How Ideal Is the IDEAL Trial?

By Daniel Raymond and Tracy Swan
Introduction/Background
Pegasys- or Peg-Intron-based combination therapy? Which is better? This is a key question that people with chronic hepatitis C and their physicians are asking every day when considering treatment with pegylated interferon and ribavirin.
Peg-Intron is the Schering-Plough version of pegylated interferon; Roche manufactures Pegsys.  The two brands of pegylated interferon have never been directly compared to each other in the same study, although in data from large clinical trials, it appears that their efficacy and side effects profile are similar.  However, without a head-to-head comparison, it’s not possible to compare results across different trials.  Major studies of pegylated interferon and ribavirin treatment have differed according to the dose of ribavirin used, the characteristics of people in the trial (age, weight, amount of liver damage), and the way side effects are managed.  That leaves little basis for making decisions about which brand of pegylated interferon is preferable. Now Schering –Plough has launched a large new clinical trial, called IDEAL, that includes a promise to provide that information.

In a Large US Cohort, Race Had No Effect on Survival Time of Liver Transplant Recipients

Previous analyses of data from the United Network for Organ Sharing (UNOS) showed that African American (AA) patients undergoing liver transplantation (OLT) had survival significantly inferior to recipients of other races ( Lancet 2002). The reason for this disparity is unclear.

Of the cases linked to a Chi-Chi's Restaurant about 25 miles northwest of Pittsburgh, 31 were in Ohio, eight in West Virginia, one in Florida, and one in South Carolina. The outbreak is believed to have begun as early as September, state Health Department spokesman Richard McGarvey said. The restaurant voluntarily closed November 2, and has not reopened. Seattle lawyer William Marler said his firm has already filed a lawsuit against the restaurant.

The web version below contains the full text of this item without the original formatting. Men who have sex with men (MSM) are at increased risk of becoming infected with both the hepatitis B virus (HBV) and the hepatitis A virus (HAV). Although these viruses can be transmitted in different ways, both can be spread through sexual activity. Hepatitis is a serious disease that can be fatal. Fortunately, both hepatitis B and hepatitis A can be prevented by safe and effective vaccines. Unfortunately, many men at risk remain unprotected. What is hepatitis B?
Hepatitis B is a sexually transmitted disease caused by a virus (HBV) that attacks the liver. The virus is found in the blood and semen of infected men and is spread in the same manner as HIV. HBV is easier to catch than HIV because it is more than 100 times more concentrated in an infected person's blood and can exist on surfaces outside the body. What is hepatitis A?

Low hepatitis vaccine rates found among Vietnamese-American kids
CHICAGO (AP) A study of three large Vietnamese-American communities found extremely low rates of vaccination among youngsters for hepatitis B, a sometimes fatal liver disease that is prevalent among Vietnamese adults. Poor access to health care as well as treatment by doctors whose training in Vietnam did not emphasize preventive care were among possible reasons cited for the low rates.

    1. Name of the Organism:
Hepatitis A Virus Hepatitis A virus (HAV) is classified with the enterovirus group of the Picornaviridae family. HAV has a single molecule of RNA surrounded by a small (27 nm diameter) protein capsid and a buoyant density in CsCl of 1.33 g/ml. Many other picornaviruses cause human disease, including polioviruses, coxsackieviruses, echoviruses, and rhinoviruses (cold viruses). 2. Nature of Acute Disease: The term hepatitis A (HA) or type A viral hepatitis has replaced all previous designations: infectious hepatitis, epidemic hepatitis, epidemic jaundice, catarrhal jaundice, infectious icterus, Botkins disease, and MS-1 hepatitis. 3. Nature of Disease: CDC Case Definition What is a "Case Definition"? Overview of Public Health Surveillance Hepatitis A is usually a mild illness characterized by sudden onset of fever, malaise, nausea, anorexia, and abdominal discomfort, followed in several days by jaundice. The infectious dose is unknown but presumably is 10-100 virus particles. 4. Diagnosis of Human Illness:

    Hepatitis E Virus
    1. Name of the Organism:
Hepatitis E Virus Hepatitis E Virus (HEV) has a particle diameter of 32-34 nm, a buoyant density of 1.29 g/ml in KTar/Gly gradient, and is very labile. Serologically related smaller (27-30 nm) particles are often found in feces of patients with Hepatitis E and are presumed to represent degraded viral particles. HEV has a single-stranded polyadenylated RNA genome of approximately 8 kb. Based on its physicochemical properties it is presumed to be a calici -like virus. 2. Nature of Acute Disease: The disease caused by HEV is called hepatitis E, or enterically transmitted non-A non-B hepatitis (ET-NANBH). Other names include fecal-oral non-A non-B hepatitis, and A-like non-A non-B hepatitis. Note: This disease should not be confused with hepatitis C, also called parenterally transmitted non-A non-B hepatitis (PT-NANBH), or B-like non-A non-B hepatitis, which is a common cause of hepatitis in the U.S. 3.
