of 3 While I knew all about the potential for fatigue and the sore breasts from engorgement prior to my labor, there were other post-labor issues that no one had ever told me about (and for which I was completely unprepared): Bleeding (a.k.a., Lochia)
No one had warned me about how much and for how long I would bleed after giving birth. Lochia , or postpartum bleeding and discharge, can last for up to six weeks post-delivery. After the birth of my son, I had what equated to the heaviest period of my life for at least two weeks running. During this time I had to wear maxi-size pads (tampons are not allowed postpartum for obvious reason) and I had to sleep on special moisture barrier squares to protect our mattress. Breastfeeding actually encourages lochia because it causes your uterus to shrink and push out whatever happens to be left in there, including blood clots, the size of which can be alarming. However, breastfeeding also shortens the lochia time, which averages about 21 days. This is one of the reasons most women are encouraged to nurse for at least the first month of their infant's life: it's beneficial to both mother and baby. As time passes, the lochia will change in color from bright red to brown to yellowish before it finally stops all together. Call your doctor if you have any concerns about the amount of lochia your body produces, especially if you are seeing fairly large clots, believe your bleeding is excessive or the lochia develops a foul odor.

"I have avoided going to the doctor for my hemorrhoids by using over the counter remedies, but it is becoming unbearable. I eat a high-fiber diet but that has not helped either. Is there anything else I can do before I make that embarrassing doctor's appointment?" rf Hemorrhoids are swollen veins just inside the anus. They produce pain if they come out of the anus with a bowel movement because they are constricted. Once they come out, the skin around them and the veins themselves can become inflamed. It sounds as if you have tried diet modification to prevent constipation. What other creams or rectally applied compounds have you used? It would also be good to know what your occupation is and what your weight is. These can be factors that contribute to development of hemorrhoids. What causes hemorrhoids and their symptoms? Sometimes the veins just inside the anus become swollen if there is pressure on the vein itself to prevent blood flow from returning to the heart. The veins may become constricted by a large amount of stool that stays in the rectum just above the anus in someone who has constipation. A job in which a person sits all of the time can produce hemorrhoids just as a person who is on their feet may get varicose veins of the leg. Straining and sitting a long time on the toilet trying to have a bowel movement is also thought to produce hemorrhoids. Chronic diarrhea causes hemorrhoids by the same mechanism.

Pea-Boy Dear Pea-Boy, Although it can be embarrassing, it is important for you and your peace of mind to get your anal bump diagnosed by a health care provider. For example, if it is a wart, you do not have to treat it with a hemorrhoid medication. Hemorrhoids, enlarged veins in the lower rectum and anus, are common and not serious. Straining to have a bowel movement, constipation, a low-fiber diet, pregnancy, overuse of laxatives, and prolonged sitting can cause hemorrhoids. Once hemorrhoids develop, they tend to persist, although they may not cause major problems. Discomfort and swelling may come and go, usually lasting between three to five days. Common symptoms of hemorrhoids include:
  • pain or itching near the growth in the anus
  • bright red blood at the end of a bowel movement or on the toilet paper
  • excessively moist anal area

What are Hemorrhoids?
Hemorrhoids are swollen blood vessels in and around the anus and lower rectum that enlarge under pressure, similar to varicose veins in the legs. Increased pressure and swelling may result from straining to move the bowel. Other contributing factors include pregnancy, heredity, aging, and chronic constipation or diarrhea. Hemorrhoids may occur either inside the anus (internal) or under the skin around the anus (external). The most common symptom of internal hemorrhoids is bright red bleeding during bowel movements. Internal hemorrhoids may protrude through the anus outside the body, becoming irritated and painful. Symptoms of external hemorrhoids may include painful swelling or a hard lump around the anus that results when a blood clot forms. Notably, the internal anal sphincter (IAS) of patients with hemorrhoids demonstrates an abnormal rhythm of contraction and also exerts a greater force of contraction than in individuals without hemorrhoids. Whether this sphincter abnormality is a cause or an effect of hemorrhoids is not known. However, an overactive sphincter might contribute to venous congestion, expose the anal cushions to greater shear forces, or do both by constricting the anal canal. Nitroglycerin is known to relax the internal anal sphincter, and smooth muscle in general, by acting as a nitric oxide donor. Cellegy is therefore exploring the possibility that nitroglycerin ointment may provide pain relief and possibly decreased bleeding for patients suffering from hemorrhoids. Such clinical studies are currently underway.
