Background This is a chronic and relapsing condition occurring within apocrine sweat glands, most commonly in women. Deep seated inflammatory nodules form in the axilla, groin, pubic region, and perineum. It is an androgen-dependent disorder and results from keratinous material occluding the sweat pores. Hidradenitis Suppurativa Synoniem: acne ectopica, acne inversa Wat is het? Dit is een voortdurend terugkerende ontsteking van de zweetklieren, vooral die welke in de oksels en de liezen zijn gelokaliseerd. Ze presenteren zich als grote rode, op steenpuisten lijkende bulten die zeer pijnlijk zijn. De ontstekingen keren vaak terug omdat onder de huid met pus gevulde holten (cysten) en zelfs samensmeltende cysten (sinussen) kunnen ontstaan. De bodem van deze cysten en sinussen is met opperhuid bedekt. Zij vertonen daarom geen spontane genezing. Hoe ontstaat het? Hidradenitis ontstaat na de puberteit. Op dat moment zijn de specifieke zweetklieren in de oksels en de liezen, de zgn. apocriene zweetklieren, volledig ontwikkeld. Het ontstaansmechanisme van de klachten is nog niet geheel duidelijk, maar waarschijnlijk spelen genetische aanleg, immunologisch bepaalde gevoeligheid voor bacteriele infecties en hormonen een rol. De basis van de klachten is een afsluiting van het kanaaltje dat het zweet uit de apocriene zweetklier naar de oppervlakte van de huid afvoert. Het zweet kan nu niet weg en een ontstekingsreactie gaat beginnen. Deze afsluiting kan worden veroorzaakt door hoornpropjes (comedonen, mee-eters) die vaak in het huidgebied dat aangedaan is door hidradenitis gevonden kunnen worden. Meestal spelen bacterieen bij deze ontsteking ook een rol, vooral de Stapylococcus aureus en Streptococcus milleri.

Hidradenitis suppurativa, ook wel acne inversa of acne ectopica genoemd, is een chronische ontsteking van de haarzakjes en talgklieren in vooral de liezen en/of de oksels. Hidradenitis suppurativa betekent etterende zweetklierontsteking, acne inversa betekent acne van de plooien, acne ectopica betekent acne op andere plaatsen dan de gewone acne. Ondanks de naam zijn het niet de zweetklieren maar vooral de talgklieren die ontstoken raken. De talgklieren zijn aangesloten op de haarzakjes en produceren vet dat de huid soepel houdt. Als de gezamenlijke uitvoergang van de haarzakjes en de talgklieren geblokkeerd raakt (door een hoornprop of door zwelling in de omgeving), ontstaat ophoping van talg. De talgklieren zwellen op en raken ontstoken. Diverse bacteriën kunnen zich gaan vermeerderen in de afgesloten ruimten. KLACHTEN Anders dan bij de gewone acne (acne vulgaris), ontstaan bij acne inversa uitgebreide ontstekingen in en rond de talgklieren, met pijnlijke zwellingen als gevolg. Soms zijn verwijde afvoergangen en puskopjes zichtbaar aan de buitenkant. Er kunnen grote en pijnlijke met pus gevulde abcessen ontstaan, die op een gegeven moment doorbreken naar buiten. In de diepte kunnen grote holtes (cysten) ontstaan. Soms zoekt de pus een uitweg via andere wegen dan de oorspronkelijke afvoergang en ontstaan gangen en tunnels (fistels) onder de huid, die soms centimeters verder weer naar de oppervlakte komen. Abcesholtes kunnen met elkaar in verbinding staan via gangen. Abcessen kunnen steeds opnieuw terugkomen op dezelfde plaats. In de ergste gevallen is acne inversa een zeer invaliderende ziekte, vanwege de pijn en de continue uitvloed van talg, vocht en pus uit de ontstoken klieren. Het kan aanleiding geven tot sociaal isolement en depressieve gevoelens.

A Brief Description of Hidradenitis Suppurativa Hidradenitis can get worse over a period of time; many of our members have multiple lesions on different parts of the body at the same time. On the other hand, there are members who flare only occasionally and/or don't progress. If HS does progress, fistulas and sinus tracking (under the skin) may form, causing even more problems. The pain and inflammation can be disabling. Experiences with HS are varied - some people go through experimental treatments (radiation), radical surgery (requiring skin grafts), the gamut of medications (long-term antibiotics, accutane), and/or frequent lancings. The sores are not typically infected; in studies, more than 50% of lesions cultured do not show any bacteria. But, it is important to note that cellulitis can take over areas, and this requires immediate medical attention. As you can imagine, many people who have HS deal with depression and physical pain on a daily basis. There are different theories about what causes HS - there appears to be a strong genetic link, and the cycles of the disease indicate hormonal imbalance(s). HS is often seen in conjunction with thyroid disease, polycystic ovarian syndrome, diabetes, crohn's disease, and several autoimmune disorders. We are always looking for reasons WHY and HOW the disease affects us - some people with HS have literally dedicated their lives going research and outreach about HS, hoping to find answers.
