Delay in the appearance and growth of ossification centres as well as in their fusion and disappearance is characteristic. The growth plate remains open and its metaphyseal side is "closed off" as bone abuts on the cartilaginous tissue. Immature body proportions and facial features, abnormal distribution of fat, and delay in eruption of secondary teeth are among the features of hypopituitarism.

Many of these patients are found to have a small pituitary gland and fossa and the posterior pituitary may be in an ectopic position, usually in the region of the pituitary infundibulum or hypothalamus. These anomalies may only be detected by high resolution coronal and sagittal MR imaging of the pituitary region. An ectopic pituitary is seen as a bright spot in the region of the tuber cinereum on sagittal T1-weighted sequences with concomitant absence of the normal posterior pituitary bright spot in the pituitary fossa. The pituitary fossa may be small and appear empty, an 'empty sella'. The condition may be associated with

production of certain hormones: insufficient gonadotropins production
(luteinizing hormone and follicle-stimulating hormone) In premenopausal women, this leads to absent menstrual cycles, infertility, vaginal dryness, and loss of some female characteristics. In men, this deficiency leads to impotence, shriveling of testes, decreased sperm production, infertility, and loss of some male characteristics. insufficient growth hormone production This usually produces no symptoms in adults. In children, this deficiency can lead to stunted growth and dwarfism.

Clinical suspicion
Suspect hypopituitarism if there are manifestations suggestive of deficiency in more than one endocrine system. The presence of CNS pressure manifestations is suggests that pituitary tumors are the cause. Local pressure manifestations include headache, visual field defects, optic atrophy, cranial nerve palsies, Korsakoff-like syndrome with recent memory loss (pressure on the 3rd ventricle) and hyperphagia or thirst sensations which may result from hypothalamic involvement.
Diagnosis
Once suspected a blood sample is drawn for hormonal analysis and hydrocortisone 100mg given once followed by 50mg 6 hours later and then regular replacement of the dose (20-30mg divided over two daily doses). This is done to avoid the risk of adrenal insufficiency or coma that may complicate the confirmatory tests.

They may present initially with deficiency of a single hormone: prolactin testosterone (in males), LH FSH (in post-menopausal females), TSH thyroxine (free) , cortisol. Non-functioning tumours Adenoma Craniopharyngioma Meningioma Germinoma Metastatic carcinoma Functioning tumours eg Prolactinoma Gonadotroph adenoma Alpha subunit - this may be the main secretory product of the tumour.
