Hereditary angioedema is an inherited abnormality of the immune system that causes swelling , particularly of the face, and abdominal cramping. Also, see angioedema
Causes and Risk
Hereditary angioedema is an inherited disorder. It is caused by the absence of a protein called C1 inhibitor (or a defect in this protein), which in turn effects blood vessels. People with hereditary angioedema can develop rapid swelling of the hands, feet, limbs, face, intestinal tract, or airway (larynx or trachea). Unlike an allergic reaction , there is usually no itching or hives . Swelling of the gut can cause intestinal cramping. If swelling closes the airway, it can be fatal. Attacks of swelling can become more severe in late childhood and adolescence. A family history is usually noted, at times only reported to descendants as an unexpected, sudden, and premature death of a parent, aunt/uncle, or grandparent.

JERINI ANNOUNCES POSITIVE PHASE II STUDY RESULTS WITH ICATIBANT FOR THE TREATMENT OF HEREDITARY ANGIOEDEMA Berlin/Germany, January 13, 2004 - Jerini today announced positive results from its POC phase II study to evaluate its lead compound Icatibant (JE049) for the treatment of acute attacks of hereditary angioedema (HAE). Angioedema symptoms resolved rapidly in all eight patients, and the treatment response was sustained over the whole observation period. Together with the excellent safety of the drug, the study outcome confirms that Icatibant can be of significant benefit for treatment of acute attacks of hereditary angioedema.
In this European trial, a single dose of Icatibant was given intravenously to eight patients with acute angioedema attacks. Icatibant was very well tolerated by all patients. No drug-related adverse events were reported.
Icatibant is a bradykinin (BK) 2 receptor antagonist that in contrast to other agents acts directly on the effects of bradykinin causing rapid symptom relief.

Angioedema. hereditary angioedema (HAE) results from absent (type I) or defective (type II) C1 esterase inhibitor (C1INH). It is an autosomal dominant trait.

Complement Defects Deficiencies in complement components Patients with deficiencies in each of the classical and alternative pathway components have been identified. However, compared to the other immune deficiencies, deficiencies in complement components are relatively rare comprising only about 2-4% of all immune deficiencies. Deficiencies in the components themselves are all autosomal recessive. They are often associated with other immune abnormalities, particularly systemic lupus erythematosis and glomerulonephritis. The alternative pathway is the evolutionarily older pathway, and provides the first line of defense against infections. Patients with deficiencies in components of this pathway do present with increased susceptibility to infections with high grade pathogens such as streptococci and H. influenza . In addition, patients with deficiencies in factor D or in properdin (X-linked recessive) decreases clearance of gram negative bacteria with increased incidence of Neisseria infections. Deficiencies in early components (C1q, C1rs, C4 or C2) do not pose as great a problem as those in the alternative pathway probably because of the redundance of the host defenses. However, long term observation of patients with defects in these early components are at greater risk of dying of overwhelming infections (again with high grade pathogens) as other defenses are compromised. These defects are also associated with increased incidence of autoimmune collagen vascular diseases such as systemic lupus, but the connection is not known.

Long-term therapy of hereditary angioedema with danazol.

We treated 69 patients who had hereditary angioedema with danazol to alleviate attacks of mucocutaneous angioedema involving the skin, oropharynx, and gastrointestinal tract, and we documented the continued efficacy of danazol for long-term treatment (1 to 6 years) of hereditary angioedema. Significant dose-related, adverse reactions occurred, including weight gain, myalgias, headaches, microscopic hematuria, abnormal liver function tests, anxiety, altered libido, alopecia , dizziness, and nausea. Alterations in menstrual function were consistently observed. About 10% of patients noted masculinizing side effects, such as acne, hirsutism, and voice deepening. We recommend downward titration of danazol dosage to achieve control of attacks and minimize adverse reactions. Periodic monitoring of patients on long-term danazol therapy is essential to avoid undesirable toxicity.
