  • What Is It? Hypoglycemia is an abnormally low level of blood sugar (blood glucose). Because the brain depends on blood sugar as its primary source of energy, hypoglycemia interferes with the brain's ability to function properly. This can result in dizziness, headache, blurred vision, difficulty concentrating and even more serious neurological symptoms. Hypoglycemia also triggers the release of epinephrine, norepinephrine and other body hormones that work to raise blood-sugar levels. The release of these hormones produces additional symptoms of tremor, sweating, rapid heartbeat, anxiety and hunger.

    What is hypoglycemia?
    Hypoglycemia is the condition of having a glucose (blood sugar) level that is too low to effectively fuel the body's blood cells. Glucose is the main source of fuel for the body. The normal range of blood sugar is approximately 60 to 120 mg/dl (milligrams of glucose per deciliter of blood). When the level is below 45 mg/dl, a serious condition is suspected. Hypoglycemia may be a condition by itself, or may be a complication of diabetes or other disorders. It is most often seen as a complication of diabetes, which is sometimes referred to as insulin reaction.
    What causes hypoglycemia?
    There are many different possible causes of hypoglycemia, including:
    • diabetics taking too much insulin to lower the blood sugar
    • other drugs used to treat diseases such as AIDS-related pneumonia
    • psychological disturbances
    • alcohol consumption without eating adequately
    • diabetics who miss a meal or exercise too strenuously
    • insulin-producing tumor in the pancreas

    What is hypoglycemia?
    If your blood sugar drops too low, you can have a low blood sugar reaction called hypoglycemia. A low blood sugar reaction can come on fast. Generally, a level below about 60 mg/dl is called low blood sugar. It may occur once or twice a week if you have Type 1 diabetes. Hypoglycemia is much less common with Type 2 diabetes, but it can happen especially with some medications. People who have tight control of their diabetes are more at risk for low blood sugar and so are the elderly. Studies have shown that 50 percent of severe low blood sugars occur between midnight and 8 a.m. Talk to your diabetes educator about what to do.

    Hypoglycaemia Emergency in Hypoglycaemia Hypoglycemia is commonly called "low blood sugar". Even people who do not have diabetes have episodic low blood sugar when they have not eaten enough or exercised vigorously. The difference in common episodic low blood sugar and the severe hypoglycemia of diabetes is the potential for natural processes of the body to respond and recover from it. In diabetes, hypoglycemia occurs in combination with medication that is intended to keep blood glucose down. If low blood sugar occurs in diabetes, it is usually the result of an oversupply of medication relative to the amount of carbohydrate eaten and/or exercise done. Because of this medication, a normal recovery from low blood sugar (increase glucose release from the liver) is not possible. When blood glucose levels drop below 60 mg/dl, most people will experience symptoms of shakiness, irritability, fatique, etc. If a person with diabetes senses these symptoms at this blood sugar level, carbohydrate treatment should be initiated and recovery quick. In some people, however, especially those who have had diabetes for many years, hypoglycemia may not be easily detected. Or it may come on and advance very rapidly. In these situations, blood glucose levels can fall to dangerous levels.

    ABSTRACT: Iatrogenic neuroglycopenia represents a medical emergency for which accepted and usually effective treatment exists. Treating physicians face a malpractice risk if there is permanent neurologic damage and there is a perception of failure to act prudently both before and after the damaging episode. We report two cases to illustrate the medical and legal issues that may be raised in such claims. A REVIEW of Indiana malpractice claims involving care of diabetic patients during a 13-year period (35 claims) found none based directly on hypoglycemic brain damage.1 We report two malpractice claims brought against internists in the southeastern United States after insulin- and glipizide-induced neuroglycopenia with permanent sequelae occurred. CASE REPORTS Case 1. A 75-year-old man with insulin-treated diabetes was hospitalized after an episode of disorientation. He was taking 50 units of NPH insulin at 8 AM and 30 units at 7 PM. He had a 10-year history of hypertension controlled by a low-salt diet and 250 mg of chlorothiazide daily. There was no history of neurologic or ophthalmologic abnormality. On admission, he was described as malnourished. His vital signs were normal. He was alert and in no distress. Neurologic and ophthalmologic examinations were unremarkable apart from absent ankle reflexes. There were no sensory abnormalities. No carotid bruits or cardiac murmurs were heard. Laboratory studies showed a fasting blood glucose level of 90 mg/dL, serum albumin value of 2.6 mg/dL, BUN value of 35 mg/dL, and creatinine value of 2.4 mg/dL, with 1+ albuminuria.

