It is caused by the absence of a protein called C1 inhibitor (or a defect in this protein), which in turn effects blood vessels. People with hereditary angioedema can develop rapid swelling of the hands, feet, limbs, face, intestinal tract, or airway (larynx or trachea). Unlike an allergic reaction , there is usually no itching or hives . Swelling of the gut can cause intestinal cramping. If swelling closes the airway, it can be fatal. Attacks of swelling can become more severe in late childhood and adolescence. A family history is usually noted, at times only reported to descendants as an unexpected, sudden, and premature death of a parent, aunt/uncle, or grandparent.

Treatment Try to prevent attacks by avoiding trauma and precipitating factors. In contrast to other causes of angioedema (e.g., from allergies), antihistamines and other related treatments are of limited benefit in hereditary angioedema. Attenuated androgens, which are derivatives of normal sex hormones, can reduce the frequency and severity of attacks. Once an attack occurs, treatment includes pain relief and intravenous fluids. A new treatment using C1-inhibitor concentrate is being tested.

Genzyme Corp. and Dyax Corp. have announced a joint venture to continue the development of a drug to treat hereditary angioedema, a rare disease that causes attacks of subcutaneous, abdominal and/or laryngeal swelling. Background: Hereditary angioedema (HAE) is an autosomal dominant disorder manifested by painless, nonpruritic swelling of the skin. Type I HAE is defined by low plasma levels of a normal C1 inhibitor (C1-INH) protein. Type II HAE is characterized by the presence of normal or elevated levels of a dysfunctional C1-INH. Pathophysiology: The gene for C1-INH has been mapped to chromosome 11 (11q12-q13.1). C1-INH is a multifunctional serine protease inhibitor that is normally present in high concentrations in plasma. It is the only known plasma inhibitor of C1r and C1s, the activated proteases of the first component of complement. It is also the major plasma inhibitor of activated Hageman factor, the first protease in the contact system. Additionally, C1-INH is one of the major inhibitors of plasma kallikrein, the contact system protease that cleaves kininogen and releases bradykinin. Presumably, uncontrolled activation of the contact system allows for the release of kininlike mediators, resulting in edema of subcutaneous or submucosal tissues. HAE is due to mutations within the C1-INH gene and is transmitted as an autosomal dominant trait. Two variants of HAE exist: type I (85%) and type II (15%). Type I HAE is characterized by low antigenic and functional plasma levels of a normal C1-INH protein. Type II HAE is characterized by the presence of normal or elevated antigenic levels of a dysfunctional mutant protein together with reduced levels of the functional protein. C1-INH deficiency allows autoactivation of the first component of complement (C1) with consumption of C4 and C2.

General Discussion
Hereditary Angioedema is a rare inherited disorder characterized by an accumulation of fluids outside of the blood vessels, blocking the normal flow of blood or lymphatic fluid and causing rapid swelling of tissues in the hands, feet, limbs, face, intestinal tract, or airway. Usually, this swelling is not accompanied by itching, as it might be with an allergic reaction. Swelling of the gastrointestinal tract leads to cramping. Swelling of the airway may lead to obstruction, a potentially very serious complication. These symptoms develop as the result of deficiency or improper functioning of certain proteins that help to maintain the normal flow of fluids through very small blood vessels (capillaries). In some cases, fluid may accumulate in other internal organs. The severity of the disease varies greatly among affected individuals.
