What is Hydrocephalus? Hydrocephalus is derived from the Greek- hydro means water, and cephalus means head. This neurological condition occurs when there is an abnormal accumulation of cerebrospinal fluid (CSF) inside each of the four ventricles of the brain. The CSF, which is normally clear and colorless, surrounds the brain and spinal cord, protecting from injury. This fluid normally flows through one ventricle to the next, and is kept over the surface of the brain and down the spinal cord. This colorless fluid is finally absorbed into the blood stream. When the circulation or absorption of CSF is blocked, or a large amount of fluid is produced, the volume of the brain becomes excessively larger then normal. Because the skull of babies does not fuse quickly, the accumulation of this fluid causes the head to enlarge due to the swelling of the ventricles. ADULT HYDROCEPHALUS DIAGNOSES INDEX What is normal pressure hydrocephalus? NPH is a disorder usually involving the elderly that presents primarily with difficulty in walking. The difficulty has been described as a "magnetic gait," one in which the patient feels that his or her feet are stuck to the floor as if there were magnets on the soles of their shoes. A person with NPH usually has problems initiating their gait - typically having to take many tiny steps in order to get going and/or if trying to stop. If severe, the patient may be completely unable to walk and be confined to a wheelchair. The second common feature of NPH is impaired bladder control. Early on, the most common complaint is urgency - having to rush to the bathroom as soon as the urge is felt. Later in the disease, urinary incontinence may develop. Declining short -term memory is the third feature of NPH. The degree of forgetfulness is above that normally attributed to aging. If advanced, the patient can become quite demented which may make the diagnosis difficult to distinguish from Alzheimer's Disease. The above three findings are commonly referred to as the "clinical triad" of NPH: 1) Gait disorder, 2) urinary incontinence, and 3) dementia. In some cases, patients with NPH can present with signs and symptoms mimicking Parkinson's disease. It should be noted that NPH is in many ways very different from hydrocephalus that begins in childhood. The symptoms, diagnosis, and treatment options are different for children, and therefore caution should be taken when reading about hydrocephalus in general at other sites.

Case Presentation:
An 84 year old man was referred from the geriatric clinic for evaluation of gait abnormality, cognitive impairment and urinary incontinence. He was found to have severe short term memory loss but no other intellectual deficit and he exhibited postural instability with evidence of frontal release.
Findings:
A CT scan showed enlarged lateral ventricles. An MRI ( transaxial and sagital ) was performed to look for cerebral vascular disease and showed changes consistent with central atrophy. Cysternography showed early entry of tracer in the lateral ventricles at 6 hours ( posterior view , arrows outline ventricular activity ). Twenty-four and 48 hour images showed persistance of activity in the lateral ventricles ( lateral view at 24 hours , arrows outline ventricular activity ). These findings were compatible with normal pressure hydrocephalus (NPH). The consulting neurosurgeon advised against any shunting procedure.
Discussion:
Cysternography has been proposed in the past as a useful tool in evaluating patients with suspected NPH (1,2). Recently, its popularity seems to have been declining (3,4). This review will discuss the strengths and the weaknesses of the test as well as the controversies surrounding its usefulness.

PEDIATRIC EPILEPSY SURGERY PROGRAM PEDIATRIC DIAGNOSES INDEX Hydrocephalus is a pathological condition that results from the imbalance between production and absorption of cerebrospinal fluid (CSF). The CSF is formed by the choroid plexus at a constant rate, therefore in hydrocephalic patients if the CSF is not removed at a conmesurable rate the lateral ventricles dilate progressively, the head circumference increases, the cerebral mantle is compressed laterally and he/she experiences the signs and symptoms of raised intracranial pressure (ICP). Hydrocephalus usually results from the obstruction to normal flow of CSF from the lateral ventricles, where it is largely formed, to the arachnoid granulation in the vicinity of the sagittal sinus where it is absorbed. Pediatric Neurosurgery at UCLA has an outstanding record of successful shunt surgeries with negligible incidence of infection or shunt obstruction. Our Pediatric Hydrocephalus Research Program is devoted to the understanding of the mechanisms underlying brain damage in pediatric hydrocephalus. In a team effort together with biochemists, neuroradiologists and neuropsychologists we are aiming at identifying a series of neuronal markers that may indicate when the developing brain is under increased risk of suffering severe brain damage. This novel research idea will improve the quality of life of every child with infantile hydrocephalus. At UCLA in all hydrocephalic patients we conduct a prospective analysis of behavioral and cognitive and we correlate this developmental outcome with the structural and biochemical status of the brain at the time of shunting.