    Finally, Help for Hypoglycemia Sufferers!
    A re you tired of the medical community telling you hypoglycemia doesn't exist or "Your symptoms are all in your head" or "You just need to take a vacation"? To date there is little to no information available for sufferers of hypoglycemia and when you do find information, you are left wondering if it is correct. For the above reasons and because of my misdiagnosis of hypoglycemia for nearly 10 years (yes...10 years) I had developed a quarterly newsletter called Sweet Nothings Through Sweet Nothings , I have met people from all over the world and from all walks of life. Sweet Nothings has even been mentioned twice in Veggie Life magazine. It has been the most rewarding experience of my life. In fact, it has made me realize that I am not meant to be an accountant the rest of my life. My purpose is to help people...not companies. I am now dumping my career of 10 years and am going to pursue medical school (my first career choice many moons ago).     Conscious Choice, July 1999 Americans love to "carbo-load." We are a society in love with carbohydrates and it shows in our poor health standing versus the other industrialized nations of the world. In fact, the United States Surgeon General's office says that eating habits helped account for two-thirds of all deaths last year from five of the top ten killer diseases and diabetes is one of them! But Americans don't get their nutrition information from the government or even from their doctors they get it from advertising. Though we talk up things like fiber, what is often palmed off as a high fiber food is a highly refined commercial "brown" bread made of food coloring and white flour in a wrapper depicting a log cabin on the front a far cry from the fiber intake of our stone-age ancestors: roots, shoots, foliage, and occasional berries.
    In essence, hypoglycemia is low blood sugar, and it is increasingly prevalent in our society. Hypoglycemia can cause an array of symptoms, including spaciness, fatigue, mood changes, PMS, sugar craving, headaches, difficulty focusing the eyes, tremors, temperamental outbursts, depression, excessive sweating, hot flashes, palpitations, cold extremities, abdominal pain, and panic attacks.
    If you have any of these symptoms, the next time you go to a doctor, try asking him or her if the symptoms you experience could be due to hypoglycemia. You may provoke a bemused or annoyed look, or perhaps your doctor will, in fact, suggest a glucose tolerance test. Don't bother. Glucose tolerance tests, as performed conventionally, are not designed to detect hypoglycemia until or unless it has reached a very extreme level.

    The Wrong Diet can produce stress in the body. The body must then compensate, or adapt to keep functioning. Selye and Tintera have found that the keystone of defense is the adrenal cortex. An individual with adrenal under-function does not handle stress well. Tintera calls this hypoadrenocorticism (Bul 96). Selye writes that the main regulators of the stress syndrome are: brain, nerves, pituitary, adrenals, thyroid, liver, kidney, blood vessels, connective tissue cells, and white blood cells. These main regulators interact, causing the body to adjust or adapt. The way in which they interact is different for every person depending on one's conditioning factors such as heredity, previous exposures, diet, etc. Thus, the same stress my result in chest pain in one person, depression in another, and allergies or general well being in others who are able to adapt, at least for the time being. The Great Number of Symptoms which may result from the failure of the hormonal adaption mechanism is admittedly a challenge to credibility. Excessive fatigue and inability to concentrate are the most common. Salzar found that relative hypoglycemia mimics any neuropsychiatric disorder. He listed the major symptoms from a psychiatric standpoint: depression, insomnia, irritability, lack of concentration, crying spells, phobias, forgetfulness, confusion, unsocial or anti-social behavior, and suicidal tendencies.

    THE SUGAR PARADOX Hypoglycemia or low blood sugar, probably contributes to more symptoms of chronic "unwellness" than any other condition. A controversy prevails among health care professionals about whether hypoglycemia exists on a wide-scale basis. Most medical textbooks explain hypoglycemia as a condition due to organic causes such as a pancreatic tumor, a liver glycogen storage disease or an insulin overdose.(1) The average physician may miss the diagnosis of hypoglycemia due to the rarity of these conditions. However, in most instances, appropriate treatment relieves many of the debilitating symptoms. Many patients report that treatment of their low blood sugar problem seems to alleviate other symptoms as well.(2,3,4) Therefore, the condition does exist. We refer to it as "reactive hypoglycemia", which occurs about two to six hours after a meal challenge.(1)

Special Medical Reports The following is part of " Tips From Other Journals " from the September 15, 1997 edition of American Family Physician Nocturnal Hypoglycemia in Children with Type 1 Diabetes Hypoglycemia is a common complication in children with type 1 (insulin-dependent) diabetes mellitus. Younger children with type 1 diabetes have a higher incidence of hypoglycemia-induced nocturnal seizures than older children. With the emphasis on tight glycemic control, adverse effects of hypoglycemia (including neurodevelopmental difficulties) have become a growing concern. Porter and associates studied children with type 1 diabetes receiving twice-daily insulin injections to determine the frequency of nocturnal hypoglycemia and to assess any predictors of this complication. Nocturnal blood glucose levels were measured from samples obtained during the evening or the night immediately before a clinic visit. A questionnaire was administered to review any symptoms that may have occurred during the previous 48 hours and during the study period. Carbohydrate intake on the night of the study was determined. The authors conclude that episodes of nocturnal hypoglycemia may be related to possible neurocognitive sequelae. Care must be taken to avoid glycemic control tight enough to cause episodes of low serum glucose.
