Question of the Week Q: I am 19 and I took a birth control pill called Trivora for a year. When I got off of it, I started growing facial hair on my chin and side of my face. Could this have caused it? And if so, can I do anything about it?
A: I am asked questions like this quite frequently. Women report that after stopping an oral contraceptive (OC), they develop acne , or increased facial hair , or alopecia (scalp hair loss). They wonder if the OC caused the problem. Actually, it is most often the opposite. Many OCs lower free testosterone by about half. This is probably why they help acne . OCs also slow growth of unwanted hair ( hirsutism ) and probably help androgenic alopecia slightly. So, rather than the OC causing a problem which appears after it was stopped, it is more likely that it was helping and this benefit was lost when the pill was stopped. In such cases, going back on the pill may be helpful. Of course there are many variables and such a step should be discussed with one's doctor. OCs may not be enough by themselves to give a satisfying result with alopecia or facial and body hair but they work in the right direction. More on this and other treatments for

Though recent decades have seen many changes in our concepts of masculinity and femininity, one thing has not changed: women still are not supposed to have hair on their face or body. That this is not biologically accurate – most women do have at least some extra hair somewhere – has not changed fashion. Indeed our society seems to have grown more “trichophobic” - phobic about body hair. Even pubic hair is now often shaved or waxed. This dislike of hair is not confined to women more and more men are opting to have chest or back hair removed. In the past, ideals of beauty were more realistic. In ancient India, a band of hair in the middle of a woman’s chest was considered a sign of great beauty. That body hair was more accepted in earlier times does not help much now since we have to be comfortable in the culture in which we find ourselves. Despite the assumption that women do not have facial and body hair, many do. How bothersome this extra hair is depends on the amount and consistency. Having to tweeze a light chin hair once in awhile can be taken in stride. But if you need to spend more than a few minutes each morning removing hair, or if your extra hair is dark and stiff, it tends to be more distressing because it is harder to conceal.

Hirsutism is the growth of long, coarse hair on the face and body of women in a pattern similar to that found in men. Besides being cosmetically distressing, hirsutism may also signal the presence of a hormone imbalance or a hormone-producing tumor. Normal hair growth

Hirsutism (hairiness)

This is usually due to above average levels of androgens, the male hormones that are normally present in women at low levels. Some women do not find the excess hair a problem, particularly if it does not affect their face, or it is blonde in colour. Sometimes excess hairiness is not abnormal and is a racial or genetic variation. Initial treatments include bleaching and electrolysis. If these do not produce an acceptable result, drugs may be used to reduce the high androgen levels, if that is the cause. The contraceptive pill contains oestrogen , which reduces androgen levels and will improve hirsutism. A formulation is available which includes a specific drug to reduce these further, known as 'Dianette'. The other component of Dianette is called cyproterone acetate, and this is the next drug to try if hirsutism persists. It is used in a higher dose than contained in the Dianette pill, but must be combined with adequate contraception, as it can cause fetal abnormality if taken during early pregnancy. Spironolactone is another alternative, but this frequently causes erratic periods, so is often given with a low dose contraceptive pill. A newer drug is called flutamide , which appears promising, though its safety profile is less clear. Side effects of the anti-androgens include tiredness, mood changes and reduced sex drive. Both flutamide and higher dose cyproterone acetate have a rare but serious side effect of causing problems with liver function, and so regular blood tests are advised.

