may also cause a curvature of the penis ( chordee ). Chordee is sometimes evident only with an erection. Severe chordee may result in the inability to perform sexual intercourse.

Q: Hypospadias My friend say he has 'hypospadias'. Please can you tell me what this is, and most important of all, is his life at risk? I am asking you because he did not tell me anything about this and I am very concerned for him, and I don't want to ask him in case he gets upset. David Dr Trisha Macnair responds Hypospadias is an abnormality of the penis which about 1 in 300 baby boys are born with. In hypospadias the opening of urethra (the tube which carries urine and semen out of the body - i.e., through which a man urinates and ejaculates) is on the underside of the penis instead of being at the end of the penis. The urethral opening may still be on the glans or head of the penis or it may be further down the shaft. In some men who are affected with hypospadias, there is another abnormality called chordee. In this condition, the penis curves downwards and the foreskin only covers the front of the penis. In the most severe forms of hypospadias the urethral opening is so far back it is almost in the scrotum. The scrotum itself may be small and the testes undescended (not in the scrotum, but still deep in the abdomen). When babies are born like this it can be difficult to work out what sex they are without further tests.

DEFINITION:
This condition is characterized by an ectopic urethral opening which may be located ventrally on the glans, penile shaft, scrotum or perineum. The hypospadiac meatus may be either normal in diameter, wide, very wide or stenotic (usually in distal forms of hypospadias). Often, there is a blind ending periurethral duct with its orifice placed distally from the hypospadiac meatus. The glans may be flattened or conical in shape, and the glanular groove can be well or poorly developed...

Hypospadias Repair
Definition:
Surgery to repair a birth defect of the male urination tube (urethra). The normal male urethral opening is at the tip or end of the penis. A defect in which the urethral opening is on the underside of the penis instead of on the tip is called a hypospadias. Hypospadias may be a mild defect (with the urethral opening slightly out of place, near the tip of the penis) or severe (with the urethral opening farther up the shaft of the penis). Hypospadias frequently includes a shortening or bending under of the penis. Description:
Surgical repair of hypospadias is done using general anesthesia (i.e., you are asleep.) The type of repair depends on the severity of the defect. Mild defects may be repaired in a single procedure, while severe defects may require two or more procedures.
The males circumcised foreskin is used to create a tube to extend the urethral length to allow placement of the urethral opening at the tip of the penis. Circumcision should NOT be performed at birth on infants with hypospaidas so that the foreskin can be used for the repair. Hypospadias repair kills two birds with one stone - getting rid of the hypospadias and properly circumcising the male.

Hypospadias is a congenital abnormality of the penis in which the urethral opening is positioned either along the shaft of the penis on its underside or on the scrotum or perineum instead of being located at the tip of the penis. With increasing degrees of severity, the penis is curved downward (referred to as chordee This is a relatively common condition, historically occurring in about 1 per 300 to 500 live male births. The incidence and severity of hypospadias has doubled in the United States over the last 25 years. This increase is believed to stem from environmental pollution with substances (chiefly insecticides) that have either estrogen-like effects or testosterone-blocking effects. Overall, the incidence is now believed to be about 1 in 100 male babies. The less common but more severe forms of hypospadias, in which the urethral opening is located on the penile shaft or in the perineum, may interfere with normal urination in the usual male standing position and may interfere with fertility. These more severe cases are usually corrected early in childhood to avoid social embarrassment and psychologic trauma. The ideal age for repair is before the age of 18 months. Routine newborn circumcision should be avoided

La pathologique chirurgicale en pédiatrie Docteur Michael Nord Hypospadias Il s'agit d'une malformation congénitale (existant à la naissance) du pénis. Elle n'est pas rare car elle touche 8 garçons sur 1000 naissances masculines. Cette affection peut être génétique, donc avec hérédité possible, mais également due à certains médicaments (hormones) pris pendant la grossesse. Le méat urinaire, l'orifice par lequel les urines peuvent s'écouler, se trouve normalement à l'extrémité du pénis, dans l'axe de celui-ci.
En cas d'hypospadias, le méat urinaire se trouve sur la face ventrale, le dessous, du pénis, entre l'extrémité du pénis et les bourses, quelques fois même dans les bourses.
En cas d'épispadias, le méat se trouve sur la face dorsale, le dessus, du pénis. Le pénis est souvent court, et peut ne pas être rectiligne. De plus, le fait que le méat s'abouche de manière anormale peut entraîner des troubles de l'érection. Plus l'hypospadias est sévère (plus le méat est situé loin de l'extrémité du pénis), plus il est important de réaliser au plus tôt une étude du sexe génétique de l'enfant (risque d'ambiguité sexuelle).
La conséquence de ces malformations est une gêne à la miction (uriner), et surtout, plus tard, une diminution du pouvoir fécondant (déposer le sperme au fond du vagin lors des rapports sexuels) pouvant aller jusqu'à la stérilité.

Introduction
Hypospadias is a congenital defect of the penis resulting in incomplete development of the penile urethra. The abnormal urethral opening may be any place along the shaft of the penis or may open onto the scrotum or the perineum. As the position of the urethral meatus becomes more proximal, ventral shortening and curvature during penile erection are more likely. In patients with severe hypospadias, the genitalia may look ambiguous at birth, resulting in emotional and psychological stress for parents in that the gender assignment of their baby immediately comes into question. Left uncorrected, patients with hypospadias may need to sit down to void and tend to shun intimate relationships because of the fears related to normal sexuality.
Incidence
Hypospadias is one of the most common congenital anomalies, occurring in approximately 1 in 250 to 1 in 300 live births. In Europe, the prevalence of hypospadias in the 1970's and 1980's has been increasing with no obvious explanation. In the United States, data from two birth defects surveillance systems has also shown an unexplained doubling in the incidence of hypospadias. (Paulozzi, Erickson et al. 1997) The U.S. study from the Centers for Disease Control is particularly intriguing in that the incidence of severe hypospadias, not just mild forms, is increasing, implying that the increase in hypospadias is not due to an increase in surveillance or reporting.

Definition Hypospadias is a relatively common abnormality in which the opening of the urethra is on the underside, rather than at the end, of the penis
Male Reproductive System Hypospadias is a congenital defect that affects up to 3 in 1,000 newborn boys. The condition varies in severity. In most cases, the opening of the urethra is located near the tip of the penis on the glans. More severe forms of hypospadias occur when the opening is at the midshaft or the base of the penis.Occasionally the opening is located in the scrotum or the perineum (beneath the scrotum). This anomaly is often associated with chordee, a tight fibrous band that results in a downward curvature of the penis seen with erection. (Erections are common with infant boys.)
