Surgery for Parkinson Percutaneous Laser Diskectomy This procedure is done under local anesthesia, with a slight sedation. It consists on the passage of a needle through the skin (percutaneously), from the back, slightly laterally, to the center of the herniated disk. The position of the needle is constantly monitored with X-rays. Once the needle has been placed, a fiber optics endoscope is driven through it, to the center of the disk. This device allows the surgeon to see the nucleus and at the same time has another fiber to conduct the laser beam which will ablate, evaporate, the disk material. Once the surgeon is curtain that the space he has produced in the center of the disk is enough, the endoscope and needle are taken out. As this procedure is done under local anesthesia, the patient is taken immediately after to his room. Typically the patient is discharged walking from hospital the same day, a few hours after he was admitted. Going back to work, depending on the patients occupation, could be in 2 to 6 weeks.

This is demonstrated on the CT scan (fig. 2), the MRI (fig. 3) and the myelogram (fig. 4). Nearly all patients complain of leg pain (sciatica) in the distribution of one or more nerve roots. Some have back pain as well. Frequently, patients can be treated conservatively with success. However, when their ability to perform normal day to day functions is degraded for a substantial period of time (4-6 weeks), or in the presence of progressing neurological deficit (i.e. foot drop, loss of control of bowel and bladder, cauda equina syndrome), lumbar microdiskectomy is the most effective treatment. During microdiskectomy, the offending disk fragment and much of the loose nucleus are removed through a small, one to two inch incision (figs. 5-8). With success rates of around 90%, patients are able to return to work and full activity in an average of about 10 weeks. Our group has the most extensive experience in Western New York performing outpatient microdiskectomy. In the past 3 years, we have performed more than 1000 of these procedures successfully. HERNIATED LUMBAR DISK Patients under treatment by their own physician who fail to improve after two to four weeks - refer to an Orthopedic Surgeon or Neurosurgeon for consultation and/or treatment. I. BACKGROUND Herniations occur most commonly through a posterolateral defect, but midline herniations may occur. Resulting compression of the spinal nerve root causes inflammation and pain, usually along the anatomic course of the nerve, In the lumbar spine, this most often occurs at the L4 and L5 disk levels, causing pressure on the corresponding L5 and S1 nerve roots. As a result of both mechanical and biochemical changes around the nerve root, the patient will experience pain, paresthesia, and possibly weakness in the leg or legs, usually below the knee. The rare herniations at the L1, L2 and L3 levels are usually associated with pain, paresthesia and weakness above the knee.
