These contents, usually portions of intestine or abdominal fatty tissue, are often enclosed in the thin membrane that naturally lines the inside of the cavity. Although the term hernia can be used for bulges in other areas, it most often is used to describe hernias of the lower torso (abdominal wall hernias). Hernias by themselves usually are harmless, but nearly all have a potential risk of having their blood supply cut off (becoming strangulated). If the hernia sac contents have their blood supply cut off at the hernia opening in the abdominal wall, it becomes a medical and surgical emergency.

Daily Variety reports that production for "The Man Who Killed Don Quixote" the $32 million project directed by Terry Gilliam 12 Monkeys ") and co-starring Johnny Depp has been indefinitely halted after the film's title character, French actor Jean Rochefort , dropped out of the picture due to a double disk hernia.
News of Rochefort 's illness came only after three weeks of principal photography for the film. The 70-year-old French star might or might not return to the project pending his condition.

This opening usually is large enough to accommodate the esophagus alone. With weakening and enlargement however, the opening (or herniation) can allow upward passage or even entrapment of the upper stomach above the diaphragm.

A hernia is an opening or weakness in the muscular structure of the wall of the abdomen. This defect causes a bulging of the abdominal wall. This bulging is usually more noticeable when the abdominal muscles are tightened, thereby increasing the pressure in the abdomen. Examples of activities that can worsen a hernia are lifting, coughing, or even straining to have a bowel movement. Imagine a barrel with a hole in its side and a balloon that is blown up inside the barrel. Part of the inflated balloon would bulge out through the hole. The balloon going through the hole is like the tissues of the abdomen bulging through a hernia. Symptoms of a hernia include pain or discomfort and a localized swelling somewhere on the surface of the abdomen or in the groin area. Serious complications from a hernia result from the trapping of tissues in the hernia a process called incarceration. Trapped tissues may have their blood supply cut off, leading to damage or death of the tissue. The treatment of incarceration requires surgery.

What is a hiatal hernia?
A hiatal hernia is an anatomical abnormality in which part of the stomach protrudes through the diaphragm and up into the chest. Although hiatal hernias are present in approximately 15% of the population, they are associated with symptoms in only a minority of those afflicted. Normally, the esophagus or food tube passes down through the chest, crosses the diaphragm, and enters the abdomen through a hole in the diaphragm called the esophageal hiatus. Just below the diaphragm, the esophagus joins the stomach. In individuals with hiatal hernias, the opening of the esophageal hiatus (hiatal opening) is larger than normal, and a portion of the upper stomach slips up or passes (herniates) through the hiatus and into the chest. Although hiatal hernias are occasionally seen in infants where they probably have been present from birth, most hiatal hernias in adults are believed to have developed over many years. As Professor of Surgery and Consultant Surgeon at the Royal Surrey County Hospital and the University of Surrey in Guildford, Michael Bailey is at the forefront of modern abdominal surgery. A pioneer of Laparoscopic surgery and its safe training, Professor Bailey performed one of the first laparoscopic gallbladder removal operations in the UK in 1990 and has since become internationally recognised for his technical and teaching expertise. He has evolved techniques for operating on many other conditions, including inguinal hernia repair and gastro-oesophageal reflux disease. In Guildford he has established and is Director of the Minimal Access Therapy Training Unit, which teaches and researches keyhole surgery. He is also President of the Association of Endoscopic Surgeons of Great Britain and Ireland, and Minimal Invasive Tutor at the Royal College of Surgeons of England. Apart from laparoscopic surgery, Professor Bailey's clinical interests are in gastrointestinal malignancy and thyroid surgery.

You have probably heard the word "hernia" many times but, like most people, when you try to define it, you may find your understanding of it a bit fuzzy. And, like many parents, you may think the condition has little to do with children. But hernias are often found in children, even in babies (especially premature babies ), who can be born with them. In fact, hernia repair is the most common surgery performed on children. What Is a Hernia?
When part of a bodily organ (such as a loop of intestine ) or some abdominal membrane or other tissue pushes through an opening or weak spot in a muscle wall, it can protrude into a space where it does not belong. This protrusion is called a hernia. Hernias look like a bulge or lump. How Does a Child Get a Hernia?
Some babies are born with various small openings in the body that are supposed to close but have not closed yet. Nearby tissues can squeeze into such an opening and become a hernia. Sometimes tissues can squeeze through muscle wall openings that are only meant for arteries or other special channels. In other cases, a child suffers a strain or injury that creates a weak spot in a muscle wall. Part of a nearby organ or other tissues can be pushed into the weak spot so that it bulges and becomes a hernia.

Introduction Local anaesthesia may be employed in hernia operations, either on its own or combined with general anaesthesia. The choice of technique will be influenced not only by local resources and skills, but also by patient preference. Advantages of Local Anaesthesia for Hernia Repair With a careful technique, local anaesthesia causes minimal physiological disturbance. This may be particularly useful for patients with cardiovascular or respiratory disease for whom there may be advantages in avoiding a general anaesthetic. The absence of postoperative sedation or drowsiness allows early ambulation and diminishes the requirement for recovery facilities. Local anaesthesia provides postoperative analgesia for up to four hours and may be administered by the surgeon. When adrenaline is mixed with the local anaesthetic (normally in a dilution of 1:200,000) useful vasoconstriction is produced resulting in a relatively bloodless field.
Disadvantages of Local Anaesthesia for Hernia Repair Surgery on the awake patient under local anaesthesia must be carried out gently. Although pain sensation is usually blocked by the anaesthetic, traction on certain tissues, particularly the peritoneum, is uncomfortable. The patient should be warned that some sensation may be experienced during the operation but that it will not be painful. Larger hernias, particularly those with incarcerated bowel may prove unsuitable for local anaesthesia.
