A patient information handout on shingles, written by the authors of this article, is provided on page 2447. H erpes zoster results from reactivation of the varicella-zoster virus. Unlike varicella (chickenpox), herpes zoster is a sporadic disease with an estimated lifetime incidence of 10 to 20 percent. The incidence of herpes zoster increases sharply with advancing age, roughly doubling in each decade past the age of 50 years. Herpes zoster is uncommon in persons less than 15 years old. In a recent study, patients more than 55 years of age accounted for more than 30 percent of herpes zoster cases despite representing only 8 percent of the study population. In this same study, children less than 14 years old represented only 5 percent of herpes zoster cases. The normal age-related decrease in cell-mediated immunity is thought to account for the increased incidence of varicella-zoster virus reactivation. Patients with disease states that affect cell-mediated immunity, such as human immunodeficiency virus (HIV) infection and certain malignancies, are also at increased risk. Chronic corticosteroid use, chemotherapy and radiation therapy may increase the risk of developing herpes zoster.

Herpes is caused by a virus
Herpes simplex virus (HSV) causes herpes. This virus has two types, HSV 1 and HSV 2. HSV 1 (oral herpes) usually causes infections on the lips or mouth (cold sores or fever blisters). HSV 2 (genital herpes) usually causes sores and blisters in the genital area. However, virus from the mouth can infect the genitals and virus from the genitals can infect the mouth. Herpes virus can also infect other parts of the body. Both viruses can live inside nerve cells and cause symptoms that occur again and again. Almost everyone has had HSV 1 Fifty to 90% of adults have had HSV 1 infection. Infections in children are often mild or cause no symptoms at all. Even if you have had HSV 1, you can still catch HSV 2.

Herpes viruses are a leading cause of human viral disease, second only to influenza and cold viruses. They are capable of causing overt disease or remaining silent for many years only to be reactivated, for example as shingles. I suspect my Vitamin D malabsorption is caused by a liver problem, probably borderline cholestasis , since I've had progressively higher bilirubin since my teens and by 1999 my bilirubin direct was .23. The Vitamin D insufficiency, in turn, may prevent adequate absorption of calcium, leading to hypercalcemia, bone pain and a host of problems. Interestingly, cholestasis is sometimes a complication in pregnancy.

Genital Herpes What you need to know:
Genital herpes is a sexually transmitted infection that can be passed to an unborn child before, during or shortly after delivery. In most cases, this happens during vaginal delivery. Typical symptoms of genital herpes in adults are clusters of small blisters in the genital area.
Infected newborns can develop skin or mouth sores or eye infections. If the infection remains limited to these areas, most babies develop normally. Often, however, the infection spreads to the brain and other internal organs. About half of infants with widespread infection die, and the half who survive often have brain damage.
The risk is highest when a woman contracts herpes for the first time late in pregnancy.
What you can do: If you have been infected with genital herpes and are pregnant, talk to your provider. If you have symptoms at the time of delivery, the baby can usually be protected from infection by a cesarean delivery (c-section). A vaginal delivery is safe for most women with recurrent herpes if they do not have signs of infection at delivery. Some mothers, however, have symptomless infections and can pass herpes to their newborns. Your provider can discuss your delivery options with you.

Genital herpes is a sexually transmitted disease that can have a devastating impact on the health of newborn babies. Approximately 45 million Americans have genital herpes, according to the Centers for Disease Control and Prevention. About 500,000 new cases occur each year, including about 1,000 in newborn babies. While most women with genital herpes have healthy babies, a small number pass the virus on to their babies during labor and delivery. For this reason, it is especially important for pregnant women to recognize the symptoms of this disease, and to seek immediate medical treatment if they think they could be infected.
What Causes Herpes?
Herpes is caused by herpes simplex viruses, which are similar to the viruses that cause chickenpox and shingles. After the initial infection, herpes simplex viruses can hide within nerve cells, where the body’s immune system cannot reach them. Then, under the right conditions, the viruses can launch new attacks. There are two main kinds of herpes simplex viruses: type 1, which is usually associated with cold sores around the mouth and lips; and type 2, which is usually associated with genital sores. However, either type can infect oral or genital areas, and pregnant women with genital sores caused by either form of herpes simplex virus can pass the infection on to their babies.

    Introduction
    This infection is caused by the Herpes simplex virus. Genital infection may be caused by Type 1 (HSV-1) or Type 2 (HSV-2). The symptoms are similar, and can result from either oral-to-genital or genital-to-genital contact. The virus causes blisters on the genitals, similar to the cold sores that occur on the mouth. Cold sores on the mouth are also caused by the herpes virus. These infections are caused by viruses and definitive cures are not available. However, many patients are distressed by the recurrent nature of the infections and perceived serious complications.
    Statistics
    Because genital herpes is not a notifiable disease, there are no accurate statistics on its prevalence in South Australia. However, in recent years, approximately 2% of the clients with STDs diagnosed at Clinic 275 have had active episodes of the infection.
