Hepatitis A
Erreger: HEPATITIS A-VIRUS (RNA-Virus). Infektionsquelle: Mensch. Übertragung: Fäko-oral, d. h. bei fehlender Wasser-, Abwasser- und Abfallhygiene über kontaminiertes Trinkwasser, Gemüse, Früchte und Meerestiere (bes. Muscheln). Inkubationszeit: 3 bis 6 Wochen. Krankheitsbild: Beginn mit uncharakteristischen Allgemeinbeschwerden wie Übelkeit und Erbrechen, eventuell Fieber, Kreuzschmerzen, Müdigkeit. In der Folge kommt es zum Auftreten einer Gelbsucht (Gelbfärbung von Haut und Augen) mit hellem Stuhl und dunklem Harn. Im Rahmen der Laboruntersuchungen ist eine deutliche Erhöhung der Leberfunktionsparameter (SGOT und SGPT, Bilirubin) sichtbar. Ganz besonders bei Kindern verläuft die Hepatitis A oft ohne die klassische Gelbsucht, es treten nur milde Symptome wie bei einem grippalen Infekt auf. Bei der Hepatitis A gibt es keine chronischen Verläufe, die Erkrankung heilt – nach einer mehrwöchigen Erkrankungsphase – komplett aus. Im höheren Lebensalter kann es aber auch bei der Hepatitis A zu einem akuten Leberversagen und damit zum Tod kommen. Diagnose: Die Diagnosestellung erfolgt ausschließlich serologisch (Hepatitis A IgG und IgM-Antikörperuntersuchung). Klinik und Befunde lassen keine Unterscheidung von anderen Hepatitisformen zu. Nur in der Frühphase der Erkrankung ist auch der Virusnachweis im Stuhl möglich.

MONACA, Pennsylvania (AP) State and federal health officials said Saturday that Pennsylvania's hepatitis A outbreak is winding down, even as the number of those infected climbed over 600. The investigation has shifted to how green onions linked to the outbreak became contaminated. Sen. Arlen Specter, R-Pennsylvania, met Saturday with Dr. Julie Gerberding, director of the Centers for Disease Control and Prevention, and state health officials at a community college near the Beaver County Chi-Chi's restaurant where the outbreak was first confirmed November 3. "We do feel like this particular outbreak has been successfully ended," Gerberding said, although the number of cases probably will rise over the next few days as lab test results come back.

WARNINGS AND CONTRAINDICATIONS
Anemia associated with therapy may exacerbate symptoms of coronary disease or deteriorate cardiac function. It is advised that complete blood counts (CBC) be obtained at baseline and at weeks 2 and 4 of therapy or more frequently if clinically indicated.
The most common adverse experiences associated with therapy are "flu-like" symptoms, such as headache, fatigue, myalgia, and fever, which appear to decrease in severity as treatment continues. Severe psychiatric adverse events, including depression, psychoses, aggressive behavior, hallucinations, violent behavior (suicidal ideation, suicidal attempts, suicides), and rare instances of homicidal ideation have occurred during combination REBETOL/INTRON A therapy, both in patients with and without a previous psychiatric disorder.
Combination REBETOL/INTRON A therapy must not be used by women, or male partners of women, who are or may become pregnant during therapy and during the 6 months after stopping therapy. Combination REBETOL/INTRON A therapy should not be initiated until a report of a negative pregnancy test has been obtained immediately prior to initiation of therapy. Women of childbearing potential and men must use effective contraception (two reliable forms) during treatment and during the 6-month posttreatment follow-up period. Significant teratogenic and/or embryocidal effects have been demonstrated for ribavirin in all animal species in which adequate studies have been conducted. These effects occurred at doses as low as one twentieth of the recommended human dose of REBETOL. If pregnancy occurs in a patient or partner of a patient during treatment or during the 6 months after treatment stops, physicians are encouraged to report such cases by calling (800) 727-7064.

Herausgegeben vom Robert Koch-Institut Erreger Vorkommen/Verbreitung In der Inkubationszeit und teilweise in der Akutphase können HCV-Infektionen mit Antikörper-Bestimmungsmethoden nicht erfasst werden. Diese Zeiträume werden als "diagnostisches Fenster" bezeichnet und betragen im Mittel drei Monate. Sie begründen das Restrisiko beim Einsatz von Blut und bei nicht inaktivierten Blutprodukten. In Einzelfällen ließen sich nach HCV-Infektionen HCV-Antikörper erst nach neun Monaten nachweisen. Ein weiterer wichtiger gesicherter Übertragungsweg ist der gemeinsame Gebrauch von Spritzen und Kanülen (das so genannte "needle-sharing", der Nadel- bzw. Spritzentausch) unter i.v. (intravenös) Drogenabhängigen. Bei der Risikogruppe der i.v. Drogenabhängigen wurden Infektionsraten gefunden, die bis zu 90% erreichen. Auch bei Strafgefangenen, unter denen sich ein hoher Anteil von Drogenabhängigen befindet, wurden in 12,5% Hepatitis C-Infektionen nachgewiesen. www.bgvv.de

  • HCV infektion giver anledning til HCV replikation primært i leveren. HCV er direkte cytotoksisk. Inflammation og nekrose er under den primære infektion ofte begrænset dvs. de fleste primære infektioner forløber subklinisk. Afgørende for evt. klinisk infektion er inoculums størrelse. 70-80% udvikler kronisk infektion. Hos det mindretal som ikke får kronisk infektion, giver anti-HCV ikke anledning til immunitet. Fulminant forløbende HCV infektion er beskrevet, men yderst sjældent. Progressions hastigheden af kronisk infektion er yderst varierende.1/3 vil få cirrose indenfor 20 år - 1/3 vil aldrig få cirrose. Samtidig HIV infektion betyder dårligere prognose.
