Hematuria (Blood in Urine)
What is hematuria?
Hematuria means blood in the urine. Microscopic hematuria means that the blood is seen only when the urine is examined under a microscope. Gross hematuria means that there is enough blood in the urine to be seen without a microscope.
How does it occur?
Hematuria is a sign that something is causing bleeding in the urinary tract. The urinary tract includes the kidneys, the ureters (tubes that carry urine from the kidneys to the bladder), the bladder, and the urethra (tube that carries urine from the bladder out of the body). Some common causes of blood in the urine are:
  • urinary tract (bladder) infection strenuous exercise kidney disease a stone in your bladder or in a kidney an inherited disease such as sickle cell anemia or systemic lupus erythematosus medicines such as blood thinners, including heparin (Calciparine, Liquaemin), warfarin (Coumadin), or aspirin-type medicines; penicillins; sulfa-containing drugs; cyclophosphamide (Cytoxan)

Hematuria
by Barb Durso, MD Hematuria refers to blood in the urine . Often, if a child complains of painful urination or abdominal pain, his doctor may want to test the child's urine using a paper test strip. This is known as a dipstick urinalysis Related Topics urinary tract infections wilm's tumor One of the tests that can be performed with these test strips is a test for blood. Sometimes the blood is obvious like a clot and this is called gross hematuria . It is important to always test the urine even when it appears red because not all obviously red urine has blood in it. For example, certain foods like beets and certain medicines like metronidazole (an antibiotic) can make the urine turn red.

      • Microscopic hematuria is a degree of bleeding so low that the blood can not be seen with the naked eye. Possible causes include kidney or bladder disease or damage. This condition can be detected through laboratory tests.

What is the significance of microscopic hematuria?
    Frederick R. Jelovsek MD, MS
"What does blood in the urine from the urinalysis indicate? You can't really see in urine, but it is detected through the urine test. Background is a 42 year old, diabetic, who gets very sick when ill because of diabetes" J.E. Blood in the urine that cannot be seen with the naked eye and is only detected through urine dip strip chemical tests or microscopic analysis is called microscopic hematuria. The most common cause of microscopic hematuria is simply unknown and not a cause for concern if major problems have been ruled out. It is estimated that possibly up to 20% of adult men and postmenopausal women have microscopic hematuria ( ). The cause for the microscopic bleeding may only be found in as few as 30-40% of patients undergoing a full urologic work up ( Microscopic hematuria is defined as more than 3 red blood cells per high power microscopic field in a centrifuged urine specimen ( ). Routine urinalysis to check for hematuria in an asymptomatic patient during an annual routine exam is not recommended. This is because of the very high incidence of false positives for serious problems and because the work up includes several expensive and slightly risky procedures (

    What is hematuria?
Hematuria means that red blood cells are in the urine. Urine does not normally contain red blood cells because the filters in the kidney prevent blood from entering the urine. In hematuria, the filters or other parts of the urinary tract allow blood to leak into the urine. Microscopic hematuria means that the blood is detected only with a microscope, while gross hematuria means the urine appears red or the color of tea or cola. Microscopic hematuria in an otherwise healthy child does not usually need to be investigated unless it is present in at least three urine tests over several months. However, if the child has high blood pressure, chronic kidney disease or protein in the urine, then hematuria should be investigated promptly What causes hematuria?

    What is microscopic hematuria?
"Microscopic" means something is so small that it can only be seen through a special tool called a microscope. "Hematuria" means blood in the urine. So if you have microscopic hematuria, you have red blood cells in your urine, but you can't see the blood when you urinate. Return to top
What are some common causes of microscopic hematuria?
Here are some common causes of blood in the urine:
    • Urinary tract (bladder) infection
    • Swelling in the filtering system of the kidneys (this is called "glomerulonephritis")
    • A stone in your bladder or in a kidney
    • A disease that runs in families, like cystic kidney disease
    • Some medicines
    • A blood disease, like sickle cell anemia

    Hematuria from bicycling or running?
    Q:
What is the mechanism of hematuria resulting from bicycling or running? When should other causes be suspected and the situation further evaluated?
Internist, Maryland A: Both contact and noncontact sports can result in gross or microscopic hematuria. When blood appears in the urine after strenuous activity, it is usually benign. Its occurrence depends on the type and duration of exercise and the person's state of hydration. When hematuria follows blunt physical contact, renal trauma is usually the cause. Hematuria after noncontact strenuous exercise may originate in the bladder from microcontusions of its mucosa, which can occur with repetitive up-and-down motion and friction on the mucosa of a partially filled bladder. These contusions rapidly resolve, and urinalysis is negative for the presence of blood in 48 to 72 hours. However, hematuria following mildly strenuous exercise, such as walking or swimming, may be the result of anticoagulation therapy or a significant genitourinary condition, such as a congenital abnormality, sickle cell trait, or a neoplasm, which tends to bleed with minimal trauma. Exercise-induced hematuria is a diagnosis of exclusion. The hematuria must be temporally related to physical exercise and must remit within several days of the exercise.
