Hemolytic-uremic syndrome (HUS) and thrombotic thrombocytopenic purpura (TTP) are microangiopathic disorders — that is, they are characterized by abnormalities (chiefly blood clots) that occur within the small blood vessels of the body. Both HUS and TTP are distinguished by blood clots within the capillaries and arterioles of many organs. Such clotting is associated with hemolytic anemia (low red blood cell count due to cell rupture) and low numbers of platelets (cell-like bodies responsible for blood coagulation). Hemolytic anemia results from the fragmentation of the red blood cells when they pass through areas of thrombi (masses or clots) or turbulence in the circulation. Such forces shear the cells in half, producing cell remnants that appear as helmets and other odd shapes when viewed under a microscope. In fact, the diagnosis of HUS-TTP is aided by microscopic examination of the blood for sheared red blood cells.

HUS
  • Hemolytic-uremic syndrome is a disease in which the number of platelets suddenly decreases, and the body's red blood cells are destroyed. Kidney failure occurs simultaneously. This is a rare syndrome that can occur after bacterial infection or develop from other diseases. Some research suggests a genetic basis to the disease, but the exact cause is unknown. It occurs most often in infants, small children, and pregnant women, but can occur in older children, adults, and women who aren't pregnant.

Hemolytic-uremic syndrome (HUS)
Definition: HUS is a disorder marked by kidney failure hemolytic anemia thrombocytopenia (platelet deficiency), coagulation defects, and variable nervous system signs.
Alternative Names: HUS
Causes, incidence, and risk factors: This disorder is most common in children. It frequently occurs after a gastrointestinal (enteric) infection, often one caused by a specific E. coli bacteria ( Escherichia coli O157:H7). It has also been associated with other enteric infections including Shigella and Salmonella and some non-enteric infections.
HUS, once relatively rare, is increasing in children. It is the most common cause of acute kidney failure in children. Several large outbreaks in 1992 and 1993 were attributed to undercooked hamburger contaminated with E. coli

hemolytic-uremic syndrome
Hemolytic-uremic syndrome is the most common cause of acute kidney failure in young children. It is a systemic disease, involving a triad of symptoms: It most frequently follows an episode of gastroenteritis caused by an toxin-producing strain of Escherichia coli (E.coli), type O157:H7. It has been less often associated with other infectious agents such as the bacteria Shigella, Salmonella, Campylobacter, and Streptococcus pneumoniae, as well as some viral infections. The syndrome is most common in children younger than 4 years old. It is usually preceded by gastroenteritis with the usual fever, vomiting, abdominal pain, and diarrhea. The diarrhea is often bloody, which in itself is always a worrisome sign. Less commonly the illness is preceded by an upper respiratory tract infection - a cold. About 5-10 days later the initial illness is followed by the sudden onset of paleness, irritability, weakness, lethargy, and decreased urination. Examination at that time may also show signs of dehydration, swelling (edema), petechiae,

Hemolytic-uremic syndrome is a serious blood disorder characterized by a sudden decrease in the number of platelets, the destruction of red blood cells and kidney failure . Hemolytic-uremic syndrome is sometimes caused by a bacterial infection, immunosuppressive drugs or anticancer medications. Usually the cause is unknown. Symptoms of hemolytic-uremic syndrome are similar to a bleeding disorder, and kidney damage is usually severe. The disorder is common in infants, young children and women who are pregnant or have recently given birth. Hemolytic-uremic syndrome has been linked to E. coli

Antibiotics increase risk of hemolytic-uremic syndrome
In a study released early by the New England Journal of Medicine because of its potential therapeutic implications, UW researchers found that antibiotic treatment of children with E. coli O157:H7 infection increases the risk of hemolytic-uremic syndrome. Researchers at Children's Hospital and Regional Medical Center led by Craig S. Wong, senior fellow in pediatrics, and Phillip I. Tarr, professor of pediatrics, conducted a cohort study of 71 children under the age of 10 . Hemolytic-uremic syndrome developed in 5 of the 9 children given antibiotics, compared to 5 of the 62 children who were not given antibiotics. According to the study, factors significantly associated with hemolytic-uremic syndrome were a higher initial white-cell count, stool culture soon after the onset of diarrhea (perhaps indicating a more fulminant course of illness that prompted an early evaluation), and treatment with antibiotics. Hemolytic-uremic syndrome can sometimes occur after a gastrointestinal tract infection. It is marked by destruction of red blood cells, kidney failure and damage to the lining of blood vessel walls.

Signs and symptoms
The classic case of HUS occurs after bloody diarrhea caused by , a strain of E. coli that expresses verotoxin (also called Shiga toxin ). The toxin enters the bloodstream, attaches to renal endothelium and initiates an inflammatory reaction leading to acute renal failure (ARF) and diffuse intravascular coagulation (DIC). The