The normal brain has four pockets, called ventricles, which are filled with fluid called cerebrospinal fluid (or CSF). The CSF is constantly being produced and is responsible for nourishing the brain and spinal cord. The CSF flows through a pathway from the brain, down through the spinal canal and is eventually reabsorbed into the bloodstream. Hydrocephalus is a condition that occurs when either too much CSF is produced, the CSF is not reabsorbed into the bloodstream or the normal flow pathway is blocked.
Types of hydrocephalus
Obstructive hydrocephalus
There is an obstruction in the flow of CSF within the normal pathway.
Communicating hydrocephalus
There is no block in the flow of CSF, but the CSF is not absorbed properly back into the bloodstream
Normal pressure hydrocephalus
Usually occurs in older age groups and is due to a lack of properly functioning brain structures.
Secondary hydrocephalus
Usually results from an infection (such as meningitis) or a severe head injury.

I have tried to ensure that the information on this page is as correct as possible. I am not a doctor, I have hydrocephalus, and I have read a bit about the condition. If any-one finds information on this page which is incorrect, I expect them to tell me about it, I do not want to misinform people.
I haven't tried to find data about the success rate of treatment for a number of reasons:
  • Every case of hydrocephalus is different.
  • Current information about the results of treatment has been very difficult to find.

When I read this sources of the information which have been used to write these pages I had no intention of writting about it, and therefore didn't keep a list of the books that I read.

Next This Ryan's Web Page site is owned by Donna Prosser Ryan the star of and reason for this site is my grandson. Here is a picture of him with my son - his Uncle David. We do not think of Ryan as a "Special Needs" child we all know that he is a "Special Child". He was a year old in this picture! Looks cute huh. But things were not always this good.
Ryan was born at Treliske Hospital, Truro, Cornwall UK to our daughter Debra, on April 25th 1998 - 17 weeks premature (23 weeks into pregnancy). He weighed just 1lb that's less than half a kilo and things looked bad. He was baptized the next morning because we were warned he had very little chance of surviving. Debbie is our second daughter and her first pregnancy had gone the same way, that little boy (Rhys) only lived a few hours. Phone calls were made to all the family and a lot of tears were shed. There was also anger "why us". Then the "Why her" syndrome set in with a vengeance. My daughter, MY BABY, had never done anything to hurt anyone. She was (and still is) soft and gentle, taking in all the "lame dogs" who cross her path. Then one day surfing on the net to find information about very premature babies (the information was depressing) I found this little story. For anyone else who goes through the "Why me" or "Why us" stage please

But in January I felt very bad again: headache, vomiting, poor apetite. And my parents had to run to the hospital with me. It was a Sunday evening. The doctor controlled my shunt, and it was so painful! Next day my parents were told, that I have a shunt infection, they have to remove my shunt. The infection was a very serious one, located in my left ventricle, as the doctors told, I was near to die. The surgeon told us, that he must do a lot of painful things (punctio, drain) to save my life. I was there in the hospital for more than four months. Often had headache, vomiting, and my appetite was horrible, I wanted to eat only when my mummy was with me. All the nurses and doctors were very kind and friendly, and were playing with me, when they had free time. And I've learned to hang up my head again, and was near to roll left and right. My parents visited me every day, but I missed my brother very much!
In May 2001 i was told, that there is no infection in my head anymore, so I had a shunt surgery again. After 7 days was at home again! But in June - after 2 weeks in my home - I had to go back to the hospital for 2 weeks, because my temperature was very high, about 40 degrees. Thanks God, the cause of it was not my shunt! So I arrived home in time for my brother's birthday party.
