ABSTRACT Background Prospective studies suggest that hyperinsulinemia may be an important risk factor for ischemic heart disease. However, it has not been determined whether plasma insulin levels are independently related to ischemic heart disease after adjustment for other risk factors, including plasma lipoprotein levels. Methods In 1985 we collected blood samples from 2103 men from suburbs of Quebec City, Canada, who were 45 to 76 years of age and who did not have ischemic heart disease. A first ischemic event (angina pectoris, acute myocardial infarction, or death from coronary heart disease) occurred in 114 men (case patients) between 1985 and 1990. Each case patient was matched for age, body-mass index, smoking habits, and alcohol consumption with a control selected from among the 1989 men who remained free of ischemic heart disease during follow-up. BACKGROUND Insulin-mediated glucose disposal varies widely in apparently healthy human beings. When insulin resistant individuals cannot secrete enough insulin to overcome this defect, glucose intolerance supervenes. However, the ability of hyperinsulinemia to prevent decompensation of glucose tolerance appears to be a mixed blessing. Individuals that are insulin resistant and hyperinsulinemic are more likely to be hypertensive and have a dyslipidemia characterized by a high plasma triglyceride (TG) and low high-density lipoprotein (HDL) cholesterol concentration. All of these have also been shown to increase risk of coronary heart disease (CHD), and in 1988 these relationships were explicitly addressed and designated as comprising a Syndrome (X). Although there is evidence that variations in insulin action are under genetic regulation, probably as much as 50% of the variability in insulin-mediated glucose disposal within a population of normal volunteers is secondary to difference in life-style. In this context, obesity and decreased physical activity play particularly crucial roles, each contributing approximately equally to the variability in insulin-mediated glucose disposal seen in the population at large. It is also worth noting that powerful ethnic differences exist, with essentially every other ethnic group studied being more insulin resistant than individuals of European ancestry.
