SCREENING GUIDELINES: We have summarized below the current national recommendations. These recommendations need to be modified for individual patients. The rationale for screening for cholesterol problems rests on the observations that there is a direct relationship between cholesterol level and the risk of heart attacks and that lowering levels (at least in high-risk patients) leads to a reduction in heart attacks. More recently, other problems related to arteriosclerosis such as stroke have also shown to be correlated with cholesterol levels. As a result, screening should be done at least once every five years for all persons age 20 and over. On May 15, 2001 the National Cholesterol Education Program Total Cholesterol: stratified according the following criteria: HDL Cholesterol: "Good Cholesterol"; it is a negative risk factor for heart attack and is stratified according the following criteria:
  • Very High — above 75 mg/dL (associated with the "longevity syndrome")

Compounded on top of this there is limited information available explaining the condition in canines, even in Miniature Schnauzers were it occurs very frequently. It can be a serious, if not a life threatening condition and must be addressed by your vet. I will be giving basic information to enable you to approach your vet and discuss hyperlipidemia and to ask questions, especially as it may pertain to a diabetic dog. Since Miniature Schnauzers are predisposed to hyperlipidemia (and since Tyler is also a MS) my focus will be directed toward Miniature Schnauzers. However, any breed of dog or cat can acquire certain types of hyperlipidemia. It is an important factor to consider and deal with when other diseases are present. With Tyler it has effected his diabetes regulation, has caused additional eye problems and other problems for him. I will discuss these problems at the end of this article. Hyperlipidemia is a condition in which the amount of fats (lipids) in the blood are elevated. This includes both triglycerides and cholesterol being too high . Hyperlipidemia occurs normally for a short time after a meal, then returns to the correct level. However, some dogs have a metabolic problem which prevents the proper cleaning of the fat from the blood stream, resulting in the high lipids. To be clear, there are two other separate conditions; hypertriglycerides (high triglycerides only) and hypercholesterolemia (high cholesterol only). These conditions are not discussed here.

Hyperlipidemia (Blood Cholesterol Levels) Why should I get my cholesterol level checked?

Medical research has proven that elevated total cholesterol levels in the blood result in an increased risk for heart disease. The National Cholesterol Education Program recommends that adults age 20 years or older have a cholesterol test done at least once every five years. How is high cholesterol detected?
When your doctor orders your blood cholesterol test, he or she may order a preliminary assessment of total cholesterol and high density lipoproteins or HDL (good cholesterol), which can be done without fasting. Your doctor may order a complete cholesterol profile which requires fasting and includes total blood cholesterol, triglycerides, low density lipoproteins or LDL, and HDL. A high LDL level is considered dangerous because it seems to promote the deposit of cholesterol in the artery walls. HDL is considered beneficial because it helps carry cholesterol away from the tissues. How is high cholesterol treated?

Department of Cardiology, Cedars-Sinai Medical Center, Los Angeles, California What is hyperlipidemia? Hyperlipidemia means an increased amount of fat in the blood. The principal fat in the blood is cholesterol, a naturally occurring substance utilized in creation of body hormones like estrogen and testosterone. Cholesterol is also an essential component of the membrane that surrounds individual cells of the body. Because fat is insoluble, cholesterol must be bound to proteins when it is transported in the blood. This complex of fat and protein is called a lipoprotein. The three principal lipoproteins that physicians measure are: low-density lipoprotein (LDL, or "bad" cholesterol), high-density lipoprotein (HDL, or "good" cholesterol) and triglyceride. Because hyperlipidemia is a pervasive problem in the United States, the National Institutes of Health established the National Cholesterol Education Program. Part of this program includes target levels of LDL, HDL, and triglyceride in people with and without coronary heart disease.

Almost 13 million American adults require drug therapy to meet the low-density lipoprotein goals set by the National Cholesterol Education Program. Attempts to achieve these goals through diet and exercise are often unsuccessful. Major studies in recent years have demonstrated that statins decrease low-density lipoprotein levels, coronary events and overall mortality. Statins are the most commonly prescribed lipid-lowering agents because they are effective, well tolerated and easy to administer. Niacin has beneficial effects on all of the main lipid components, and new extended-release tablets have fewer adverse effects. Fibrates remain the most effective agents in lowering triglyceride levels and should be limited to this use. Bile acid sequestrants are seldom prescribed because of their adverse gastrointestinal effects and cumbersome administration. (Am Fam Physician 2000;61:3371-82.) C oronary heart disease (CHD) is the most common cause of death in the United States. Each year, 500,000 Americans die of this disease, and associated morbidity costs more than $200 billion annually.

Lipoprotein particles are responsible for the transport of cholesterol and triglycerides in the bloodstream. Apolipoproteins, which are the proteins on the surface, play a crucial role in regulating their transport and metabolism. Lipoproteins are divided into four major classes (which are further sub-divided), based on density, composition, and electrophoretic mobility.

What is Hyperlipidemia?
Hyperlipidemia is a term that means you have a high level of lipids (fats / cholesterol) circulating in the blood. There are different types of hyperlipidemias, all of which are risk factors for developing heart disease. Other factors such as genetics, environment, habits, and presence of other diseases such as diabetes and hypertension may also contribute to the development of heart disease. Some of these factors are within our control; others are not. Studies have shown an association between high blood cholesterol and premature heart attacks. Excess cholesterol in the blood can settle in the arteries and form a plaque (a raised lesion on the inside of an artery). Over time, this plaque can build up and narrow the arteries, which in turn may clog the flow of blood. This process can begin in early childhood and may eventually result in coronary artery disease, heart attacks or stroke.

While this shouldn't come as a surprise, given that HIV-positive people are now living longer, it's also clear that many HIV-positive people are experiencing profound increases in their cholesterol and triglyceride levels as a result of the anti-HIV medications they are taking. This has raised fears that HIV-positive people taking these medications are at a higher risk of experiencing a heart attack or a stroke as a result of their hyperlipidemia . But we're also learning how best to manage these increased lipid levels, through delaying anti-HIV treatment until it's absolutely necessary, switching treatments when problems arise, initiating dietary changes and exercise programs, and prescribing lipid-lower drugs to help bring cholesterol and triglyceride levels under control. This lesson reviews what we know – and what we don't know – about increased lipids in HIV-positive people. The first part of this lesson reviews the basics of elevated lipids, including the medical terms that are important to know. The second part of this lesson reviews the causes of increased lipid levels, including the various anti-HIV drugs that have been shown to play a role. The third part reviews what we know about managing increased lipids, including switching anti-HIV drugs and using other treatments to reduce lipids and to reduce the risk of a heart attack or stroke.
