CONGENITAL DIAPHRAGMATIC HERNIA What is Congenital Diaphragmatic Hernia? Your baby has a problem called "Congenital Diaphragmatic Hernia" or CDH. Congenital means "born with" and a hernia is a problem where something goes through a hole it is not supposed to. The hole is in your baby's diaphragm. The diaphragm is a muscle that helps us to breathe and keeps the organs in the abdomen from going into the chest cavity. This is a picture of how things are normally: When your baby was very small (early in the pregnancy), there was a hole in the diaphragm. This is normal, but the hole usually closes by the end of the third month of pregnancy. In your baby's case, the hole stayed open. This allowed some of the intestines to go into the chest cavity. Since the intestines were in the chest cavity and not where they were supposed to be, the lungs could not grow in the normal space that they need (they had to share the space with the intestines). This means that the lungs are smaller than they should be. The intestines can also push on the other lung and keep it from growing fully and can sometimes keep the heart from growing normally. INGUINAL HERNIA What is a hernia? A hernia is the bulging of a portion of an organ through an abnormal opening. The danger from herniation arises when the organ protruding through the opening is constricted to the extent that circulation is stopped or when the protruding organ impairs the function of other structures. What is an inguinal hernia? Inguinal hernias account for about 80% of all hernias and are the most common surgical procedures done in infancy. These hernias appear more frequently in boys than in girls. An inguinal hernia is derived from persistence of all or part of the processus vaginalis, the tube of peritoneum that precedes the testicle into the scrotum during the eighth month of pregnancy. Following the dropping of the testicles into the scrotum, the processus vaginalis withers and closes forming the tunica vaginalis that lies below the testicles in the scrotum. When this fails to happen, fluid from the abdomen or an abdominal organ (usually the intestines) can be forced into it causing a bulging or mass that can be felt. The process vaginalis can extend only partly from the inguinal canal or extend completely into the scrotum. Normal scrotum:

Local Anesthesia for Inguinal Hernia Repair (Originally posted 22 September 1997 on About Anesthesiology) INTRODUCTION
Many practitioners and institutions routinely and successfully employ local anesthesia for inguinal hernia repairs. Although there are certainly other options (including general anesthesia and regionl anesthesia such as spinal or epidural), local offers some unique advantages to the patient. As such, here is a short summary about the use of local for these operations including a discusssion of advantages and disadvantages. Obviously, the choice of anesthesia will be influenced by patient preferences and needs. In addition, to complete any operation successfully under local anesthesia requires a surgeon who is comfortable with the technique and willing to stop surgery and place additional local anesthesia should this become necessary. Interestingly with inguinal hernia repair it is rarely, if ever, necessary to supplement with additional injections once the initial infiltration is complete. Reported experiences with it have been uniformly positive. Advantages of Local Anesthesia
Minimal physiological disturbance which may be an advantage in the patient in whom you wish to avoid a general anesthetic and in whom a regional technique may be contraindicated. This could include patient's with serious cardiac or respiratory diseases which could tolerate other types of anesthesia but would be at reduced risk if given only local anesthesia.

What is Heartburn? Potentially dangerous confusion can arise when someone neglects their heart pain believing it to be heartburn. What causes Heartburn? Acid is present in the stomach to digest food. Heartburn occurs when small amounts of this acid rise up into the gullet (oesophagus) - the tube which carries food from the mouth to the stomach. This is called reflux. The gullet, unlike the stomach, does not have a protective lining. So when it is exposed to the acid, it can become inflamed and painful.
Why doesn't everyone get Heartburn? Some known factors that can lead to heartburn include:
  • pregnancy smoking eating large meals, especially near bedtime being overweight bending a lot wearing tight clothing around the waist
In most of these cases, it is the increased pressure which overcomes the normal muscle contraction and causes heartburn. Smoking, on the other hand, relaxes the muscle around the gullet, with the same result. What is Hiatus Hernia?

Laparoscopic hernia repair is a technique to fix tears or openings in the abdominal wall using small incisions, laparoscopes (small telescopes inserted into the abdomen) and a patch (screen or mesh) to reinforce the abdominal wall. It may offer a quicker return to work and normal activities with decreased pain for some patients. WHAT IS A VENTRAL HERNIA?
  • When a ventral hernia occurs, it usually arises in the abdominal wall where a previous surgical incision was made. In this area the abdominal muscles have weakened; this results in a bulge or a tear. In the same way that an inner tube pushes through a damaged tire, the inner lining of the abdomen pushes through the weakened area of the abdominal wall to form a balloon-like sac. This can allow a loop of intestines or other abdominal contents to push into the sac. If the abdominal contents get stuck within the sac, they can become trapped or "incarcerated." This could lead to potentially serious problems that might require emergency surgery.
