Congenital hydrocephalus... can be traced to a birth defect or brain malformation
In 1983, at the age of 13, I acquired hydrocephalus when I was hit in the head by a baseball. It was a freak accident.
The term "communicating hydrocephalus" means that the site of increased resistance to CSF drainage resides outside of the ventricular system in the subarachnoid space.
Non-communicating, or obstructive, hydrocephalus is caused when there is an obstruction in the flow of CSF within the ventricular system of the brain.
Hydrocephalus is often associated with Dandy-Walker syndrome, neural tube defects, and spinal bifida.
CSF is constantly being produced, flowing through, bathing, protecting, and cleansing the structures of your brain....
To communicate effectively with neurosurgeons, patients who have hydrocephalus need to become familiar with the basic structures of the brain. The effect that hydrocephalus has on an area of the body greatly depends on where the blockage or abnormal accumulation of CSF is located. Hydrocephalus occurs when there is an obstruction of one of the ventricles or ventricular foramina (CSF passageways) that restricts the flow of CSF

As the two entities overlap and it is not always possible to distinguish between them on the basis of imaging studies, the term of hydrosyringomyelia is often more properly used. The clinical picture in hydrosyringomyelia is independent of the aetiology and is a function of the sectional and vertical extent of the cavity. The classical clinical syndrome is marked by segmental anaesthesia of the dissociative type (loss of sense of pain and temperature with preservation of the sense of touch) associated with segmental weakness, absence of tendon reflexes and atrophy of the upper limbs and back, and spastic paresis in the lower limbs. The complete spectrum occurs relatively infrequently. About 80% of patients present with complaints of sense of stiffness in the legs and weakness in the legs or hands. Pain is also, despite the anaesthesia, a fairly frequent complaint. Hydrosyringomyelia can accompany both congenital and acquired spinal lesions. In some cases no associated lesion is demonstrable and the disease is defined as idiopathic.

The most common minor anomaly is the isolated posterior rachischisis, present in about 2% of asymptomatic persons; isolated anterior rachischisis is unusual, as well as isolated defects away from the midline. Ponticulus posticus , an ossified groove of the vertebral artery, asymmetrical atlanto-occipital joints and pseudonotch of the atlas, are other asymptomatic anomalies or variants. Other craniovertebral anomalies belong to two main groups: manifestations of an occipital vertebra and occipitalization of the atlas ( Fig.1 Fig.2 ). The presence of bony formations around the foramen magnum like a third condyle, and epitransverse or paracondyloid processes belong to the first group; fusion or assimilation of part or the entire atlas to the occiput belong to the second. Other craniovertebral anomalies are absent or separate odontoid, basilar impression and basilar invagination.

Additional images (click to enlarge): Radiologic Findings: MRI T/L-spine: Longitudinal low-signal abnormality in an expanded cord consistent with a syrinx. On post-Gadolinium, images a l cm uniformly enhancing lesion is seen in the lower T-spine cord. No flow voids are present. A similar smaller lesion is present lower at approximately the T-11 level. MRI Brain/C-spine: Moderate hydrocephalus and cervical continuation of the syrinx. A 3-cm. bilobed brightly enhancing cystic lesion involves the inferior cerebellum adjacent to the fourth ventricle and extends into brainstem and upper cervical cord. Smaller, similar intramedullary lesions are also present in the upper cervical cord. Cerebellar tonsils are normally placed.

Department of neurology and Psychiatry, Ain Shams University ABSTRACT In the present study 20 patients with the clinico-radiological diagnosis of congenital syringomyelia are included. All patients were examined by MRI and CT myelography. Special focus of attention was made on the associated congenital cranio-cervical anomalies with the aim of establishing possible cause-effect relationship between some of the associated cranio-cervical anomalies and the development of hydrosyringomyelia. The single congenital anomaly that was demonstrated in all patients was hindbrain herniation below the level of the foramin magnum (Chiari I malformation was demonstrated in 70% of cases and chiari II malfomation in 30% of cases). Hindbrain herniation induced the following anatomical aberrations; 1) stenosis of the foramin of magnum and the foramin of magendi, and 2) the foramin of magendi was demonstrated below the level of the calamus scriptorious. Spinal cord cavitation is a frequent finding in optic neuromyelitis (Devic's syndrome) (DS) but it is also, although rarely, observed in patients with multiple sclerosis (MS). The objective of our study was to compare the MRI characteristics of the syringomyelic cavities in 6 patients with DS and 3 patients with MS. All the patients with DS had a relapsing clinical form with normal brain MRI. Spinal MRI revealed unenhanced central cavities which extended more than 3 vertebral bodies and remained unchanged in follow-up studies. Two patients presented multiple cavities.MS patients suffered a relapsing remitting form of the disease, they all had hyperintense T2 enhancing lesions on their spinal MRI. Moreover spinal MRI also revealed non communicating cavities which extended less than 2 vertebral bodies.