Pathophysiology
Ultrasound and histological studies reveal that women with PCOS have numerous tiny cysts covering the ovaries. These differ from ovarian cysts which are sacs filled with fluid or a semi-solid material, that tend to appear either singly or in small numbers. The cysts in PCOS tend to be smaller, denser and more extensive. Moreover, the ovaries of PCOS sufferers are usually enlarged. In contrast, the adrenal glands are the major source of androgen secretion in idiopathic hirsutism. These abnormalities lead to the symptomatic hallmarks: hirsutism develops in 17-83 per cent of women with PCOS and 11-43 per cent suffer from acne (Falsetti, 1997b).
Signs and symptoms
PCOS can be a distressing and debilitating illness and the effects extend beyond the ovaries. As well as hyperandrogenism (excessive production of androgens, such as testosterone) PCOS is characterised by insulin resistance and obesity (Rittmaster). Patients gain weight, show excess body hair (hirsutism), develop acne and experience irregular or absent periods. A range of other symptoms appear to be associated with PCOS, including those reminiscent of premenstrual syndrome, such as hair loss, breast pain, abdominal pain, aching joints, dizziness, hot flushes, a tendency to faint and chronic fatigue. Indeed, this wide range of symptoms leads some experts to regard PCOS as a group of several related syndromes.

What Treatment is Available for my Excess Hair? Hair Basics
Hair is really just an outgrowth of the skin layer called epidermis. In fact, hair and skin are composed of the same protein (keratin). The hair shaft is produced by the hair follicle within the skin. The hair follicle has two regions: the hair bulb and the mid-follicle region. The hair bulb contains actively growing cells and pigment (melanin) producing cells. In the mid-follicle region the actively growing cells die and harden into what we call hair.
The follicle can produce two types of hair: (1) vellus hairs, which are pale, fine, and silky; (2) terminal hairs, which are darker, coarser and larger. During its life span, hair goes through three distinct phases: anagen, catagen and telogen. In the anagen phase, protein and keratin are continuously made to promote development and active growth of the hair shaft. At any point in time, 85-90% of hair is in the anagen phase, which can last from a few months up to six years. Hair then enters a transitional, or catagen, phase, when chemical and structural changes cause the follicle to regress and stop growing. In the final part of the cycle, the telogen phase, the hair follicle shuts down and goes into resting mode. In this stage hair can shed so that new hair growth can begin. The telogen phase can last up to 100 days.

Excessive growth of facial or body hair in women is called hirsutism.
Description
Hirsutism is not a disease. The condition usually develops during puberty and becomes more pronounced as the years go by. However, an inherited tendency, over-production of male hormones (androgens), medication, or disease, can cause it to appear at any age. Women who have hirsutism usually have irregular menstrual cycles. They sometimes have small breasts and deep voices, and their muscles and genitals may become larger than women without the condition.
Types of hirsutism
Idiopathic hirsutism is probably hereditary, because there is usually a family history of the disorder. Women with idiopathic hirsutism have normal menstrual cycles and no evidence of any of the conditions associated with secondary hirsutism. Secondary hirsutism is most often associated with polycystic ovary syndrome (an inherited hormonal disorder characterized by menstrual irregularities, biochemical abnormalities, and obesity). This type of hirsutism may also be caused by:

HIRSUTISM An abnormal growth of thick hair on the face, chest, back and extremities indicates Hirsutism. It is defined as a male hair growth pattern on a female body. These thick, dark hairs are different from the hair that some women have on their upper lip, chin, breasts, stomach or the fine "baby" hair all over their body. Others discover it during menopause when estrogen levels drop and in some cases, hair grows during periods of emotional stress. In other cases, hirsutism indicates a condition known as polycystic ovary disease or enlargement of the ovaries, irregular menstruation, and infertility. The cause of the condition, in rare cases, may even be an ovarian or adrenal gland tumor. These tumors also produce testosterone. What is excessive hairiness?
Excessive hairiness, also known as hirsutism, is characterized by abnormal hair growth on areas of skin that are not normally hairy. Although the condition can affect both men and women, it usually only presents a problem to women.
What causes excessive hairiness?
Excessive hairiness tends to run in families, especially in families of Mediterranean descent. The excessive hairiness in children and women may be caused by pituitary or adrenal glands disorders. In addition, women may develop excessive hairiness after menopause. Anabolic steroids or corticosteroids, and certain medications, also may cause excessive hairiness.
