RNA viruses from several different viral families: the Filoviridae, which consists of Ebola and Marburg viruses; the Arenaviridae, including Lassa fever, Argentine and Bolivian hemorrhagic fever viruses; the Bunyaviridae, including various members from the Hantavirus genus, Congo-Crimean hemorrhagic fever virus from the Nairovirus genus, and Rift Valley fever from the Phlebovirus genus; and Flaviviridae, such as Yellow fever virus, Dengue (hemorrhagic fever) virus, and others. Signs and Symptoms: VHFs are febrile illnesses which can be complicated by easy bleeding, petechiae, hypotension and even shock, flushing of the face and chest, and edema. Constitutional symptoms such as malaise, myalgias, headache, vomiting, and diarrhea may occur in any of the hemorrhagic fevers. Diagnosis: Definitive diagnosis rests on specific virologic techniques. Significant numbers of patients with a hemorrhagic fever syndrome should suggest the diagnosis of a viral hemorrhagic fever. Treatment: Intensive supportive care may be required. Antiviral therapy with ribavirin may be useful in several of these infections. Convalescent plasma may be effective in Argentine hemorrhagic fever.

How are hemorrhagic fever viruses associated with bioterrorism? Some evidence exists for respiratory transmission of certain VHF viruses, certainly most cause high infections rates by direct contact. Some types of hemorrhagic fever viruses can cause relatively mild illnesses, but many of these viruses cause severe, life-threatening disease. The Special Pathogens Branch (SPB) primarily works with hemorrhagic fever viruses that are classified as biosafety level four (BSL-4) pathogens. The Division of Vector-Borne Infectious Diseases, also in the National Center for Infectious Diseases, works with the non-BSL-4 viruses that cause two other hemorrhagic fevers, dengue hemorrhagic fever and yellow fever.

Viral Hemorrhagic Fevers
Viral Hemorrhagic Fevers are a group of illnesses caused by several distinct families of viruses. The term "viral hemorrhagic fever" describes illnesses such as ebola that affect multiple organ systems. The illnesses associated with these viruses range from mild to life threatening. The viruses associated with VHFs do not naturally reside in humans, but humans can become infected when they come into contact with an animal that hosts the virus. People can transmit some of these diseases to others through close contact. Rodents and arthropods are frequently the hosts of these viruses. However, the hosts of some viruses, including the ebola virus, are not known. Traditionally, the presence of each of these viruses was contained to the area where the host animal lived. But this is beginning to change due to the increasing frequency with which people travel. With a few exceptions, there are no cures or established drug treatments for VHFs.

The viral hemorrhagic fevers are limited to relatively small endemic outbreaks. Smallpox has not been diagnosed for 25 years. The highly infectious nature of these diseases, a lack of good vaccine candidates for viral hemorrhagic fever, and few treatment options portend grave outcomes should these agents be released on a mass scale. The supportive measures that we rely on to treat the viral hemorrhagic fevers would quickly become unavailable because hospital surge capacity would be reached with even a small outbreak. Quarantine and ring vaccination are currently the only reliable measures to prevent a global disaster in the event of the intentional release of one of these agents.     Hemorrhagic fevers are caused by viruses that exist throughout the world. However, they are most common in tropical areas. Early symptoms, such as muscle aches and fever, can progress to a mild illness or to a more debilitating, potentially fatal disease. In severe cases, a prominent symptom is bleeding, or hemorrhaging, from orifices and internal organs.
    Description
    Although hemorrhagic fevers are regarded as emerging diseases, they probably have existed for many years. This designation isn't meant to imply that they are newly developing, but rather that human exposure to the causative viruses is increasing to the point of concern. These viruses are maintained in nature in insect, arthropod (insects, spiders and other invertebrates with external hard skeletons), or animal populationsso-called disease reservoirs. Individuals within these populations become infected with a virus but do not die from it. In many cases, they don't even develop symptoms. Then the viruses are transmitted from a reservoir population to humans by vectorseither members of the reservoir population or an intervening species, such as mosquitoes.

Significant numbers of military personnel with a hemorrhagic fever syndrome should suggest the diagnosis of a viral hemorrhagic fever. Treatment: Intensive supportive care may be required. Antiviral therapy with ribavirin may be useful in several of these infections (Available only as IND under protocol). Convalescent plasma may be effective in Argentine hemorrhagic fever (Available only as IND under protocol). Prophylaxis: The only licensed VHF vaccine is yellow fever vaccine. Prophylactic ribavirin may be effective for Lassa fever, Rift Valley fever, CCHF, and possibly HFRS (Available only as IND under protocol). Isolation and Decontamination: Contact isolation, with the addition of a surgical mask and eye protection for those coming within three feet of the patient, is indicated for suspected or proven Lassa fever, CCHF, or filovirus infections. Respiratory protection should be upgraded to airborne isolation, including the use of a fit-tested HEPA filtered respirator, a battery powered air purifying respirator, or a positive pressure supplied air respirator, if patients with the above conditions have prominent cough, vomiting, diarrhea, or hemorrhage. Decontamination is accomplished with hypochlorite or phenolic disinfectants.

    What are viral hemorrhagic fevers?
    Viral hemorrhagic fevers (VHF) is a term that refers to a group of illnesses caused by several distinct families of viruses. While some types of hemorrhagic fever viruses cause illnesses that are relatively mild, many of these cause severe, life-threatening diseases with no known cures. Perhaps the most known of this group of diseases is the Ebola virus, made famous by the movie "Outbreak." Hemorrhagic fever viruses are caused by viruses of four distinct families:
    • arenaviruses filoviruses bunyviruses flaviviruses
    These viruses share the following common features:
    • The viruses are geographically restricted to the areas where the host species live.
      Their survival depends on an animal or insect host, called the natural reservoir.
      Humans are not the natural reservoir for any of these viruses; they only become infected when coming into contact with an infected host. However, with some of these viruses, humans can transmit the virus to one another after the accidental transmission from the host.
      They are all RNA viruses, covered in a fatty (lipid) coating.

    WHAT ARE VIRAL HEMORRHAGIC FEVERS?
    Viral hemorrhagic fevers (VHFs) refer to a group of illnesses that are caused by several distinct families of viruses. In general, the term "viral hemorrhagic fever" is used to describe a severe multisystem syndrome (multisystem in that multiple organ systems in the body are affected). Characteristically, the overall vascular system is damaged, and the body's ability to regulate itself is impaired. These symptoms are often accompanied by hemorrhage (bleeding); however, the bleeding is rarely life-threatening. While some types of hemorrhagic fever viruses can cause relatively mild illnesses, many of these viruses cause severe, life-threatening disease.
    VHFs are caused by viruses of four distinct families: arenaviruses, filoviruses, bunyaviruses, and flaviviruses. Each of these families share a number of features:
