Hughes' syndrome
Word: Word Starts with Ends with Definition Antiphospholipid syndrome is a disorder of coagulation Coagulation is a fairly complex process by which liquid blood becomes solid clots. It is an important part of hemostasis where a damaged blood vessel is ultimately covered by a fibrin clot to stop blood loss. Disorders in coagulation can lead to hemorrhage, thrombosis, or embolism. Ordinarily coagulation is initiated within seconds after an injury occurs when platelets form a plug at the site of injury. This is called primary hemostasis . Following this, various plasma components, called clotting factors respond (in a complex cascade) to form fibrin strands which strengthen the platelet plug.

The arterial system is a high-pressure system that must accommodate the systolic and diastolic pressures generated by the strong contractions of the heart's left ventricle. To withstand and adapt to these pressures the arteries are surrounded by a varying degree of smooth muscle which contracts or relaxes in response to adrenergic and other locally produced peptides. Hughes's syndrome This syndrome was first described in 1983-1986 as the association of arterial and venous thrombosis with antibodies directed against phospholipids. Originally noted as a complication in approximately 30% of patients with systemic lupus erythematosus, it is now also diagnosed in patients with thrombotic episodes and anti-phospholipid antibodies (aPL) but without clinical features of SLE - primary antiphospholipid syndrome.

Case History A 41-year-old woman complained of recurring headaches and memory loss. She had frequently driven past the entrance to her own street on her way home. She had suffered from migraines, had a leg vein thrombosis in her 20s and had six failed pregnancies, all ending in spontaneous abortion (miscarriage). Her blood tests were unremarkable, apart from showing a positive anticardiolipin test. Brain scan (MRI) showed a number of small white spots suggestive of impaired blood flow. She was treated with coumadin (warfarin), a bloodthinning agent. The headaches ceased and over the following months there was a striking improvement in her memory. Introduction In 1982, I presented clinical details of a new syndrome at the British Society of Rheumatology and the American College of Rheumatology During the years 1983-85, we went on to publish a series of papers describing a tendency among certain patients to artery as well as vein thrombosis, brain disease, recurrent miscarriages, low platelets, pulmonary hypertension, epilepsy and skin rashes (livedo). We showed that this syndrome, now frequently known as "sticky blood", was very strikingly associated with a blood antibody called "anticardiolipin" or "antiphospholipid" antibody.

What are the symptoms of Lupus?
Listed below are merely some of the possible symptoms of lupus. As you can see, these can be fairly wide ranging. Lupus is a disease with many faces, and rarely do two people have exactly the same symptoms. Moreover, symptoms will vary from day to day. You may have just one symptom one day, none the next day, and several together the next. Often these symptoms are difficult to identify as being specifically related to lupus as many of them are common to other disease conditions or may even be side effects of medication.
  • Permanent rash over cheeks (often in the shape of a butterfly)
  • Blush rash which comes and goes
  • Discoid rashes
  • Oral ulcers, either apthous or large, irregularly shaped erosions. See Glossary
  • Hair loss (often leaving short brittle stubble)
  • Itchy skin

Lupus-like Disease Causes Miscarriages, Strokes By Dianna Borsi O'Brien (Columbia, Mo.; January 16, 2000) Graham Hughes, an expert on lupus, recalls a patient he saw several years ago. Confined to a wheelchair, she was completely paralyzed from a blood clotting disorder that clogged her veins, causing paralysis, bladder disease, and multiple miscarriages. Hughes, an international expert on lupus, ran tests and found she also had antiphospholipid antibodies, an indicator of what would be later called "Hughes' Syndrome," or "sticky blood " as it is known in his native United Kingdom. Today, the once seriously ill woman lives a full and normal life, working as a secretary, freed of the paralyzing effects of the disorder. Hughes spoke on Thursday, Nov. 11, 1999, at the 23rd annual Michael Einbender Distinguished Lectureship in Medical Research of Lupus at the University of Missouri's Health Sciences Center.

What is the antiphospholipid syndrome?
The antiphospholipid syndrome is a disorder of the immune system that is characterized by excessive clotting of blood and/or certain complications of pregnancy (premature miscarriages, unexplained fetal death, or premature birth) and antiphospholipid antibodies (cardiolipin or lupus anticoagulant antibodies). Patients with antiphospholipid syndrome have developed abnormal symptoms while having antiphospholipid antibodies detectable in the blood. Antiphospholipid syndrome is also called the phospholipid antibody syndrome. Antiphospholipid syndrome has been referred to as Hughes syndrome in honor of the doctor who first described it. It is important to note that antiphospholipid antibodies can also be found in the blood of individuals without any disease process. In fact, antiphospholipid antibodies have been reported in approximately 2 percent of the normal population. Harmless antiphospholipid antibodies can be detected in the blood for a brief period occasionally in association with a wide variety of conditions, including bacterial, viral (hepatitis, HIV), and parasite (malaria) infections. Certain drugs can cause antiphospholipid antibodies to be produced in the blood, including antibiotics, cocaine, hydralazine
